Depression secondary to tumors in patients who underwent surgery for mammary carcinoma: psycho-pharmaceutical and psychotherapeutic care.

The Authors repeated a pilot psycho-social research they had already undertaken in this University in 1986-1987, with an increased number of patients, administering a psycho-therapeutic treatment to patients who had undergone surgery for mammary carcinoma and correlating the evolution of the tumoral disease with the role of depression and the immune system. These 50 patients were compared to another randomly chosen control-group of 50 patients. Both groups were homogeneous as regards medical-surgical and psycho-social parameters and had routine oncological care. Only the patients of the study-group were submitted to individual psychotherapy and treated psychopharmacologically. At the end of the study, the two groups showed a statistically significant difference as regards the evolution of the tumoral disease in favour of the study-group. In addition, a relevant improvement from depression was recorded in the study-group along with a normalization and a boost of the immunological measurements.

[Squamous-cell carcinoma of the thyroglossal duct. A clinical case].

The authors report a rare case of squamous cell carcinoma arising in a thyroglossal duct cyst. The lesion is a malignancy arising directly from the lining epithelium of the thyroglossal duct remnant and as a rule has a poor prognosis with survival ranging from few months to few years.

[Loco-regional anesthesia in the surgery of inguinal and crural hernias. The authors' own experience with 1730 treated cases].

The authors report their experience with the use of loco-regional anaesthesia for the surgical treatment of inguinal and femoral hernias. Their series includes 1730 operations, 235 of which for hernia relapses, performed exclusively in the outpatient department. In the authors opinion, with loco-regional anaesthesia it is possible to treat any type of inguinal-femoral hernia, reducing the risks related to total anaesthesia and the complications arising from bedrid. Excellent results were achieved especially in the elderly and high risk patients like cardiopathic or those affected by respiratory and hepatorenal insufficiency with a complete absence of mortality and morbidity.

[Recurrence of inguinal and crural hernias].

The main problem in inguinal and femoral hernia repair is the risk of recurrence. In this study the Authors analyze possible causes of recurrence on the basis of their own experience (123 cases of recurrences) and literature review. They classify factors influencing recurrence in: preoperative, depending on patient's features, intraoperative, the most important ones, depending on the surgical technics used and postoperative, essentially due to complications.

Outpatient surgery of inguinal and crural hernias: a report on 468 cases.

A personal series of 468 inguinal and crural hernias treated with local anesthesia and outpatient surgery is reported. The marked advantages of loco-regional anesthesia are stressed and the various surgical procedures used in the different conditions examined. Need for maximum reduction of suture tension is underlined. All patients of the series resumed immediate postoperative ambulation and a number were discharged within 1-2 hours returning to their working activity the day following surgery. Follow-up was performed for cases operated on from 6 months to 6 years: overall incidence of recurrence was 1.1% and 0.37% in cases where Marlex mesh was used, whose marked advantages are evidenced.
