Urethral prolapse in children.

We report our experience with 23 girls with urethral prolapse. Vaginal bleeding was the most common complaint, and predisposing factors (cough, trauma, and constipation) were found in 10 children. Although reported almost exclusively in black girls, 14 of the 23 patients were white. Three basic techniques were used for therapy: conservative management, ligation over a Foley catheter, and total excision of the prolapse. Ligation over a Foley catheter had a high incidence of complications (partial recurrence, infection, postoperative pain) and is no longer used. The best results were obtained by complete excision of the urethral prolapse. We propose that treatment should be based on the etiopathogenesis of the prolapse and the clinical condition of the child; patients with a single and acute episode of increased abdominal pressure, such as trauma, and those at high risk for general anesthesia are managed by conservative therapy. All others, and patients who fail medical treatment, undergo surgical excision.

A study on the best order for autoregressive EEG modelling.

The autoregressive (AR) model is a widely used tool in electroencephalogram (EEG) analysis. The dependence of the AR model on both the segment length and several characteristic EEG patterns is addressed. The best AR model order is computed with three different criteria. The results show that the Rissanen criteria provides the more consistent order estimate for the EEG patterns considered. This study shows that for our data set, a 5th order AR model represents adequately 1- or 2-s EEG segments with the exception of featureless background, where higher order models are necessary.

Posterior chamber silicone phakic intraocular lens.

BACKGROUND: We investigated the efficacy, safety, and stability of correction achieved with a silicone posterior chamber intraocular lens used to correct high myopia in phakic eyes. PATIENTS AND METHODS: A silicone posterior chamber plate-style intraocular lens (Chiron, Adatomed) was implanted in 38 consecutive phakic eyes with high myopia (-7.00 to -28.00 diopters (D) over a period of 21 months. Follow-up ranged from 3 to 24 months. RESULTS: Three months after surgery, refractions in 27 eyes (71%) ranged from -1.00 to +1.00 D and remained stable thereafter. Spectacle-corrected visual acuity improved at least two lines in 24 eyes (63%), and none was lost in any eye. Patient satisfaction was high (92%). The few complications that occurred were mostly related to imperfections in the surgical technique rather than lack of biocompatibility of the lens. No cataract or other vision-threatening complication was present at the end of the study. CONCLUSION: This silicone posterior chamber plate-style intraocular lens provides a reasonably predictable, safe, and stable means of correcting high myopia.

[Wernicke-Korsakoff syndrome: malignant tumour as triggering factor].

Gastrectomy, alcoholism and malignant tumour are three predisponing risk factors for the development of Wernicke-Korsakoff syndrome. We described the clinical case of a patient with history of alcoholism that developed Wernicke-Korsakoff syndrome 30 years after undergoing gastrectomy. This patient had, in the last year, a diagnostic for prostatic adenocarcinoma and changes in dietary habits. We presented the clinical and neuropathological features of the Wernicke-Korsakoff syndrome. As well as some aspects in the treatment and prognosis.

[Influence of body fat topography on glucose homeostasis and serum lipids].

Predominant fat distribution in the upper body segment evaluated by waist to hip circunference ratio (WHR) has been associated with diabetes mellitus and cardiovascular morbidity excess. To investigate metabolic alterations underlying this risk excess, we selected 2 groups of 10 obese women without history of hypertension, menstrual irregularities or oral contraceptives, matched according to age (mean +/- SD): 30.5 +/- 5.3 vs 30.6 +/- 5.8 years and BMI 35.5 +/- 6.5 vs 35.7 +/- 6.7 Kg/m2. Each matched pair had a difference in WHR superior to 0.15 (0.83 +/- 0.04 vs 1.02 +/- 0.05). Insulin and C peptide were determined during an oral glucose tolerance test (75 g). At 30, 60, 90 and 120 minutes differences were significant for glycaemia, insulinaemia and C peptide. Fasting triglycerides were 103 +/- 48 in the lower WHR group vs 164 +/- 84 mg/dl (p less than 0.05); total cholesterol 186.5 +/- 31 vs 215.2 +/- 29.4 mg/dl (p less than 0.05); LDL cholesterol/HDL cholesterol 2.46 +/- 0.89 vs 3.18 +/- 0.96 (p less than 0.05). No significant differences were found in androgenic activity. We conclude that preferential fat distribution in the upper segment is, by itself, an aggravating factor of metabolic alterations associated with obesity, particularly dyslipidaemia and hyperinsulinaemia.
