[Inhalation allergy to fly larva. A clinical case].

The authors describe a clinical case of rhinitis and asthma in an angler after exposure to antigenic material released from larvae of fly, commonly used by anglers as bait. The common names of these larvae are "blue bottle", "green bottle" and "grey bottle". The patient was usually in good health, free from allergic diseases and free from asthmatic attacks when far from exposure to the baits. Spirometric evaluation revealed a fall of 21% in VEMS after challenge with antigen. Skin test (prick) with an extract obtained from the baits and RAST were strongly positive. By avoidance of exposure to antigen obtained changing type of bait, the asthmatic attacks completely disappeared. The authors discuss the clinical, diagnostic and preventive aspects of this allergic syndrome, and deal with the taxonomy of the insect.

Cow's milk hypersensitivity in an elderly woman: clinical and immunologic findings.

We report a case of allergy to cow's milk diagnosed in a 79-year-old woman, who had the habit of drinking large amounts of milk and other dairy products for several years. She had been an asthmatic since the age of 40. At age 65, she noticed a correlation between cheese and milk intake and onset of asthma and two episodes of shock, and she has been avoiding milk for the past 15 years. Positive skin prick tests for cow's milk proteins, grass pollens, and house dust mite were observed. Serum IgE levels were above 3,000 U/mL, with high molecular weight IgE detected by gel chromatography. RAST results confirmed the presence of sensitizations detected by skin prick tests. The patient never suffered from atopic eczema. This unusual case of food allergy with onset after the age of 40 may indicate that prolonged exposure to food antigens in predisposed individuals may lead to the development of allergy even in adult age, lasting for decades after partial elimination of the allergen from the diet.

[Effects of oral glucose load on blood lipids and on platelet function].

Oral glucose tolerance test (OGTT) has been performed on 12 healthy men. Plasma glucose, cholesterol, triglycerides, total lipids, NEFA, and number, adhesiveness and aggregating of platelets have been measured before and during OGTT (at 30, 60, 90, 120, 150 mins). Results demonstrate that OGTT decreases during the 1st hour, increases during the 2nd hour, and brings again to the initial value the platelet aggregation during the 3rd hour. The biphasic trend of platelet aggregation, in comparison with the monophasic OGTT curve, allows us to hypothesize some relationships between platelet aggregation and not only blood glucose concentration but also the activity of some hormones controlling both blood sugar level and platelet function.

IgE and IgG4 levels in allergic patients during immunotherapy.

The IgG4 is the predominant antibody response in patients receiving chronic exposure to high doses of antigen, and this seems to be true for immunotherapy as well. The duration and the dose of immunotherapy (IT) seems to increase specific IgG4 levels, but this increase does not seem to be related to the clinical response, seasonal exposure, duration of disease, and IgE antibody levels. The measurement of IgE and IgG4 antibodies to allergens did not predict the clinical effect of immunotherapy in our study. However, it indicated that the patients were continuing to receive the allergenic extracts and that the extract used in vivo, although produced by a different manufacturer, was closely resembling the one used for coating plastic wells in the FAST assay. The increase of antibody titres demonstrate that long term monitoring of IT administration may be accomplished by these in vitro tests, although individual patients may exhibit different responses in time but similar clinical results. Longitudinal studies on a cohort of atopic subjects may help define more precisely doses and timings required to achieve useful indications on both compliance with IT injections and predictivity of its outcome.
