Management of rhinosinusitis in children.

The authors provide definitions for the different forms of pediatric rhinosinusitis, with an enumeration of the main symptoms and signs. They also provide the indications for CT scan examination and microbiological investigations. In addition, they emphasize the importance of concomitant systemic disease, such as allergy and immunological disorders. The adequate medical management, which is mandatory before any surgery, is considered and discussed, and the indications for surgery are provided.

Management of rhinosinusitis in children: consensus meeting, Brussels, Belgium, September 13, 1996.

OBJECTIVES: To (1) provide definitions for the different forms of pediatric rhinosinusitis, with an enumeration of the main symptoms and signs; (2) provide indications for microbiological, allergic, and immunologic assessment as well as for imaging studies; (3) suggest standard medical management with judicious use of antimicrobial agents; and (4) discuss indications for surgery. DATA SOURCES: Clinical studies and literature data relevant to the different topics of pediatric rhinosinusitis. CONCLUSIONS: Rhinosinusitis in children is a multifactorial disease in which the importance of several predisposing factors changes with increasing age. Continued study to obtain a better understanding of the disease and carefully controlled comparative evaluations of medical and surgical therapies are suggested.

The socio-economic distribution of heart diseases: changing gradients in The Netherlands.

Two different Dutch data-sets were used to examine trends in the association between socio-economic characteristics and the risk of heart disease. Data of the Dutch Quality of Life Surveys allowed the examination of trends in the association between educational level and self-reported heart disease during the period 1974-1993. For both the general Dutch population and the subpopulation of men aged 40 years and older, we found an inverse gradient during the whole period. The gradient climaxed at 1980-1983, and narrowed afterwards. Furthermore, ecological analyses, relating regional mean personal incomes to regional directly standardised mortality rates of coronary heart diseases (CHD) and all heart diseases, showed similar patterns of social differentials. The findings suggest that, in the Netherlands, there is a narrowing gradient of the association between socio-economic characteristics and heart disease in the late eighties and early nineties.

[Quality of work and absenteeism: lower or higher in elderly workers?].

Data of the Quality of Life Surveys 1990-1993 (n = 9969) were used to examine if and to what extent elderly workers differ from younger workers with regard to perceptions of the quality of work, especially the workload, and to the percentage absence due to sickness. Adjusted for occupational level, educational level and sex, persons aged 60-64 were characterised by either a substantially lower or a similarly perceived workload as compared to those aged 55-59. Subsequently, adjusted for the occupational level, educational level, sex and perceived workload, a substantial lower rate of absenteeism was found for persons aged 60-64. To a certain extent comparable results were found in the comparison of persons aged 55-59 with those aged 45-54. Based on the low grade of job participation of the elderly, it is proposed that these favourable figures support the notion of an age-related healthy-worker selection in the Netherlands.

Risk of mortality and coronary heart disease by marital status in middle-aged men in The Netherlands.

Between 1972 and 1974, a cardiovascular screening survey was conducted in a stratified sample of 3365 men aged 45-59 in Rotterdam, the Netherlands. Follow-up data collected in 1982 were used to examine the association between marital status and mortality and coronary heart disease while adjusting for various control variables. Nonmarried men had significantly higher relative risks (RR) (95% confidence interval (CI)) of 1.7 (95% CI: 1.2-2.3) and 2.2 (95% CI: 1.2-4.2) for all-cause mortality and coronary mortality than the married. Never married men showed the most consistent relationships with all-cause and coronary mortality, with RR of 2.3 (95% CI: 1.6-3.4) and 2.9 (95% CI: 1.4-6.2) respectively. The RR for these endpoints among the widowed and divorced were all close to unity, except for the risk of coronary mortality among the widowed, which was 2.9 (95% CI: 0.9-10.2). Not being married also increased the risk for fatal and total reinfarction, with RR of 3.6 (95% CI: 1.4-9.1) and 2.5 (95% CI: 1.1-5.6) respectively. The results suggest that in middle-aged Dutch males, the health consequences of not being married may differ for the never married, divorced, and widowed. Selective mating, differential lifestyles or health habits, and lack of social integration were offered as possible explanations.

[Destruction of the nasal skeleton by cocaine sniffing].

An ill-understood, severely ulcerating inflammation of the nose in a 27-year-old female patient turned out to be due to cocaine abuse. The course of the disease process in the hospital, the differential diagnosis and the therapy of this cocaine-induced osteocartilaginous necrosis are described.

The Rapp-Hodgkin syndrome.

We report on a family with the Rapp-Hodgkin ectodermal dysplasia syndrome. Four affected family members are described and a review of the literature is given.

Long-term follow-up of chronic maxillary sinusitis in children.

In 26 children, aged between 3 and 7 years, the course of therapy-resistant chronic maxillary sinusitis over a mean period of 6 years and 3 months, was analysed. The results showed that spontaneous cure had occurred in 24 of the 26 children, on average after they reached the age of 7 years. The chronic character of upper respiratory tract infections in young children is difficult to explain.

Ultrasonography as a method of examination of the frontal sinus.

This prospective study was performed to evaluate the reliability of ultrasonography for the detection of fluid and mucosal swelling in the frontal sinus. The evaluation was based on a comparison of the preoperative ultrasonographic results with the surgical findings in 27 frontal sinuses. Ultrasonography was found to be a reliable method for the demonstration or exclusion of mucosal swelling and accumulated fluid (sensitivity 92%; specificity 93%).

Otitis media with effusion and chronic upper respiratory tract infection in children: a randomized, placebo-controlled clinical study.

This study was performed to investigate the course of spontaneous recovery from otitis media with effusion in children with chronic rhinosinusitis treated in various ways. One hundred forty-one children between 3 and 10 years of age were selected for the presence of chronic rhinosinusitis and unilateral or bilateral otitis media with effusion. The children were assigned at random to one of four treatment groups, i.e., placebo, amoxicillin combined with xylometazoline hydrochloride nose drops, maxillary sinus drainage, or a combination of the latter two forms of therapy. The follow-up period was 6 months. Drainage of the maxillary sinus had no effect on either the recovery of the chronic upper respiratory tract infection or otitis media with effusion. Amoxicillin combined with xylometazoline nose drops had no significant effect on recovery from the upper respiratory tract infection, but did have a small but significant effect on recovery from otitis media with effusion. However, the general tendency of the upper respiratory tract and ears to recover was poor. Persistence of the chronic upper respiratory tract infection during the follow-up period proved to be a negative prognostic factor with respect to cure of otitis media with effusion. Children with chronic rhinosinusitis as defined in this study appear to have a high risk of developing chronic otitis media with effusion. The results of the study are discussed.

The diagnostic value of transillumination for maxillary sinusitis in children.

In 52 children between 3 and 9 years of age drainage of the maxillary sinus was performed because of chronic purulent rhinitis and opacity of the maxillary sinuses on the radiographs. The results of preoperative transillumination were compared with the drainage findings. In this selected group of children the sensitivity of transillumination for the demonstration of congestion in the maxillary sinuses was found to be limited. The authors consider transillumination to be an obsolete method for this purpose in children.

Treatment of chronic maxillary sinusitis in children.

In the present study 141 children aged between 3 and 10 years and suffering from chronic maxillary sinusitis were treated non-selectively in one of 4 ways: amoxicillin combined with decongestive nose drops, drainage of the maxillary sinus, a combination of the two, or a placebo. The duration of the follow-up period was 6 months. The therapeutic effects of the 4 forms of treatment did not differ significantly. Haemophilus influenzae and streptococcus pneumoniae were the micro-organisms encountered most often in these children. The results are discussed.

Sequential administration of human and porcine factor VIII for surgical treatment of a parotid tumour in a patient with a factor VIII inhibitor.

The sequential use of human and porcine factor VIII for the treatment of a patient with a moderately high titre (38 Bethesda units) factor VIII inhibitor enabled us to provide adequate haemostatic cover for 17 days. In this period a pleomorphic adenoma of the parotid gland was removed. Bleeding did not occur and wound healing was uneventful.
