[Efficacy and safety of telithromycin in the treatment of acute exacerbation of chronic obstructive pulmonary disease].

UNLABELLED: The aim of this study was to evaluate the clinical efficacy of telithromycin administered for 5 days at a dosage of 800 mg/day, in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) presenting with at least 2 of Anthonisen's criteria including the increase of purulence. METHODOLOGY: During this multicenter (211 private lung specialists), prospective, non-comparative, open-labeled French study, 365 patients were included between April 2002 and March 2003. Clinical efficacy was assessed on D12-D19 by the rate of clinical success as defined by recovery or clinical improvement (main endpoint) according to the number of exacerbation episodes during the previous year. RESULTS: On D12-D19 clinical success rate in the per protocol global population was 88.0% and respectively 87.9% in patients with or=4 episodes in the previous year. These success rates were similar to those in the intent-to-treat population. Safety, assessed on 359 patients, was satisfactory, with mainly digestive disorders related to the treatment in 3.9% of the patients. No treatment-related serious adverse events were observed. CONCLUSION: This study, conducted among private practitioners in France according to COPD classification as defined by official recommendations, validates the results obtained in previous studies. Our results confirm the place attributed to telithromycin in the treatment of patients presenting with AECOPD without chronic respiratory failure, according to ongoing official recommendations.

Serum levels of antibody to toxoid during tetanus and after specific immunization of patients with tetanus.

Levels of serum antibody to toxoid in patients with tetanus who were immunized with toxoid differed from those in immunized healthy persons in two respects. (1) Base-line levels preceding immunization were higher among patients with tetanus than among young or elderly healthy person, even though the patients with tetanus were evaluated before specific serotherapy. This observation suggests the occurrence of immunostimulation, whether specific or not, by disease during the incubation period. (2) No significant increase in levels of antibody to toxoid was detected among patients with tetanus from the first to the last evaluable serum sample, whereas a significant increase was observed among healthy persons. Specific immunosuppression induced by tetanus toxoid or serotherapy could account for this ineffectiveness of immunization among patients with tetanus, although nonspecific immunosuppression induced by prolonged and severe illness cannot be excluded. Among healthy persons, active immunization is less effective in elderly persons than in young persons [2]. However, age cannot, by itself, account for the type of antibody production observed in patients with tetanus.

[Severe pneumococcal infections of adults. 100 cases collected in three years (Claude Bernard Hospital, Paris) (author's transl)].

After retrospective in intensive care unit of 100 severe cases of pneumococcal infection in three years, the authors analyse the circumstances of onset (occurring in autumn of winter), the general status (without evident high risk population), the symptomatology (84% of meningitis, 30% of pneumonia), the origin of infection (essentialy otitis media in meningitis). The case fatality rate is 39%. Frequency of multiple visceral localisations and of bacteremias is emphasized.

In vivo measurements of oxyhaemoglobin saturation by a fiberoptic catheter. The effects of variations in pH and haematocrit.

The development of plastic fiberoptic catheters, cheaper and less fragile than glass ones, has enabled the more widespread use of oxyhaemoglobin saturation (SO2) monitoring. They allow direct determinations of SO2 to be made, using reflection spectrophotometry. The purpose of the present work was to evaluate the accuracy of SO2 measurements as provided by these catheters. The studies were performed in dogs, with large variations of SO2, acid-base balance and haematocrit levels.
