[Evaluation of cerebral blood perfusion in patients undergoing carotid endarterectomy using densitometric analysis of conventional cranial CT].

OBJECTIVE: To evaluate the efficacy of densitometric analysis of cranial CT imaging in the measurement of areas with low cerebral blood flow and lack of hemodynamic reserve in patients undergoing carotid endarterectomy. PATIENTS AND METHODS: 40 consecutive patients undergoing carotid endarterectomy have been included in the study. All of them had preoperative cranial CT, pre and postoperative basal and acetazolamide SPECT. Cranial CT imaging after digitalization and computer processing were obtained with 4 densitometric patterns: 1). normal cerebral blood perfusion; 2). ischemic or low brain blood perfusion (patron I); 3). parenchyma without cerebral hemodynamic reserve (patron nR), and 4). brain infarction. RESULTS: 32 out of 40 (80 %) patients had abnormal densitometric patterns: 19 nR patterns (47.5 %), 11 I pattern (27.5 %) and 2 established brain infarction. The correlation between densitometric cranial TC imaging and SPECT was 92% when analyzing areas with hypoperfusion (S: 96.5 %; E: 82 %). Cranial CT detected 12/13 of patients who lacked cerebral hemodynamic reserve in the SPECT analysis (S: 74 %; E: 92 %; Vpp: 95 %). CONCLUSIONS: There is a good correlation between analysis of densitometric cranial TC imaging and SPECT in determining low cerebral blood flow areas and lack of cerebral hemodynamic reserve. This method could help to improve carotid surgery indications in patients with carotid stenosis.

Multiple cholesterol embolization syndrome: a lethal complication of vascular procedures. Report of two histologically proven cases.

Two cases of multiple cholesterol embolization syndrome (MCES) leading to death are presented. Rarely described and labeled as a "great masquerader", its presence in the medical literature is growing, since it is best diagnosed, but despite the high frequency with which MCES may occur, only a few cases are reported. This syndrome is not universally recognised.

[Long-acting and repeatable dose bromocriptine in the prolonged treatment of prolactinoma].

BACKGROUND: The presentations of intravenous or depot bromocriptine (bromocriptine LA or long acting, Pariodel LA and bromocriptine LAR or long acting repeatable, Pariodel LAR) have improved the efficacy and the tolerance of oral bromocriptine. In contrast to bromocriptine LA, bromocriptine LAR may be repeatedly administered intramuscularly. METHODS: Five patients with macroprolactinoma and 4 with microprolactinoma were included in the study. A 50 mg bottle of bromocriptine LAR was administered intramuscularly every month, over a minimum period of 6 months. PRL was determined prior to the study, 1, 3, 7, 14, and 28 days following the initial dose of bromocriptine and thereafter with monthly periodicity. RESULTS: The PRL values decreased in those patients with macroprolactinomas following the administration of bromocriptine LAR; in 2 patients the monthly doses of bromocriptine LAR was increased to 100 mg since the month after the initial dose PRL remained greater than 200 ng/ml with serum RPL normalizing in most of the patients at 6 months of treatment. In two of the three patients who presented visual changes a clear improvement was observed and in all the cases a reduction in the size of the macroprolactinoma was found upon CAT control at 6 months. The PRL values also decreased in the patients with microprolactinomas following administration of bromocriptine LAR, although the response was not as homogeneous as in the patients with macroprolactinomas due to that at 6 months 2 patients continued to have slightly elevated serum PRL levels. The microadenoma persisted in the control CAT at 6 months except in one case. Local and general tolerance to bromocriptine LAR was very good. CONCLUSIONS: This study indicates good tolerance to bromocriptine LAR, being a therapeutic option in the treatment of macroprolactinomas.

Mating system in rye: variability in relation to the population and plant density.

The amount of outcrossing was estimated using seven enzyme loci assayed in seven populations of rye (Secale cereale L.). Single-locus outcrossing values fluctuated widely from locus to locus in each population. The weighted mean single-locus estimates ranged from 0.716 to 0.946, and multilocus estimates ranged from 0.701 to 0.910. The analysis showed that self-pollination occurred in the rye populations, and, as a result of selfing, populations contained homozygotes in excess of random mating expectations at the seedling stage of development. Low plant density, which causes low pollen density during fertilization, seems to weaken the self-incompatibility system; at low plant density, the outcrossing estimate was significantly lower than was obtained at high plant density.

[Vascular diseases masking malignant tumors of soft tissues].

Two case reports with Myxoid and Pleomorphic varieties of Malign Fibrohistiocytoma are presented. In both cases, the first consult took place in an Angiology and Vascular Surgery Service, because their clinical symptoms were similar in the first case to such produced by the arteriovenous fistulas and in the second case, symptoms remaid to an hematoma secondary to a varicose vein rupture. Such rare and malign tumors often are diagnosed as vascular diseases, and they may be discovered in the usual clinical practice, so its important an accurate recognition as well as an adequate treatment.

[Arteriosclerotic aneurysms isolated from the internal iliac artery].

A case of an isolate, symptomatic, atherosclerotic aneurysm of the left internal iliac artery is presented. A review from this very rare type of pathology reveal that clinical symptoms depends on the comprised anatomical structures. Diagnosis was made by rectal or vaginal touch, ultrasonography and CT. The operatoire mortality rate is high when aneurysms are ruptured.

[Gigantic aneurysm of the popliteal artery].

A giant popliteal aneurysm case, whose first symptom was an acute ischemia on the limb, caused by thrombosis, which was successfully treated, is reported. Although popliteal aneurysm is not a rare event, the interest of this case is focused on its extraordinary size and unique location.

[Inflammatory aneurysm of the abdominal aorta. Apropos of a case].

The presentation of a case of inflammatory aneurysm in abdominal aorta and its bibliographic revision, makes evident the short knowledge of its etiopathogenesis and the need of TAC for its differential diagnosis and its treatment and correct follow up.

[Prophylactic treatment of episodic cluster headaches with methysergide and prednisone].

In two consecutive series of 25 cases each suffering from episodic cluster headache, one of the clusters was treated prophylactically in the first series with methysergide and in the other with prednisone. The duration of the clusters under treatment was similar in both series. The results of our study suggest that neither the methysergide nor the prednisone were able to shorten the duration of clusters.
