[Variations in lipid metabolism in long-term monitoring of children treated for diabetes mellitus].

The authors investigated selected indicators of the lipid metabolism in 31 children with insulin-dependent diabetes for a period of 3 to 14 years. They divided the patients into two groups those with a glycosylated haemoglobin below or above 9 mumol/1 g haemoglobin. They compared the assembled results of the two groups and also with the results obtained in a control group. The total cholesterol was in both diabetic groups higher than in healthy subjects and was higher than the value which is considered from the aspect of the genesis and development of atherosclerosis as a risk value. Children with poorly compensated diabetes, i. e. with a glycosylated Hb level above 9 mumol had a higher cholesterol level as compared with well compensated diabetic children. The pre-beta fraction of lipoprotein increased in both groups of patients, however, more in those where the disease was not well compensated. There was a parallel decrease of the alpha fraction and the lipoprotein profile had a markedly atherogenic character. The apolipoprotein B concentration was in patients with well compensated diabetes lower, as compared with controls but in patients with poorly compensated diabetes after 4-5 years treatment is was significantly higher. Poor compensation of diabetes led after 4-5 years duration to a significant rise of the serum triglyceride level. As the blood lipid levels are influenced in a significantly way by diet, compensation of the disease and a low-fat diet are essential with regard to the results assembled in this investigation for prevention of ischaemic heart disease in diabetic subjects.(ABSTRACT TRUNCATED AT 250 WORDS)

[Personal experience with the care of children and adolescents at risk for ischemic heart disease].

Children and adolescents with hereditary disposition for ischemic heart disease form a substantial part of the clients of preventive cardiological surgeries for children. The authors present their results and experience they assembled by monitoring and supervising these subjects for a five-year period. During the initial examination they found in these subjects, as compared with controls, a more frequent prevalence of habits dangerous with regard to the genesis and development of ischaemic heart disease and deviations in the lipid metabolism with an atherogenic character. Arterial hypertension and protracted insulin secretion were also more frequent. The collaboration with their parents as regards adherence to a regime and therapeutic provisions was very satisfactory. After five years' supervision in children and adolescents with hereditary disposition for ischaemic heart disease the prevalence of obesity, arterial hypertension and abnormalities of blood lipid levels declined significantly. The above measures had the least effect on the impaired insulin secretion.

Longitudinal study of serum cholesterol, apolipoproteins and sex hormones during puberty.

In order to determine the age and sex dependency of some of the major risk factors for coronary heart disease during the period of sexual maturation a five-year longitudinal study was carried out on 105 boys and 133 girls living in Plzen, Czechoslovakia. Serum cholesterol was significantly elevated in 13-year-old girls (p less than 0.01), while no statistically significant sex-related differences in total and lipoprotein cholesterol and apolipoprotein A-I and apolipoprotein B levels were found in other age groups during the period of follow-up. In boys the total cholesterol showed a constant and statistically significant decrease (p less than 0.01) between the ages 11 to 13 years, but tended to rise during the last two years of study. In girls, serum cholesterol decreased significantly between the ages 11 to 15 years (p less than 0.05). A statistically significant decrease in pre-beta cholesterol was found in girls during puberty (p less than 0.05). The changes in beta lipoprotein cholesterol were similar to those for total cholesterol. The apolipoprotein A-I concentration decreased significantly between 11 and 15 years in boys (p less than 0.05), while it underwent no substantial change in girls. The apolipoprotein B level decreased permanently and significantly (p less than 0.05) during the 5-year period in girls. In boys, the concentration of this protein decreased between 11 and 14 years and increased during the 15th year when a value significantly higher than that for girls was attained (p less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical contribution of the examination of immunoglobulins and albumin in the feces in children (a group of 610 patients 1-18 years of age].

The authors submit their experience with the examination of immunoglobulins and albumin in the faeces of patients with different diseases of the gastrointestinal tract. The presence of antibodies IgA, sIgA, IgM and albumin were assessed in lavages from the rectum or supernatant of the faeces, using the qualitative method of double immunodifussion according to Ouchterlony. A normal finding is characterized by the presence of IgA, sIgA. For evaluation of the clinical importance and applicability of the examination method the authors draw attention to the importance of asessing the degree of positive or negative correlation between findings and the final diagnosis. The authors present three basic pictures; acute inflammation, chronic inflammation, non-inflammatory exsudation (exsudative enteropathy). They analyze the clinical application of different findings and their practical impact. They give an account of limiting factors in the evaluation of the presence of immunoglobulins and albumin in faeces.

Lecithin-cholesterol acyltransferase activity in children in the early neonatal period.

Changes in the rate of plasma cholesterol esterification occurring shortly after birth were followed by determining the activity of lecithin-cholesterol acyltransferase (EC 2.3.1.43) in newborn children, and at the age of 3 days and 3 months. The radioassay method using 'common' substrate was applied. The molar esterification rate (MER) in umbilical cord serum was about half of the values found in the sera of 3-day-old and 3-month-old children, or adult subjects. Because of a positive correlation between MER and the plasma concentration of unesterified cholesterol, the values of fractional esterification rate in children were in good agreement with those characteristic for healthy adults.

The pattern of major serum apolipoproteins during the early neonatal period.

Serum triglycerides, total cholesterol, lipoproteins and apolipoprotein A-I, B and C-III were investigated in 24 normal newborns at birth (umbilical cord blood) and again 4 days after birth (venous blood). Blood samples from 95 fasted normolipidemic male and female subjects aged between 20 to 60 years were also analysed. Immunochemical studies of serum from umbilical cord blood have shown that all investigated apolipoproteins were present although at lower levels than found in adults. The concentration of lipids, lipoproteins and apolipoprotein B and C-III in the serum from blood collected 4 days after birth did not practically differ from that of adult.
