Effective control of hypertension: a project of the Hungarian Society of Hypertension, baseline data.

The aim of the project is to assess the quality and improve the preventive and curative practices at the primary care level in Hungary. A total of 50 general practitionaires were selected on a voluntary basis in Budapest, Hungary, and from them, 30 were randomized to the intervention (I) group and 20 to the reference (R) group. The members in the I group have been trained for the official hypertension guideline and their everyday work is monitored. Those in the R group have only been monitored to measure the efficacy of the training. In all, 10% from the known hypertensive persons (N=10,799) and 5% of the remaining (nonhypertensive) patients (N=60,341) were selected randomly from the GP's computer files and invited for screening investigation performed by trained medical students. They measured the blood pressure of patients, assessed the cardiovascular risk status and the quality of education of patients by standardized questionnaires. In total, 4083 patients were invited, but only 39.2% attended the screening visit. The prevalence of undetected hypertension was 34.6%. This prevalence was significantly higher in the older (>60 years: 46.8%) than in the younger (<50 years: 20.8%, P<0.0001) age group and it was higher in men (41.5%) than in women (30.1%, P<0.001). The proportion of H patients on drug treatment was 85.3% and the frequency of patients under effective blood pressure control (eg<140/90 mmHg) was 27.8%. Counselling to patients for a healthier lifestyle (exercise, smoking, alcohol consumption, diet) was very rare. In conclusion, our data represent the primary care of Budapest and may not be relevant to the whole country. As a consequence of this study, education of primary care physicians and patients is a must for further improvement of hypertension care.

[Double supraventricular tachycardia and its surgical treatment].

A case history is presented of a patient in whom a left-free-wall accessory pathway was surgically ablated to treat symptomatic Wolff--Parkinson--White syndrome. Subsequently, AV-nodal reentrant tachycardia became manifest. The patients's AV-nodal reentrant tachycardia was interrupted by Cox's discrete cryosurgical procedure. As a result of analysis of these cases of dual substrates for reentrant supraventricular tachycardia, ECG and electrophysiological means for differential diagnosis has been developed. These are discussed herein.

Position of the mandibular canal.

Because of its numerous advantages, sagittal split osteotomy of the ramus has become the method of choice during the past 30 years. Operative injury to the vascular and nerve bundle within the mandibular canal represents a main risk factor. X-ray studies have been carried out on dry bone material to determine position of the canal. On the basis of the results, the variety of its position was ranked into 4 groups. The risk factor of, and the operative contraindication for, splitting technique in groups 3 and 4 have been emphasized.

Hemihypoplasia of the mandible combined with lateral open bite corrected by sandwich technique. A case report.

A hemihypoplasia of the body of the mandible and the ascending ramus on the left side, combined with a lateral open bite on the right side is described. Operative reconstruction on the left side was performed by sandwich technique. The correction of the lateral open bite involved the following: vertical ramus osteotomy on the right side, paramedian vertical body osteotomy on the left side, moving the body of the mandible upwards into the predetermined position.

The groove of the mandibular neck as a risk factor in vertical osteotomy of the ramus of the mandible.

Surgical damage of anatomical structures within the mandibular canal represents one of the risks of dysgnathia operations performed on the ramus of the mandible. The paper presents a review of the various measurements of anatomical landmarks available in the pertinent literature. The occurrence of the sulcus coli mandibulae, as well as the depth of the groove have been studied. In some cases the groove has to be considered as a surgical risk factor. On the basis of the examination, attention is called to the individual variations and to the necessity of a cautious surgical technique.

[Surgical and radiological anatomy of the mandibula (author's transl)].

One of the risks in dysgnathia-operations performed on the mandibula is the injury to the anatomic formations inside the mandibular channel. The paper deal with anatomic sites recommended in the literature for measurements. The occurrence of a sulcus colli mandibulae and the depth of the bone groove is investigated, and their part as an operative risk factor is presented. Attention is drawn consequently to individual variations and a careful surgical technique is recommended.
