[The development of suicide rates in the Bezirkskrankenhaus Bayreuth 1976 - (including) 2000].

Newer publications about psychiatric in-patient suicide discuss the so-called "increase hypothesis" of suicide of mentally ill in-patients controversially. In our study of in-patient suicides of the Bezirkskrankenhaus Bayreuth, a state mental hospital for adult psychiatry, the number of suicides and suicide rate over 25 years 1979 - 2000 did not show any increase. During the last 4 - 5 years we observe a trend toward a decrease.

[Treatment of inoperable non-small cell bronchogenic carcinoma with a combination of cisplatin and vinorelbine--preliminary evaluation].

The optimal therapeutic approach to patients with inoperable non-small-cell carcinoma is still a matter of discussion. The reason is that chemotherapy improves the quality of life only in some patients. It prolongs their life only by several weeks. In recent years in this indication a new cytostatic is tested--vinorelbine which when used in monotherapy achieves a therapeutic response in 16--30%. In clinical trials of phase 2 a combination of vinorelbine and cisplatinum was most effective and therefore it was selected for the third phase of clinical trials. In the Czech Republic a clinical investigation was made with the objective to verify published data on the success of treatment, evaluate undesirable effects and consider whether it is suitable for routine use. Cisplatinum (Platidiam Lachema) was administered--80 mg/m2 on the first day, vinorelbine (Navelbine Pierre Fabre)--30 mg/m2 on the first and eighth day. The cycle was repeated on the 22nd day. All patients were treated for 12 weeks. Then followed the first evaluation and treatment was continued only in patients with regression or stabilization of the disease. In the group of 126 patients in 44 (35%) partial and in 3 (2.3%) complete therapeutic responses were obtained. In 35 (27%) the disease was evaluated during treatment as stabilized, in 38 (30%) of the patients the disease progressed despite treatment. The tolerance of treatment was, when effective antiemetic treatment was used (ondansetrone or granisetrone), relatively satisfactory.

[Psychodynamic aspects of dyspnoea attacks in childhood (author's transl)].

It was attempted to describe some important aspects of the psychodynamics of respiratory attacks in childhood. Breathing is an activity, that is obviously bound up with life itself. From this fact derives the symptom of a sudden manifestation of dyspnoea attacks, disturbed respiration, palpitation and apprehensiveness. Three forms of asphyxia are described by the patients themselves: constriction of the throat, destruction of trachea, and obstruction of the respiratory tract as a whole. Sometimes a "lump in the throat" made swallowing impossible. The bases of this sensations are fears in connection with sudden death in the family, separation or loss of parental love. Among 18 cases we found a prevalence of boys in childhood, a predomination of girls in adolescence. Sexual fears proved to be an important pathogenic factor. Five individual cases among our patients illustrate the role of the underlying influences. In acute cases hospital admission is necessary, treatment by a child psychiatrist if available. Therapeutic problems are discussed.

[Comparative evaluation of hydroxyproline in urine and in serum as a possible clinical parameter for radiation-induced destruction of connective tissue due to fractionated radiation therapy (author's transl)].

In the course of postoperative fractionated radiation therapy hydroxyproline was evaluated as a biochemical parameter of radiation damage in 60 patients with different tumour diseases. At different times before, during and after therapy, hydroxyproline in serum was evaluated according to the method of Dabew and Struck, hydroxyproline in urine according to the test combination "hypronosticon" (Organon-Technika). There was no correlation to be found between hydroxyproline in serum or urine, clinical course of disease and radiation dose. Possible explanations were discussed.

[Studies in animal experiments on determination of blood and organ levels of o-(beta-hydroxyethyl)-rutosides (HR) after i.p. administration (author's transl)].

The concentration of O-(beta-hydroxyethyl)-rutosides (HR, Venoruton) in different organs and serum was investigated after i.p. injections in C3H-mice. There was found a distinct but brief maximum level in serum within 10 to 25 min after i.p. injection. This result explains some of the positive and negative papers about the problem of radiation protection by HR. Therefore it seems important to expand knowledge on time factors and excretion mode of HR in different organs and serum to judge on the radioprotective ability of this substance.

[Clinical experience with daily doses of misonidazole (author's transl)].

In this pilot study daily low doses of misonidazole (in the range of 1 to 2 g) up to cumulative doses between 7 and 19 g/m2 were used. Serum levels were analysed at different times after administration and according to several dose regimens. We related the cumulative doses to the incidence and severity of the observed peripheral neuropathies. The aim was to find an effective daily low-dose schedule of misonidazole with a clinically acceptable incidence of side effects. Some impressive clinical responses were observed.

[Ozone-oxygen therapy for gynecologic carcinomas. The effect of parenteral-ozone oxygen mixture administration on free fatty acids and triglycerides in patients with gynecologic carcinomas].

As some authors suspect that ozone influences the metabolic process of fat, we tried to analyse the influence of an ozone-oxygen gas which was applied parenterally. 40 patients with gynecological cancer received 10 ml ozone-oxygen gas with a content of 450 gamma ozone and venous blood was removed before and 10 minutes after application. The serum was lyophilized and the level of fatty acids and triglycerids was determined by the method of Randerath (1965). a statistically significant decrease of the concentrations was observed after application of ozone. Different theories as to the cause of this action are discussed.

[Investigations into the clinical significance of beta-glucuronidase as tumour indicator during radiation therapy of patients with carcinoma of the bladder (author's transl)].

We examined the activity of beta-glucuronidase in serum and urine in two different patient groups with carcinoma of the bladder. In one group the tumour was excised totally, in the other group tumour material was left. There was no correlation to be found between extension of the tumour and enzyme activity, or between the enzyme levels in urine and serum. This seems to be due to multiple factors like postoperative restitution, cystitis and radiation effects on bladder tissue.

[On "gespür" (author's transl)].

The term "Gespür" frequently used by H. ASPERGER is analyzed by the author. Predispositions for perceptual abilities and formal interpretation of expression and behavior, their ontogenetic and phylogenetic background are examined as well as defects (lack of sensitivity for expression), and their consequences for subjects involved. The questions of trainability and differentiation to parapsychological phenomena are also discussed.

[Phospholipids under combined ozone-oxygen administration].

The parenterally application of oxygen-ozone gas mixture gives good resultats in the treatment of various deseases. Ozone seems to influence the metabolic process of fat, so it was of interest to analyse this influence especially to phospholipids. 40 women with gynaecological cancer got 10 ml oxygen-ozone gas mixture with a content of 450 gamma ozone into the cubital vene. Venous blood was removed before and 10 minutes after application and the level of lecithin, lysolecithin, cephalin and spingomyelin was determined by the method of Randerath. A decrease of all four substances was obvious, although all values remained in normal range.
