In a nonblind, randomised, parallel-group study, initial empirical monotherapy with meropenem 1 g intravenously every 8 h was compared to an identical dosage of imipenem/cilastatin for the treatment of 66 febrile episodes in 61 adult neutropenic patients. 25/31 episodes treated with meropenem and 24/30 imipenem/cilastatin-treated episodes were still receiving unmodified therapy at 72 h (primary endpoint); this difference was not statistically significant. By the end of the treatment courses, 18/31 meropenem-treated episodes had responded clinically (cured or improved) compared with 18/30 episodes treated with imipenem/cilastatin. Another ten episodes initially treated with meropenem and six episodes treated with imipenem/cilastatia were cured after an additional antimicrobial agent had been administered (cured with modification). Satisfactory bacteriological responses (eradication plus presumed eradication) at the end of unmodified therapy was 9/11 in the meropenem group and 14/16 in the comparator group. Both regimes were well tolerated; however, there were more reports of nausea and/or vomiting in the impenem/cilastatin group (7/33 vs. 2/33 in the meropenem group). The carbapenems meropenem and imipenem/cilastatin appear to be suitable agents for empirical monotherapy of febrile episodes in neutropenic patients. Meropenem may be better tolerated than imipenem/cilastatin, allowing optimal dosing in this patient population.
It is reported on a 21-year-old female with super-female syndrome in association with an early childhood brain damage exhibiting idiopathic epilepsy, severe oligophrenia, emotional instability, Little syndrome, areflexia of the legs, strabismus, telecanthus, broad nose, epicanthic fold, clinodactyly of little fingers, radio-ulnar-synostosis, retardation of ossification, aortic insufficiency, erythro-cyanosis, one-side clubfoot and nephrosclerosis, also menstrual cycle dysfunction.
Report of a boy aged 17 years and review of the literature on the neoplasias of CAS. The cause of examination was a psychic maldevelopment. Cranial arteriography exhibited extreme wide Carotis communis and CT temporal meningioma right and septum pellucidum cyste. A psychic frontal lobe syndrome and cosmetic impairment by skull changes of the CAS are discussed being partial causes of the psychic maldevelopment. Atypical there were retardation of calcification and dentition, bilateral coarctation of the auditory passage with resulting hearing loss. Till now only J. Frankel and coworkers have published the case of a 50 year old Pakistani with the combination of CAS and Meningioma as a complication of fibrous dysplasia of the skull. We suppose relations between CAS and neurofibromatosis and its blastomatous tendency to meningioma.
Among 43 female patients aged 17-46 years with almost severe oligophrenia there were four with primary hypogonadism, one of them a case of Richards Rundle syndrome, now aged 20 years with absence of secondary sex characters, hypoplastic genitals, deafness, ataxia, wasting of muscles and reduced jerks. In cases of hypogonadism and hypogenitalism should be searched for mental and neurologic disorders, also for genetic counseling of the siblings.
Report of an 18-years old mentally retarded patient with Prader-Willi syndrome, who exhibits severe disorders of behaviour including self-injurious behaviour. A psychic frontal lobe syndrome as the result of premature craniosynostosis is discussed.
Report of a case of the fetal face syndrome associated with severe oligophrenia and septum pellucidum cyste in a male patient aged 30 years. The case demonstrates the good prognosis of the hypogenitalism resp. small genitalia; the correct diagnosis in this syndrome can the patients save a plastic of the penis.
Report of an association of Saethre-Chotzen- and fragile X-Syndrome in a 19-years old imbecile with autism and aggressiveness. Both syndromes exhibit symptoms of minimal brain damage and disorders of personality.
Report of an imbecile female aged 20 years with congenital absence of the pectoral muscle (abortive Poland anomaly) at the right and peromelia of the left forearm.
A male infant with obstructive uropathy developed yeast cell agglomerations which were detectable macroscopically and by image-generating techniques within both renal pelvises after Candida albicans infection of the urinary tract. Therapy with flucytosine induced excretion of 'fungal balls' via the urethra. Continuation of therapy with liposomal amphotericin B (AmBisome) prevented a relapse after development of fungal resistance against flucytosine. Sonographically or radiographically detectable formation of 'concrements' within the urinary tract of patients with an immature or compromised immune system and additional features such as obstructive urinary tract should suggest a localized or systemic mycosis, particularly in the context of long-term antibiotic treatment.
