Cardiomyopathy in cystic fibrosis: lymphoedema of the heart with focal myocardial fibrosis.

Cardiomyopathy in cystic fibrosis (CF) is an unusual heart disease, mainly characterized by a multifocal fibrosis of the left ventricle. The disorder chiefly occurs in the age group of 1-2 years and leads to fatal cardiac failure. The causal pathogenesis of the disease has not been discovered up to now. In two cases of CF-associated cardiomyopathy we found an oedema (mainly lymphoedema) of the myo- and epicardium and a lymph stasis in lymph vessels and lymph nodes of the heart. Based on a comparative study using animal models we speculate that a) CF may be complicated by a disorder of cardiac lymph circulation, and b) chronic cardiac lymphoedema of the heart in CF can cause focal myocardial damage with fibrosis.

Lethal vasculitis of coronary arteries in a neonate and two infants: possible neonatal variant of the MLNS/IPN complex?

An unusual case of fatal myocardial infarction in a neonate due to coronary arteries is compared with two lethal cases of Mucocutaneous Lymph Node Syndrome and/or Infantile Periarteritis Nodosa (MLNS/IPN). The neonatal coronary arteritis was pathologically indistinguishable from typical forms of MLNS/IPN, which cannot be reliably separated and whose strange epidemiologic features as well as their etiology(-ies) remain to be clarified. It is suggested that further research of the etiology of MLNS/IPN should be aware of a possible neonatal variant of this syndrome.

[Early detection and determination of the incidence of side effects in the hospital by intensive (comprehensive) drug monitoring].

In the "Comprehensive Hospital Drug Monitoring Berne" (CHDMB) the frequency of adverse drug reactions can be either determined by counting individual patients with probable or proven reactions to a drug or by mathematic-statistical analysis of exposed and nonexposed patients with and without the reaction. These two methods are used to show the frequency of a maculopapular rash in connection with three antiinfectious drugs. The mathematic-statistical frequency of a maculopapular rash was 11.0% for aminopenicillins, 3.7% for other penicillins, and 3.4% for co-trimoxazole and sulfonamides compared to 1.2% in patients treated with other than the especially mentioned drug groups. Two new adverse reactions were detected by this system: arterial hypotension due to novaminesulfon-dipyrone (Novalgin) and drug fever to the antidepressive drug nomifensin (Alival). Also the frequency of some other, not unusual adverse reactions is demonstrated.

[Formation of chondroid tissue in the left ventricle and bundle of His in complex congenital cardiac abnormality (author's transl)].

The formation of chondroid tissue in the heart ventricles and in the region of the ventriculoatrial transition is regarded as a rare lesion of the human heart. We present the observation of a not well-known disorder of cardiac development connected with a cartilagineous transformation of endo- and myocardial tissue. Chondroid change was found in the innermost parts of the left ventricle in a heart with severe hypoplasia of left parts and aortic atresia. Cartilage-like tissue reached to the region of the bundle of His and to supraanular parts of the interatrial septum. The possible genesis of this change, that may be of clinical relevance, is discussed with regard to normal endo- and myocardial development.

[Blood pressure determination in children: normal values for a given age group].

Blood pressure was measured under standardized conditions in 151 first-grade schoolchildren. The values of this small sample were normally distributed and statistical analysis was therefore performed. The systolic blood pressure of boys and girls was 102.0 +/- 9 mm Hg and the diastolic blood pressure 69.5 +/- 7.3 mm Hg. Boys and girls were also analyzed separately. The data fit in very well those from the literature, and it is concluded that the values from the literature, especially those of the "Task force of blood pressure control", can be used with high probability for Berne and Switzerland as well. Additional findings: The skin thickness of all children and of girls correlates with blood pressure in the normal range. The family history with regard to risk factors is probably only of importance if precise questions are asked, since general inquiries about obesity, myocardial infarction, and hypertension did not yield useful results.

Age Factors↗

Pericarditis purulenta in children.

The case history of seven children aged 1 5/12 to 5 9/12 years with non tuberculous bacterial pericarditis, observed in the last 8 years at the University children's hospitals of Basle, Berne and Zurich is reported. The history showed febrile illness of 3--14 days duration, which led to an admission diagnosis of pneumonia, angina or pseudocroup. From the signs of heart failure and cardiomegaly on chest X-ray the differential diagnosis of myocardial disease or pericardial effusion was made. The ECG-changes were uncharacteristic, and a friction rub and pulsus paradoxus was encountered once only. The effusion diagnosis should preferably be substantiated by a non-invasive method (scintigram, echocardiogram) as diagnostic pericardiocentesis does often not allow to aspirate the thick pus through the needle. Diagnostic and therapeutic surgical pericardiotomy with consecutive drainage is therefore mandatory. Halothane should be avoided as an anesthetic for this procedure of hemodynamic reasons. With surgery and antibiotics the recovery rate in our series was 100%, and no pericardial constriction was observed on follow-up 1 to 8 years later.

[Analysis of two different surgical corrections of the coarctation of the aorta (follow up examination of 49 children) (author's transl)].

36 children operated upon with the diagnosis coarctation of the aorta have been controlled in a follow-up examination. We payed special attention to the surgical technique and brought the results in connection with the method of operation. One group had the stenosis corrected by resection and end-to-end-anastomosis, the second group with plastic operation, either by a longitudinal incision, resection of a diaphragm and transverse closure, or by improving the dilatation using a dacron patch. The results in both groups are satisfactory. Actually we prefer to carry out the resection with end-to-end anastomosis. Plastic correction remains reserved for selected cases. We believe that the high mortality rate in infants can be reduced by operation in hypothermy and by removing the stenosis as completely as possible.

[Proceedings: The evaluation of the mixed venous O2-saturation in the oxymetric determination of heart minute volume and shunt size].

An investigation has been conducted into three groups of patients without intracardiac shunt (A: 59 adults without anesthesia, B: 58 children in anesthesia, C: 53 children without anesthesia). Oxygen saturation in the pulmonary artery was plotted against saturation in both venae cavae or against saturation in only one of them. The resulting 9 equations can be used as formulas to calculate the saturation of mixed venous blood in patients with an intracardiac shunt. Furthermore, mixed venous saturation can be estimated from "subclavian catheter" blood samples in patients without shunt.

Arteriovenous Anastomosis↗

[Computation of mixed venous oxygen saturation in intracardial shunts and its dependence on age, anesthesia and premedication].

In patients with an intracardiac shunt, the oxygen saturation of the mixed venous blood cannot be measured, but it must be estimated from the oxygen saturation in the superior and in the inferior vena cava. We have evaluated the results of patients who had definitively no shunt and who had been catheterized for other reasons. Based on statistical principles we developed formulas to calculate the oxygen saturation of the mixed venous blood. We examined three group of patients: a) Children up to 11 years catheterized during general anaesthesia (58 patients) b) Children up to 11 years catheterized after premedication but without general anaesthesia (53 patients) c) Adults catheterized without general anaesthsia (59 patients) We found that the formulas from the patients who had been catheterized during general anaesthesia were quite different from those without anaesthesia. In the patients without an anaesthesia the superior vena cava was coupled with a much higher factor than the inferior vena cava; in patients with anaesthesia, the contribution of the inferior vena cava was stronger. The age of the patients had no significant influence on our formulas.
