[Possibilities of therapeutic photokeratotomy with the excimer laser in treatment of Schnyder crystalline corneal dystrophy].

PURPOSE: To evaluate the efficacy of excimer laser phototherapeutic keratectomy (PTK) in treating the Schnyder cristalline corneal dystrophy. METHODS: We examined the members of three generations of a family affected by Schnyder crystalline dystrophy. Two patients with disabling visual acuity were treated with PTK. RESULTS: The best corrected visual acuity (BCVA) of the patient with the cristalline form of the dystrophy progressed from 20/70 to 20/25. The BCVA of the other patient affected by the sine crystals form of the Schnyder dystrophy progressed from 20/70 to 20/30. CONCLUSIONS: Excimer laser photokeratectomy is a valuable tool in treating the Schnyder cristalline form of corneal dystrophy.

[Adjuvant treatment with mitomycin eyedrops after pterygium excision: Ticino experience].

AIM OF THE STUDY: Prospective pilot-study in the canton of Ticino to evaluate the prevention of recurrence of pterygium after excision and mitomycin drops. METHODS: Mitomycin 0.02% drops have been used from 6 to 10 days after 24 cases of corneoscleral excision of the pterygium. RESULTS: After a mean observation time of 13 months, we did not see any recurrence. The vision was improved in 8 eyes over 21, was unchanged in 15 eyes and deteriorated in 1 eye due to unrelated causes. In two cases we observed a thinning of 0.3 mm deep over 1 mm2 of the sclera. CONCLUSIONS: The rate of recurrence after excision of the pterygium and followed by instillation of mitomycin drops is lower than the other described treatments. However, complications described in literature, yet not observed in our own study, underline the necessity of a prolonged perspective study.

Topical timolol maleate might adversely affect serum lipoproteins.

Conflicting observations have been reported about the effects of topically administered timolol maleate on serum lipoproteins. We therefore considered this issue in a series of eight glaucoma patients receiving timolol maleate. Cholesterol and triglycerides were measured in plasma and in low-density lipoproteins (LDL), and high-density lipoproteins (HDL), both before and following three months of treatment. Following the treatment, the mean atherogenic index was increased from 2.72 to 3.38 (p = 0.012). This suggests that the atherogenic index should be determined before and during timolol maleate treatment in high-risk cardiovascular patients.

Threshold perimetry in tilted disc syndrome.

Twelve consecutive patients (17 eyes) with tilted disc syndrome underwent quantified visual field examination with the G1 Octopus program (Interzeag AG, Schlieren, Switzerland). Visual fields were abnormal in all cases. Although field loss was more pronounced in the superotemporal quadrant (P less than .001), it also involved the other three quadrants, thus demonstrating that optic nerve hypoplasia in tilted disc syndrome is apparently not entirely sectorial. Linear regression analysis showed that an increase in mean defect correlated with increase in myopic refractive error (P less than .05). However, the increase in myopic refractive error was found to be related to a decrease in corrected loss variance (P less than .05), indicating that in tilted discs with higher myopia, field loss is more homogeneous. With this series of patients, short-term fluctuation was within normal ranges in all visual fields, showing that this may be an additional clue for differential diagnosis from acquired disorders.

[Preliminary results of the pterygium excision technic with conjunctival autograft].

14 eyes with a pterygium, including one eye from which a pterygium had previously been removed by a simple excision, underwent a conjunctival autograft. During an average follow-up of 13 months, we observed a recurrence in 5 eyes (35%). The visual acuity stayed unchanged in 10 eyes, worsened in 1 eye and improved in 3 eyes. The corneal astigmatism measured with a Javal keratometer showed a modification in 12 eyes. This method should not be used as a standard primary surgery for pterygium in view of the high recurrence rate observed.

[Abnormalities of retinal function (light sense) following idiopathic central serous chorioretinopathy and diffuse retinal epitheliopathy].

Although central serous retinopathy is considered a "benign" condition, it is associated in healed forms with frequent and troublesome impairment of visual function. The abnormalities are even more severe after diffuse retinal pigment epitheliopathy. Since Snellen visual acuity testing is a relatively imprecise index of visual function, it does not enable the subtle visual disturbances experienced by these patients to be fully appreciated. The authors evaluated visual function disorders by static automated perimetry and contrast sensitivity in 30 patients with healed central serous retinopathy and in five patients with diffuse epitheliopathy at a cicatricial stage. The results of this study are examined and discussed.

[Lamellar corneal graft in the treatment of pterygium. A 10-year retrospective study of the recurrence and changes of astigmatism].

57 eyes with a pterygium, including 12 eyes (22%) from which a pterygium had previously been removed by a simple excision, underwent a corneoscleral lamellar excision of pterygium followed by a corneal lamellar keratoplasty. During a mean follow-up of 37 months, we observed a recurrence in 2 eyes (3.5%). The corneal astigmatism measured with a Javal keratometer showed a modification in 54 eyes (95%). Visual acuity improved in 4 (7%) eyes, was unchanged in 29 (51%) and deteriorated in 23 (40%) eyes. The benefits of the low recurrence rate after this surgical procedure are to be weighed against the risk of modification of the corneal astigmatism and the possible modification of the visual acuity.

[Evaluation of retinal function following central serous retinopathy and diffuse epithelial retinopathy].

The so called "benign" central serous retinopathy is associated in healed forms with frequent and troublesome visual function impairment. The abnormalities are still more severe after diffuse retinal pigment epitheliopathy. Snellen visual testing is relatively imprecise index of visual function, so it does not permit to appreciate the subtle visual disturbances experienced by these patients. We evaluated visual function disability with static automatized perimetry and contrast sensitivity in 30 patients with diffuse epitheliopathy in a cicatricial phase. Colour vision was also evaluated by 16 patients after central serous retinopathy. The results of this study are examined and discussed.
