[Traumatic vertebral artery dissection in an 8 year old boy].

Dissections of extracranial brain supplying arteries are a common cause of ischemic strokes in young patients. Accidents are often accountable for that. We report the case of an 8 year old boy with traumatic vertebral dissection after he was bumped while playing football. He developed an ischemia on both sides of the thalamus which was caused by a bilateral system of the vertebral artery. He showed motor eye and progressive neuropsychological deficits. The CT and MRI scan confirmed the diagnosis. The symptoms ameliorated under anticoagulation treatment with intravenous heparin. At the end of the hospital stay, neuropsychological deficits persisted. Doppler ultrasound showed recanalization of the vertebral artery. Further treatment consisted of anticoagulation with marcumar for 6 months.

[Anterior knee pain in children and adolescents. Diagnosis and conservative treatment].

Anterior knee pain is a common problem in childhood and adolescence with a prevalence of 4-30% in regional surveys. Although different etiologic theories are discussed in a large number of published studies, most cases are probably attributed to lower extremity malalignment, muscular imbalance, or physical overactivity (or even a combination of all three). Thorough clinical investigation should result in a definition of relevant static or functional problems as underlying causes of the disease. Radiographic analysis is mainly performed to exclude severe pathology (i.e., malignancies), as the correlation between described morphometric variables and clinical problems is weak. Prior to therapeutic recommendations, the benign natural course of the disease should be discussed with patients and their parents. In a large number of published studies with different conservative treatment protocols, positive results for many regimens are reported. The size, however, is not always large enough to prove superior outcome when compared to the natural course of the disease. There is also no evidence to support the superiority of one physiotherapy intervention over others.

r-Process in Neutron Star Mergers.

The production site of the neutron-rich heavy elements that are formed by rapid neutron capture (the r-process) is still unknown despite intensive research. Here we show detailed studies of a scenario that has been proposed earlier by Lattimer & Schramm, Symbalisty & Schramm, Eichler et al., and Davies et al., namely the merger of two neutron stars. The results of hydrodynamic and full network calculations are combined in order to investigate the relevance of this scenario for r-process nucleosynthesis. Sufficient material is ejected to explain the amount of r-process nuclei in the Galaxy by decompression of neutron star material. Provided that the ejecta consist of matter with a proton-to-nucleon ratio of Ye approximately 0.1, the calculated abundances fit the observed solar r-pattern excellently for nuclei that include and are heavier than the A approximately 130 peak.

[Osteoinduction with the dog tibial defect model].

The objectives of this study were to investigate the osteoinductive effect of demineralized bone matrix powder and of cancellous bone in a comparative experiment in a standardized dog defect model. 16 adult shepard dogs were divided into two test subgroups. One group underwent application of demineralized bone matrix (DBM) into the defect. The second group had their defects treated with autogenous cancellous bone (ACG) only. The progress of bone formation was observed for twelve weeks. The excised tibiae were examined by correlating serial X-rays with histologic sections stained with Giemsa and fluorescent labels. In this experiment no osteoinductive effect of demineralized bone powder could be demonstrated. The amount and localisation of bone regeneration in the matrix group seemed to be spontaneous, corresponding to the radiological and histological parameters. In the group of cancellous bone grafts, the defect (length 30 mm) was bridged by incorporation of the graft and graft mediated bone formation within twelve weeks. Disturbing factors such as instability, infection, insufficient vascularity, and cellular immune response to the graft were excluded in this model. The failure of the osteoinductive implants in our experiment compared with the good results obtained in rodents limits their value also in clinical situations. As long as there is no explanation for this failure, this technique should not be used in a clinical situation.

[Long-term results of the treatment of juvenile bone cysts].

38 children between 5 and 14 years of age with juvenile bone cysts were followed up to a maximum of 20 years. In two-thirds of the cases the diagnosis was made on the occasion of a fracture in the cyst region. One child received conservative treatment, 37 children were operated on. The roentgenological diagnosis of all children operated on was confirmed histologically. The operative procedure consisted in milling out the cysts and filling the cavities with autogenous, allogenous or xenogenous spongiosa. Because of the high recurrence rate (10-40% in the literature and 19% from our own experience) we suggest a modified surgical procedure assuring complete recovery. Subtotal diaphysectomy with interposition of a corticospongious splinter and allogenous spongiosa using a stable osteosynthesis in the case of large primary cysts in metadiaphyseal position is discussed.

[Relationships between interruption abortion, and premature birth and low birth weight (author's transl)].

Polydimensional retrospective studies were conducted into correlations between interruption of pregnancy, abortion, and premature birth of infants weighing less than 2,501 g, on the one hand, and the general phenomenon of low birth weight, on the other. Reference was made to parity. The studies were undertaken over a period between 1969 and 1977. The birth total investigated was 17,076, stillbirths and twins excluded.-The impact of all factors involved as well as the statistical significance were tested and verified by calculation of information measure 2-I, followed by an approximative chi 2-test. The results suggested that the parameters studied were of somewhat staggered impact upon low birth weight. Premature birth, interruption of pregnancy, and abortion was the order of impact at all parity levels studied.

[Polydimensional studies into frequency of "infants of low birth weight", with reference to age, parity, and occupation (author's transl)].

Retrospective polydimensional studies were conducted into correlations between age, parity, and occupation (social status) of gravidae, on the one hand, and deliveries of "infants of low birth weight", on the other. The reference group amounted to 16.729 newborns, twins and stillbirth excluded, with 5.85 per cent of those newborns with low birth weights up to 2,500 g. Included in the above figure are all newborns delivered in the Regional Hospital of Schwerin, between 1969 and 1977.--Any single factor quoted, age, parity, and occupation, was found to affect birth weight, effects being by the following order: parity, age, occupation.

[Multidimensional investigations to elucidate relationships between case histories of interruption of pregnancy and premature deliveries and low birth weight (author's transl)].

Multidimensional retrospective studies were conducted into possible correlations between case histories of induced abortion and premature deliveries, on the one hand, and deliveries of "children with low birth weight", on the other, with reference being made to the ordinal number of birth. Included were 13,287 single newborns, after mothers with twins, stillbirths, and abortion records had been excluded. "Children with low birth weight" up to 2,500 g accounted for 5.4 per cent of these newborns. The investigation was complete, in that all newborns were examined in the obstetric department of the Regional Hospital of Schwerin, between 1969 and 1977. Records of premature delivery and induced abortion were found to play a great epidemiological role in the context of "children with low birth weight" up to 2,500 g, but the general trend for such children for the period under review had remained unaffected by the Free Medical Abortion Act of 1972. Nevertheless, mothers with previous abortions during the period studied contributed to a moderate rise of the total number of "newborns with low birth weight".

[The induction of new bone formation after transplantation (author's transl)].

Bone induction by implantation of decalcified bone is described by many authors. This statement was proffed by implantation of decalcified pig bone in the abdominal wall muscle of rats. An enchondral osteogenesis was found following the implantation in a time intervall of 12 weeks according to the results of many other authors.

[Relationship between employment and the birth of low-birthweight infants].

In the period from 1969 to 1976 there born 14.368 living single infants with known duration of pregnancy in the gynecological clinic of the district hospital of Schwerin.--By means of this material was proved, if there is a relation between professional activity and the birth of "infants of low birth weight".--For some professional groups such a relation could be demonstrated.
