Survey of Escherichia coli septicemia over a six-year period.

Escherichia coli was the most frequent species isolated from blood cultures in the Hospital Covadonga of Oviedo (Spain) over a six-year period (474 episodes, 15.3% of the total septicemias and 2.7 episodes per 1,000 patients). Escherichia coli strains were susceptible in greater than 95% of episodes to cefoxitin, cefotaxime, gentamicin, tobramycin and amikacin. In a series of 72 episodes, microbiological features and host factors were studied. No endemic stains were found. Type 1 fimbria was detected in 73.6% of strains and P-fimbriae in 12.5%, without correlation between P-fimbria and urinary infection; 84.7% of the strains were resistant to decomplement human serum; 61.1% produced aerobactin and 20.8% were hemolytic. Factors such as age, hospital location, metastatic focus and surgical treatment were significantly correlated with morbidity and mortality. The global mortality rate was 18%, and in 8.3% of cases was directly associated with septicemia.

Characteristics of Pseudomonas aeruginosa strains causing septicemia in a Spanish hospital 1981-1990.

The epidemiological and biochemical characteristics of Pseudomonas aeruginosa strains causing septicemia in a Spanish hospital over a ten-year period (1981-1990) were analyzed. A total of 207 episodes, corresponding to 0.7 episodes per 1,000 inpatients and 3.2% of the total number of episodes of septicemia, were registered. Males were more often affected than females (rate 3.2:1). The respiratory (24.6%) and urinary (21.2%) tracts were the main portals of entry, while haematologic and solid tumours (15.4%) were the most frequent underlying diseases. More than 86% of the strains were susceptible to ceftazidime, mezlocillin, piperacillin and amikacin. Seventy strains were subjected to typing and analysis of virulence factors. Serotypes O:6, O:11 and O:2 could be considered endemic (each present in more than 11.4% of strains). Pyocin typing, antibiotyping and resistotyping were preferred as secondary typing methods to phage typing and plasmid profile analysis. The combination of methods revealed a large diversity of strains although some clusters predominated. More than 80% of the strains produced several exoenzymes, possessed pyoverdin and showed haemolytic activity, and all except one showed serum resistance. All strains were susceptible to silver and more than 80% to mercury and boron, but all were resistant to iodine.

[The abdominal complications of ventriculoperitoneal diversions].

We report several cases of abdominal complications in hydrocephalus children with ventriculoperitoneal shunts implanted by different ethnology, as well as the pathology produced. The number of children affected was 16, although some of them had suffered more than one complication, making up a total of 25 cases. The most frequent trouble was due to the intraperitoneal catheter fall, followed by intestinal perforation at different levels, allowing the catheter to get out through the anus, abdominal wall or surgical incision. Some intestinal occlusive situations was due to either volvulus or external compression caused by cerebrospinal fluid abdominal pseudocysts, secondary to ventriculoperitoneal shunt. We took different characteristics of this group of patients and studied the different complications in relation with specialized reports by international authors.

Vesicular pemphigoid.

A patient with a pruritic vesicular eruption closely resembling dermatitis herpetiformis is described. Histopathologic examination of a biopsy specimen showed a subepidermal blister containing abundant eosinophils and neutrophils, whereas results of direct immunofluorescence studies demonstrated linear and homogeneous deposition of IgG and C3 along the basement membrane zone. Prednisone therapy was necessary to control the clinical symptoms. Similar cases have been described as "vesicular pemphigoid."

Malignant cutaneous mixed tumor. A new case with unusual clinical features.

We report the case of an ulcerated malignant cutaneous mixed tumor occurring on the nose of a 75-year-old woman. Malignant cutaneous mixed tumor (CMT) is a rare neoplasm. Of the 14 previously reported cases, 10 occurred on the extremities, three on the trunk, and one on the scalp. This contrasts with the benign CMT which is found mainly on the head and neck region, the nose being the most commonly affected site. Although eight of the cases of malignant CMT recurred and 10 metastasized, the primary lesion never ulcerated.

Modification of motor activity, passive avoidance conditioning and evoked potentials by microinjections of picrotoxin in both caudate nuclei in cats.

Inhibition of movements is the result of the caudate nucleus (CN) activity. This action seems to be necessary for passive avoidance conditioning (PAC). Much data indicates that GABA has inhibitory effects in the CN. The present report provides further evidence for GABA inhibition in th4 CN. In cats, the acquisition session of a PAC response was carried out following the bilateral administration of 6 microgram of picrotoxin or saline in the CN. Twenty four hours later (1st test session) the animals were placed in the security compartment and remained there during 600 sec. In the second test, the cats injected with picrotoxin crossed to the other compartment while the subjects injected with saline did not move. Another series of acute experiments were also carried out. Evoked potentials (EP) were recorded in the CN produced by nucleus centralis medialis electrical stimulation. After picrotoxin application the first and second peaks, which could represent EPSPs, incraeased while the third peak, which could represent IPSPs, decreased. In conclusion, it seems that GABAergic intracaudate transmission maintains a lower level of neuronal discharges and probably participates in the maintenance of a PAC but not in its acquisition.

Bacteremia due to Staphylococcus epidermidis: microbiologic, epidemiologic, clinical, and prognostic features.

Staphylococcus epidermidis was established as the causal agent of true bacteremia on the basis of microbiologic, epidemiologic, clinical, and prognostic data on 65 episodes of S. epidermidis bacteremia (SEB) recorded in the Hospital Covadonga of Oviedo, Spain, during 1982-1986. All isolates were susceptible in vitro to vancomycin, mercury, silver, cadmium, and malachite green, and 95.2% of isolates were susceptible to cephalothin; a dominant biotype, four resistotypes (ion-dye resistance patterns), and four antibiotypes (antibiotic resistance patterns) were found. A high percentage of isolates (76.7%) produced slime, and several potentially toxic exoproteins were detected in various and generally lower proportions. The main clinical features of SEB were high frequency in male patients greater than 40 years of age, fever, leukocytosis, anemia, and phlebitis. Indwelling devices were the most common portal of entry. The overall death rate was 36.9%. Underlying disease, hemodynamic status, predisposing factors, and treatment were statistically significant parameters in relation to mortality.

[Presence of Triatoma infestans in relation to various cultural patterns in a rural population of the Province of San Juan, Argentina].

This paper reports results from a punctual epidemiological survey performed in San Juan, at Bermejo, a rural village located 100 km east from the provincial capital, in December 1987. The village had been rebuilt after almost complete destruction by an earthquake, in 1977. According to a census performed by local Primary Health Care Agents, there were 82 households, where 72 (88%) of them were inhabited, with a total human population of 198 people. Forty-eight from those inhabited dwellings (67%) were visited, and a general questionnaire was completed. Information gathered included, sex, age, working activity, number of people and domestic animals at the house, construction patterns of houses and outdoor premises, resting habits of animals, specially dogs, domestic use of insecticides and migration patterns of inhabitants. A timed collection of triatomines (man-hour method) was undertaken in 17 out of 48 of the visited households. Sampled triatomines were kept and classified by collection site, instar, and sex at the field. Species identification, microscopical examination of fecal contents to detect trypanosomes and blood-meals identification, were performed at the laboratory. Sixty-nine per cent (33/48) of the dwellings were "ranchos" with mud-brick ("adobe") and cane walls, roofs made of cane and mud dirt floors. The remainder were as follows: 11 (23%), partially improved houses and 4 (8%) had brick walls and were tiled or had zinc roofs. Data of migration patterns of local population were obtained in 31 (65%) out of 48 households. Seasonal movements within San Juan Province, mainly related to vintage were reported in 20/31 (65%) families.(ABSTRACT TRUNCATED AT 250 WORDS)

Gonorrhea in women prostitutes: clinical data and auxotypes, serovars, plasmid contents of PPNG, and susceptibility profiles.

Eighty-nine women prostitutes who underwent clinical and microbiologic examination were found to have gonococcal infection. The median age was 22; 92.1% were from urban areas. Nearly all the women prostitutes refrained from barrier methods (92.1%) and had contact with several partners (91.0%). The most frequent clinical findings were leukorrhea (50.6%), cervicitis (20.2%), and pelvic inflammatory disease (PID) (18.0%). Eighty-one women prostitutes (93.1%) had experienced a previous STD, with Chlamydia trachomatis (34.8%), Trichomonas vaginalis (30.3%), Neisseria gonorrhoeae (29.2%), and Ureaplasma urealyticum (23.6%) as the most frequent microorganisms isolated. Microorganisms associated with N. gonorrhoeae were isolated, mainly T. vaginalis (40.4%), C. trachomatis (31.5%), and Mycoplasma hominis (21.3%). For N. gonorrhoeae, the most frequent auxotypes were prototrophic (67.4%) and Proline (Pro)-dependent (14.6%); 2.2% of the strains were non-auxotypable. Beta-lactamase production was detected in three strains (3.4%) belonging to the auxotype/serovar: Lys/IA, Prototrophic/IB, and Pro/IB. The two former produced the 3.2-MDa "African" plasmid; the latter produced two plasmids (the 4.5-MDa "Asian" and the 24.5-MDa transfer plasmid.
