Asymptomatic degenerative disk disease and spondylosis of the cervical spine: MR imaging.

Evidence on magnetic resonance (MR) images of disk degeneration and herniation, as well as of cord and root impingement, may be regarded either as normal, age-related changes or as causative of symptoms. Individuals referred for MR examinations of the larynx without symptoms referable to the cervical spine were studied retrospectively (35 patients) or prospectively (65 patients) over a 2-year period. With a solenoid surface coil, 5-mm-thick sections were acquired in sagittal, axial, and coronal planes with T1-weighted spin-echo pulsing sequences. Disk protrusion (herniation/bulge) was seen in five of 25 (20%) patients aged 45-54 and 24 of 42 (57%) patients older than 64 years of age. Posterolateral protrusions were seen in only nine of 100 patients and occurred with greatest frequency in patients over 64 years of age. In no patient was obliteration of the intraforaminal fat seen. Spinal cord impingement was observed in nine of 58 (16%) patients under 64 years of age, and in 11 of 42 (26%) patients over 64 years of age. Cord compression was observed in seven of 100 patients and occurred solely secondary to disk protrusion in all cases. The percentage of cord area reduction never exceeded 16% and averaged approximately 7%.

Multifocal CT enhancement in MS.

Review of 146 CTs in 125 MS patients showed that 53.4% (39/73) of those done during acute relapse showed enhancing lesions. Of scans done during the "active" stage of disease, 40.8% (20/49) showed enhancement, whereas only 12.5% (3/24) showed enhancement during clinically inactive periods. Autopsy confirmation that enhancing lesions represent demyelinating plaques was obtained in one patient. This study shows that enhancing lesions in MS reflect biologic activity and are most likely to be seen during acute clinically apparent relapses. More than one-half of the patients hospitalized with acute attacks appeared to be having multifocal relapses. These findings show that acute relapses are commonly multifocal and suggest that they are mediated by blood-borne factors.

Two different calibrated-leak balloons: experimental work and application in humans.

Two different types of latex calibrated-leak balloon catheters have been developed. One consists of a Teflon catheter with a detachable latex balloon and is used to embolize branches of the external carotid artery. The other consists of a Silastic catheter with a nondetachable latex balloon and is used to embolize brain arteriovenous malformations. An experimental model and animal experiments have determined the best conditions for safe and reproducible embolization with isobutyl-2 cyanoacrylate (IBC-2). Selective catheterization of branches of external carotid, middle cerebral, anterior cerebral, and posterior cerebral arteries in humans is facilitated, and embolization with IBC-2 has been achieved with these balloon catheters.
