Departmental audit of tonsillectomy haemorrhage rates: pitfalls in interpretation.

Concerns that a largely anecdotal increase in post tonsillectomy haemorrhage rates was related to the introduction of disposable instruments have prompted much investigation. The result has been, rather, to highlight other variables influencing this risk, but especially to insist on the following: 1. Training in traditional 'cold' techniques. 2. Regular departmental audit of haemorrhage rates. 3. Presentation of such data to patients to ensure informed consent. This audit demonstrates the pitfalls in interpretation of crude data, unadjusted for case-mix, in predicting individual patient risk and in national ranking of unit performance.

Audit of stapedectomy results in a district general hospital.

We report a series of 100 stapes operations performed for otosclerosis. The problems in reporting the results of stapedectomies are discussed. The audit showed a difference in the measured results between two consultants. As a result, the consultant with the worse outcome chose to stop operating on patients with otosclerosis. The advantages and disadvantages of a single surgeon performing all the cases are discussed. We advocate a central registry of all surgeons performing stapes surgery to allow periodic national comparative audits.

Trans-nasal endoscopic holmium: YAG laser correction of choanal atresia.

Five patients with choanal atresia were treated, using a trans-nasal endoscopic Holmium:YAG laser. The age of patients ranged from 15 months to 53 years. The nasal endoscope provides excellent visualization. The advantage of the Holmium:YAG laser is its fibre-optic delivery, allowing the laser to be hand-held, giving greater precision. Good nasal airways were achieved in four out of five cases with a single treatment. The procedure was found to be safe, quick, with minimal morbidity and a shorter in-patient stay.

Unusual presentation of verrucous carcinoma of maxillary antrum.

Verrucous carcinoma is a rare type of squamous cell carcinoma that is most often seen in the oral cavity and larynx. This paper describes a case report of a rare tumor with an unusual presentation. This patient presented with recurrent pyoceles and was diagnosed by chance. Carcinoma of maxilla is rarely diagnosed until the bony walls are eroded. This condition should be kept in mind in patients presenting with recurrent pyocele and a biopsy carried out from the maxilla.

An evaluation of post-operative packing in nasal septal surgery.

The need for nasal packing in septal surgery is not proven though its use is widespread. Post-operative complications, while uncommon, are frequently pack related. Consenting adults were prospectively randomized to one of the following: Vaseline gauze nasal packing or 30 Dexon mucosal suture. All patients were operated on by one surgeon who was made aware of the randomization decision only when the corrective surgery was complete. Details of post-operative morbidity were collected and pain scored subjectively by a visual analogue scale the morning after surgery. The first 50 available pain scales illustrated a difference between the groups (P less than 0.05), means 4 and 3 in the pack and suture group respectively. There was no demonstrable difference in post-operative haemorrhage, adhesions, nasal crusting or mucosal atrophy. The need for nasal packing is not supported.

Endaural meatoplasty for mastoid cavities.

Obtaining an adequate meatoplasty when performing mastoid surgery via an endaural approach is not always easy. This paper reviews the different types of meatoplasty available and describes a transposition flap of pretragal skin which can be used to provide a meatoplasty for mastoid cavities, acquired stenosis or congenital stenosis.

Fluctuating hearing loss in presbyacusis.

Thirty-six patients with bilateral symmetrical presbyacusis who reported a temporary improvement in the hearing of one ear following a Valsalva's manoeuvre were further investigated. Bone conduction and air conduction thresholds, middle ear pressure and middle ear compliance were measured before and after Valsalva's manoeuvre in the ears which had a subjective improvement in hearing following auto-inflation. Bone conduction thresholds remained unaltered in 66 per cent of ears while average air conduction thresholds varied by less than 5 decibels. Middle ear pressure was unchanged in over half the ears tested and in 81 per cent of the ears there was no change in middle ear compliance. There appears to be no simple explanation for the temporary subjective fluctuation in hearing reported by patients with presbyacusis.

Tinnitus: a double-blind crossover controlled trial to evaluate the use of lignocaine.

A double-blind crossover controlled trial assessing the use of intravenous lignocaine in the treatment of tinnitus is reported. Possible modes of action of the drug are suggested. It is shown that intravenous lignocaine is effective in reducing tinnitus, although duration of such effect is short. Consistently effective oral preparations to control tinnitus are yet to be described.

Craniometaphyseal dysplasia.

A case report of craniometaphyseal dysplasia is presented with a classification of this disease, supported by a review of the literature. Certain unusual features are observed-mastoid surgery and later contralateral tempory facial paresis. The patient showed no further deterioration of symptoms when last reviewed.

Is a scanning ion microscope feasible?

Atomic collisions of high-energy heavy ions produce large yields of x-rays. The small de Broglie wavelength of massive ions leads to an estimate of 0.2 angstrom for the resolution of a microscope utilizing nitrogen ions with energies of 14 million electron volts. Estimates of the yield of x-rays relative to molecular radiation damage of the bases in DNA are made.

In vitro measurement of pollen tube growth inhibition.

A method for estimating inhibition of pollen tube growth was developed. Pollen is placed in straight lines on an agar surface where it responds uniformly and predictably to aqueous solutions of germination-inhibiting substances located in wells at the ends of the lines. A scale of ratings, roughly corresponding to serial, doubled concentrations of inhibiting substances, was devised. Water-soluble organic solvents are relatively noninhibitory, salts are variable, and metabolic inhibitors have strong inhibitory effects. Pollens differ in their susceptibility to inhibition and in their response to particular substances.
