[Plastic surgery application in artistic studies of breast cosmetic].

As, quest for beauty has always been an aim. There is a need to define the "ideal" beautifull breast with objective caracteristics, usable in plastic, reconstructive and aesthetic surgery. After, we have defined the necessary parameters usable in breast surgery. We realized an anthropomorphic study including 70 volunteer women and one molded model. Multiple pictures have been taken for each volunteer, that were evaluated and noted by five plastic surgeon, five doctors from other speciality, five women, five men. A twenty notation was used for this study. The main values for the ten women that obtained best notation (>or=14/20) were used in order to defined the ideal measures for surgery. Breast arrow and breast arrow over the trunck height were the most significant factors. According statistical studies, this fraction is about 0,387 (P=0,000969). The nice breast has a round shape and look like juvenile breast. Bra size, cup size, base, segments II and III, fraction II/III are not significant in our series of (non operated) natural breast. The ideal breast is in harmony and proportional to the trunck height, with the main fraction of breast arrow on the trunck height about 0,387 in our serie.

[Breast implants and quality process].

Quality processes have been compulsory for breast implants manufacturing since June 14th 1998. According the new article dated July 2005 the 11th (R 74-4), it also becomes compulsory for plastic surgeons to follow some strict quality processes in practising breast implants. A recent new quality approach was carried out over 302 patients who had breast implants. Herewith our primary conclusions: Some best practises have been defined as the benchmark. (What is supposed to be done or not to be done). A flow chart shows the interaction between the different processes required for a good quality system. Key indicators for monitoring quality levels are indicated: patients satisfaction survey, both reporting of problems and surgery recalls.

[Paranasal mucormycosis with involvement of the sphenoid sinus].

Mucormycosis with hemipansinusitis and left orbital involvement was observed in a 66-year-old diabetic woman. Diagnosis was confirmed by mycology and pathology examinations. The patient was treated surgically with wide exeresis of necrotic tissue and medically with amphotericin B and insulin. Cure was obtained after three months of treatment at the cost of retraction of the left nostril due to infectious lysis and resection of the nasal infrastructure. The sinus involvement was exceptional compared with reports in the literature. Mucormycosis is an uncommon condition caused by opportunistic infection in immunodepressed subjects. The causal agent is a fungus belonging to the Rhizopus, Absidia or Mucor genera. Prognosis is poor with overall mortality reaching 50%.

[Liposarcoma of the head].

Cephalic localization of liposarcoma is exceptional. We report three cases, in an 87-year-old patient with liposarcoma of the tongue, a 78-year-old patient with multiple recurrent liposarcoma of the orbit and a 42-year-old patient with ethmoido-orbital liposarcoma of metastasis origin. These three cases and a review of the literature were the basis of a summary of the incidence, the anatomic classification and therapeutic options for this type of liposarcomas.

[Gastro-omental free flap: technique, indications and results in buccopharyngeal reconstruction after cancer excision. Apropos of 7 cases].

The gastro-omental flap is centered around the midportion of the greater curve of the stomach, with the attached omentum. The flap is based on the right gastro-epiploïc pedicle. The size of the mucosal patch is adaptable. This flap offers several practical advantages: great plasticity (including the omentum), large length and size of the pedicle, mucus secretion which may improve xerostomia, and less morbidity. Disadvantages include the propensity for excessive mucus production (needing tracheotomy), the requirement for an abdominal operation, and the possibility of peptic ulceration (no case reported in the literature). Seven patients have been treated, with a good result. This flap is suitable for repair of complex soft-tissue defects after extensive cancer surgery.

[Surgery of a voluminous palatal Kaposi's sarcoma developing in AIDS].

The authors report of a case of voluminous palate Kaposi's Sarcoma in a patient with A.I.D.S. Oral Kaposi's Sarcoma occurs in a large percentage of patients with A.I.D.S. (30 to 50%). The therapeutic of this voluminous Kaposi's Sarcoma is not easy. Surgery is the best therapeutic it can bring, as in this case, functional and durable effect.
