[Infected wounds].

Continuous prophylactic therapy with antibiotics has been used for a long time to try to prevent the step from bacterial contamination to clinical infection of surgical wounds. Earlier results of clinical trials were poor and side-effects common. For those used in recent years, however, the side-effects of pre- or perioperative prophylactic antibiotics have been minimal, although the therapeutical benefits have not yet been proven. This has also been the case with prophylactic infusion of gamma-globulin for prevention of postoperative wound infections.

Adult

[Hygiene and asepsis in surgery: introduction (author's transl)].

Since the development of asepsis and clinical hygiene by European and American surgeons in the second half of the nineteenth century, the prevention of infection has been one of the most important concerns of surgical practice. However, owing to the rapid expansion of therapeutic methods, medical practice now makes greater demands on clinical hygiene than has hitherto been the case; it has not always been possible for developments in clinical hygiene to keep pace in this respect, and the closing of the resulting gap represents one of the most urgent tasks confronting medicine today.

Age Factors

[Immunologic aspects to the prevention of infection cardiac surgery (author's transl)].

1. The humoral immunosystem is impaired in those cases of catabolic metabolism in which quantitative and qualitative insufficient alimentation is expected after operation. 2. We showed that after cardiac surgery with extracorporal circulation as well as after other operations a reasonably balanced parenteral nutrition composed of carbohydrates and amino acids could considerably inhibit the changes in the humoral immunity, which is a valuable contribution to the prevention of infection. 3. In our experiments on transformation of lymphocytes after mitogenic stimulation, the depressive effects of operation on the cellular immunity were found to be etiologically more complex than was previously assumed in publications.

Bronchoscopy
