[Reflections on the significance of detecting Campylobacter pylori in a biopsy of the stomach mucosa].

The inevitable confrontation with Campylobacter pyloridis implicates the surgeon to take into consideration this germ. For that reason we have examinated Campylobacter-positive patients with regard to histological changes of the gastric mucosa and to clinical symptoms. Apart from the fact, that there was no Campylobacter colonisation in the healthy gastric mucosa, there were not found specific features for the germ.

[Steroid hormone receptors in struma].

Oestradiol and progesterone receptors occur in different hormonally dependent tissues. The presence and number of steroid receptors in malignant breast tumours is well known. Endocrine treatment is effective. An attempt was made to quantify the oestradiol and progesterone receptors in human goitre. The behaviour of steroid receptors was studied in the cytosol of tissue samples from 33 surgically-treated patients. Neither oestradiol nor progesterone receptors were found in human goitre tissue.

[Results of studies in 165 patients following selective proximal vagotomy].

Selective proximal vagotomy was performed on 165 patients at the I. Surgical Clinic of Innsbruck University, between 1970 and 1984. Results of 90 patients were retrospectively evaluated. Lethality was 0.6 per cent, while the rate of recurrence amounted to 14.4 per cent and average acid reduction to 50 per cent. 65 per cent of the above 90 patients were almost without any postoperative complaint, when rated by the I. and II. degree according to Visick. Results were particularly favourable in cases of perforated ulcer, combined ulcer (Type II according to Johnson), hyperacid gastroduodenitis, and in cases of duodenal ulcer of women.

Antimutagenic effect of rat small intestine in vitro on 1,1,2,3-tetrachloropropene and 1,1,2,3,3-pentachloropropene.

The direct mutagenic activity of 1,1,2,3- tetrachloropropene and 1,1,2,3,3- pentachloropropene in Salmonella typhimurium was measured before and after incubation in the presence of intact segments of rat small intestine in vitro. The number of revertants in tester strain TA100 was reduced by about 95% when these chloropropenes were exposed to rat small intestine for 10-30 min immediately prior to determination of mutagenicity. The existence of an intestine-mediated detoxication reaction was postulated, and was supported by observations that incubation with the chloropropenes for 30 min caused a 48-68% depletion of intestinal glutathione in vitro. Although addition of glutathione to the chloropropenes reduced mutagenicity, the amount of tissue glutathione consumed during incubation of mutagen with intestinal segments is probably insufficient to account for the detoxication. Additional metabolic reactions and/or non-specific protein binding may occur in the intact intestine which contribute to the antimutagenic effect. These initial results support the existence of an effective detoxication mechanism by the small intestine which is likely to reduce the absorption of direct-acting mutagens and other electrophiles.

[Heart replacement (biventricular assisted circulation) with the ellipsoid heart (E-BVAD) (author's transl)].

At the present time mechanical assisted circulation is indicated in patients with cardiac failure after open heart operations. The results after clinical implantation of the left ventricular assist devices are not encouraging. However, most of these patients suffer from irreversible cardiac failure and no restitution is to be expected. In patients with postoperative cardiac failure, a distinction must be made between isolated left heart failure and total heart failure. In patients with total heart failure - for example with diffuse, marked coronary sclerosis - both left and right ventricle failure exists. In these patients, no left ventricular assist device per se can bring the desired effect, since due to the right heart failure syndrome, the left system does not work adequately. With the biventricular bypass it is possible to maintain complete circulation in cases of cardiac insufficiency. The cannulation concept is biventricular transatrial to the aorta or to the pulmonary artery. The technique is simple and can be carried out very quickly. In cases of clinical emergency this device can be recommended due to the satisfactory haemodynamic effects achieved and the small degree of traumatic haemolysis. It represents an easy and quick implantable system for total functional heart replacement.

[Acute implantation technique of the ellipsoid heart for clinical left ventricular assistance (author's transl)].

The results of the first clinical trials of left ventricular assist devices are not encouraging. The only indication at present is in patients with cardiac failure after cardiac operations. The acute type of ellipsoid heart described in this paper-displays a canulation technique where the inflow canula inserted in the left ventricular cavity via the left atrium and the outflow canula is directly connected to the aortic canula of the cardiopulmonary bypass. The haemodynamic response shows the efficacy of this system. The circulation can be maintained in severe cardiac failure, as well as in ventricular fibrillation and cardiac arrest. On the basis of data obtained in experiments on 7 female calves this system is considered worthy of clinical trial.

[Childrens opinions of hospitals (author's transl)].

57 children aged 5 to 13 years who were hospitalized for minor surgical interventions were asked to express any positive or negative criticism about their stay. One month later the parents were asked to tell us about any behavioural changes they noticed in their children. The surprisingly positive answers are discussed.
