Antiarrhythmic properties of benzyl-triamterene derivatives in the coronary artery ligated and reperfused rat.

Triamterene (CAS 396-01-0) and a series of benzyl-triamterene derivatives were evaluated for their antiarrhythmic properties in the coronary artery ligated and reperfused (CAL-R) rat. The effects were compared with the antiarrhythmic activity of the potassium sparing diuretic amiloride and drugs out of the class-I (lidocaine) and class-III (amiodarone and sotalol). Triamterene and sotalol revealed at high doses antifibrillator activity, while the benzyl-triamterenes 2, 3, 5 and 6 could also depress ventricular extrasystoles (VES) and ventricular tachycardia (VT). At low doses the most benzyltriamterenes protected significantly against ventricular fibrillation (VF) and so they were equieffective or more effective than amiodarone or lidocaine. Amiloride showed in the CAL-R rat no antiarrhythmic activity, so that we conclude different mechanisms responsible for antikaliuretic and antiarrhythmic properties of amiloride and triamterenes. Taking into account the results of recently reported in vitro studies, where we could demonstrate antiarrhythmic activity combined with positive inotropic properties for triamterenes, the antiarrhythmic profile of these compounds may offer new possibilities for the treatment of ventricular arrhythmias.

Export and activity of hybrid FhuA'-'Iut receptor proteins and of truncated FhuA' proteins of the outer membrane of Escherichia coli.

The FhuA protein in the outer membrane of Escherichia coli serves as a multifunctional receptor for the phages T5, T1, phi 80, for colicin M, for ferrichrome (Fe3+-siderophore) and for the structurally related antibiotic, albomycin. To determine structural domains required for these receptor functions and for export, a fusion protein between FhuA and Iut (receptor for Fe3+-aerobactin and cloacin DF13) was constructed. In the FhuA'-'Iut hybrid protein, 24 amino acids of FhuA were replaced by 19 amino acids, 18 of which were from Iut. The number of plaque forming units of phage T5 and T1 on cells expressing FhuA'-'Iut was nearly as high as on cells expressing plasmid-encoded wild-type FhuA. However, 10(7)-fold higher concentrations of phage phi 80 and 10(3) times more colicin M were required to obtain a zone of growth inhibition. Truncated FhuA' proteins in which the last 24 amino acids at the carboxy-terminus were replaced by 16 (FhuA'2) or 3 (FhuA'T) amino acids could hardly be detected on polyacrylamide electrophoretograms of outer membrane proteins, due to proteolytic degradation. Sensitivity of cells expressing FhuA'2 to phage T5 and T1 was reduced by several orders of magnitude and sensitivity to phage phi 80 and colicin M was totally abolished. In contrast, cells expressing FhuA'T were nearly as sensitive to pahge T5, T1, and phi 80 and to colicin M as cells containing FhuA'-'Iut. None of the constructs could grow on ferrichrome as sole iron source and none was sensitive to albomycin.(ABSTRACT TRUNCATED AT 250 WORDS)

[Basic diet in Crohn's disease].

26 children with acute symptoms of Crohn's disease were treated with steroids and sulfasalazine (group A, 9 patients), with an elementary diet and sulfasalazine (group B, 10 patients), or with an elementary diet together with steroids and sulfasalazine (group C, 7 patients). Mean ages in each group were very similar, and treatment periods lasted 41.3 +/- 23.5, 37.4 +/- 12.7, and 36.9 +/- 9.1 days in group A, B and C, respectively. During treatment the sedimentation rate and the frequency of defecation decreased significantly in each group. Leucocytes in plasma fell to normal levels in group B but increased slightly in group A and C. Total proteins in plasma rose in group B and C but declined in group A. The body weight increased in each group but differences before and after treatment were significant only in group B and C. The findings suggest that children with active Crohn's disease may benefit from treatment with an elementary diet. Patients with an elementary diet showed significantly more weight gain than patients on steroid treatment. Besides nutritional effects a lower activity of inflammation was also noted.

[Fascial closure of median laparotomies with a synthetic, resorbable suture material (polyglycolic acid)].

In a prospective randomized study polyglycolic acid sutures (PGA) and polyester sutures for closure of median abdominal fascial incisions are compared. Occurrence of infection, incisional hernia, wound dehiscence and suture fistulas is evaluated. With regard to these complications no significant differences could be detected. PGA shows its advantage in less tissue reaction (avoiding fistulae).
