[The epidemiological aspects and prevalence of markers in those with viral hepatitis C in some population categories in 1994-1997 in Iaşi County].

The results of the epidemiological, clinical and laboratory investigations, carried out in the 1994-1997 interval in the Iaşi district, on various populational groups, to indicate the prevalence of the antibodies anti-HVC possessors are presented. In 3611 patients with clinical-epidemiological HCV diagnosis, it was recorded a prevalence of the anti-HCV antibodies (AcHCV) presence of 22.7%. Posttransfusional hepatitis, epidemiologically diagnosed in 205 hospitalised patients was attributed to HCV in 11.6% of the cases, and in 94 patients with neurological diseases, AcHCV were identified in 9.6%. The presence of AcHCV at 825 persons in medico-sanitary staff, from risk domains, has recorded a prevalence of 8.0%. In the January 1997-September 1997 interval, the investigation by IIIrd generation ELISA of 51,149 blood samples of donors from various areas indicated the presence of AcHCV with an interval average of 2.6%.

[Clinical imaging correlations in lacunar infarcts].

PURPOSE: This paper is a study concerning the clinical-imagistic correlations in lacunar infarcts causes and associated lesions in these ones. We made an attempt to find out and emphasize the risk factors, causes and pathogenic mechanisms involved in lacunar syndromes. MATERIALS AND METHODS: We studied 138 patients with lacunar infarcts diagnosed by clinical examination and computer tomography. The investigation of risk factors evidenced arterial hypertension in 75 cases (54%). The clinical features of lacunar syndromes were: 1) pure motor stroke 41c; 2) pure sensory stroke 8c; 3) ataxic hemiparesis 9c; 4) dysarthria-clumsy hand 1c; 5) sensorimotor stroke 21c; 6) Landry syndrome 1c. RESULTS: We correlated the clinical manifestations and complementary investigations what enabled us to charge in determining of lacunar infarcts: focal microangiopathy-small artery disease (75c), cardio-embolic mechanism (12c), arterio-arteriolar embolic mechanism (15c). The risk factors and pathogenic mechanisms need more adequate research in order to find appropriate prevention and medical attendance.

Neurofibromatosis. Nosological considerations.

The neurofibromatosis (NF) represent a set of conditions having different clinical manifestations, prognosis and inheritance. It has been presented--on clinical grounds--seven types of NF, but for only two of these National Institute of Health Consensus Development Conference (NIHCDC) advent a set of diagnostic criteria. The genes responsible for NF1 and NF2 were mapped to the long arm of chromosome 17 (17q11.2) and respectively 22 (22q11.2), and their protein product (neurofibromin and respectively merlin or schwanomin) was identified. Recent studies are proved that NF1 and NF2 genes act as a tumour suppressor gene. Up to now, only a limited number of mutations in these genes have been characterized but even in these cases the genotype/fenotype correlation has not provided enough information to allow speculation on the etiologic role NF1 or NF2 mutations might play in the variant forms of NF. Further studies are required to elucidate the genes functions and mutation spectrum. This should provide a framework for the molecular classification and diagnosis and the development of new therapy for NF.

[Neurogenetics in Parkinson's disease].

The pathogenesis of Parkinson's disease, a progressive degenerative disorder of the central nervous system, remains obscure. 20% of cases are familial. The disorder may be due to a combination of genetic and environmental factors. Some monogenic forms of Parkinson's disease have been described. Many genetic studies explore whether a candidate gene may confer an increased risk for this disease. The search for a genetic susceptibility may lead to establish a preventive therapy.

[Transcranial Doppler exploration of the brain stem vascularization in neurological pathology].

The hemodynamic alterations in the vertebrobasilar system arteries of 1051 patients are discussed. The aspects revealed by transcranial Doppler are classified as obstructions, stenoses and atherosclerotic type lesions. Their prevalence compared with the clinical diagnosis or through other investigation methods is commented upon, an etiological classification of such alterations in hemodynamics in the basilar and vertebral arteries being attempted. Multiple defect theory seems to be, in such cases as well, the clinical expression accounting for the pathogenic phenomena and response to drug tests.
