[The prognosis for high-risk newborns].

By good perinatal care the perinatal mortality rate can be brought down to 10 promille. A great number of high risk newborns will survive. The developmental prognosis of these at risk babies has strikingly improved. Among these babies the rate of neuromotor defects has been reduced by about 50%. Babies with birth weights of less than 1501 g will develop spastic diplegia in less than 5% and more than 80% of the babies with birth weights of less than 1001 g and the babies after artificial ventilation will develop normally.

[Severe infections in the newborn after direct kardiototography during delivery (author's transl)].

In a total of 720 CTG-controlled newborns two cases of severe septic general infections were observed; a full-term baby showed an extensive skin of the head phlegmone and meningitis purulenta, both conditions caused by Bacteria coli. A pre-term baby developed suppuration at the site of the CTG-wound and a pulmonary abscess under a general septic condition. The tracheal secretion culture of the patient contained Bacteria pyozyanea. In the case of the full-term child histologic evidence of a chorion infection was established. Both children recovered. Therefore, as we see it, the application of direct cardiotokography makes extensive antiseptic measures and thorough examination of the newborn for infectious complications imperative, a general antibiotic treatment, however, is not considered indicated.

[The mother and her sick newborn (author's transl)].

The care of the mother during the pre- and postnatal period is of great importance not only for the act of birth but also for the establishment of the mother child relationship. There are indications that the quality of mothering during later childhood is determined by the way the mother child relations are established during the first days of life. It is important to realise the needs of the mother to see and to touch her child and to enable her to do so as often as possible even if this child is under special care. In cases of neonatal illness the mother especially needs the psychological help of her doctor and the nurses. The professional persons can provide this help better if they are aware of the normal reactions of the mother to neonatal illness or even death of her child.

[Development after respiratortreatment during the newborn period (author's transl)].

There were 17 survivors of severe neonatal respiratory distress treated with IPPV for more than 24 hours (36-520 hrs, mean duration of IPPV 6 days) followed into the second year of life. 11 of these young children were physically and neurologically normal. 3 were developmentally retarded (3 months or more), 2 had neurological defects without mental subnormality. These defects (Hemiplegia and Diplegia) were correlated with low gestational ages (32 and 33 weeks respectively) 1 child was severely defective. This one was treated because of apnea caused by seizures in postmaturity syndrome (44 weeks gestation). The overall incidence of defects was 3/17 (= 17%).

[One year's experience with mechanical ventilation in newborn infants (author's transl)].

During the first year of newborn intensive care at the Landeskinderkrankenhaus Linz/Donau there were 80 cases of artificial ventilation, 62 of these were Low Birth Weight Infants (LBWI). 12% of all LBWI required mechanical ventilation. 53 ventilated infants suffered from severe Hyalin Membrane Syndrome (HMS). 17 survived, 18 died of intraventricular hemorrhage and 18 died of HMS. Bronchopulmonary Dysplasia was found in all patients ventilated for more than 7 days. 5 cases of pneumothorax occurred during ventilation. Good results were obtained in cases of respiratory insufficiency other than HMS (15 cases, 12 survivors. Mortality rate of LBWI delivered in Hospitals was 9.9%, whereas 34% of the LBWI delivered at home but cared for under identical conditions in the nursery died. This difference offers good evidence for the importance of the quality of the obstetrical care for the results of newborn intensive care.

[Neuromotor follow-up of premature infants with lowbirth-weight (author's transl)].

In the years 1971-1972 53 premature babies weighing between 1000-1500 g survived in the special department of the Linz hospital ward for premature children i.e. 45.5%. Their postnatal management and neurophysiologic follow-up are reported. 20 children were "normal", 38% had slight, 20% more pronounced developmental delay. In 81.4% there were in addition minimal disturbances of movement. 2 had severe spastic tetraplegia. The study of these data indicated ways of improvement which have been effected since.
