Hepatocyte growth factor gene therapy accelerates regeneration in cirrhotic mouse livers after hepatectomy.

BACKGROUND: Impaired regeneration and dysfunction of the cirrhotic liver following partial hepatectomy (PHx) are the most serious risk factors for postoperative liver failure. AIMS: Using naked hepatocyte growth factor (HGF) plasmid by the electroporation (EP) in vivo method, we investigated HGF for its role and mechanism of proliferation and restoration of liver mass in cirrhotic mice following PHx. ANIMALS: Eight week old female mice were used. METHODS: HGF plasmid 50 micro g was injected intramuscularly and transferred by EP in vivo once a week for three weeks. After establishment of carbon tetrachloride induced cirrhosis, mice underwent PHx. The HGF treated group was given naked HGF plasmid four days before PHx, and additional HGF was given once a week until they were killed, while a control group was given only empty plasmid. Mice were killed 2, 4, 10, and 14 days after PHx. Morphological and functional restoration of the liver were examined, as well as activation of mitogen activated protein kinase (MAPK) and mRNA levels of HGF activator (HGFA). RESULTS: The HGF treated group demonstrated a continuous threefold increase in HGF levels in plasma. Therapy with HGF in cirrhotic PHx resulted in effective liver regeneration via restoration of HGFA and activation of MAPK p44/p42, accelerated normalisation of liver function, and increased collagen degradation. CONCLUSIONS: HGF gene therapy by in vivo EP may be useful for hepatic resection in cirrhotic livers by stimulating liver proliferative and collagenolytic capacities, as well as accelerating functional recovery.

Attenuated acute liver injury in mice by naked hepatocyte growth factor gene transfer into skeletal muscle with electroporation.

BACKGROUND: Hepatocyte growth factor (HGF) plays an essential role in hepatic development and regeneration, and shows proliferative and antiapoptotic activity in hepatocytes. AIMS: To establish an effective new method for HGF gene transfer in vivo and to investigate its effects in acute experimental liver injury. ANIMALS: Eight week old female mice were used. METHODS: Rat HGF gene in a modified pKSCX plasmid was transferred to the tibialis anterior muscle by electroporation using a pulse generator. Four days later, plasma HGF concentrations were determined by enzyme linked immunosorbent assay every two days for three weeks. To confirm the efficacy of electroporation, a plasmid bearing green fluorescence protein (GFP) was transferred similarly. Four days after electroporation, carbon tetrachloride (CCl(4)) was administered to mice to induce acute liver injury. Plasma alanine aminotransferase (ALT) activity was measured. Hepatic apoptosis was assessed by Hoechst 33258 staining and the TUNEL method. RESULTS: Fluorescence microscopy showed strong green fluorescence where the GFP gene had been transferred into muscle. In mice given the HGF gene, HGF in plasma was increased up to fourfold from pretreatment amounts, peaking 6-9 days after electroporation and quickly decreasing within three weeks. Compared with the group without HGF transfer, the percentage of apoptotic hepatocytes after CCl(4) intoxication was significantly lower, as was ALT activity. In addition, ALT activity normalised more rapidly in the HGF gene transfer group. CONCLUSIONS: Naked DNA injection and transfer by electroporation efficiently brings about HGF expression in vivo, which can attenuate acute liver injury.

Inclusion compounds of tetrakis(4-nitrophenyl)methane: C-H...O networks, pseudopolymorphism, and structural transformations.

Tetrakis(4-nitrophenyl)methane is a new host material with considerable structural adaptability over a range of solvents. The crystal structures of 14 of these solvates have been determined and classified into three groups. The diamondoid group, wherein the host molecules form a 2-fold interpenetrated diamondoid network structure, is unprecedented in that network connections are made exclusively with weak C-H...O and pi...pi interactions. This group consists of the solvates of THF, dioxane, nitrobenzene, 4-bromoanisole, anisole, phenetole, p-xylene, and chlorobenzene. The rhombohedral group, which is characterized by specific host.guest interactions of the C-H...O and halogen...O2N type, consists of the solvates of CHCl3 and CHBr3 and somewhat surprisingly DMF, which shows an unusual 3-fold disorder mimicking in part the shape and size of the haloform molecules though not their orientation. The third group comprises solvent-rich solvates of the host with mesitylene, collidine, and o-xylene with quite different crystal structures. The THF solvate was found to lose solvent over limited temperature ranges transforming reversibly from the diamondoid structure to the rhombohedral structure. A mechanism for this process is outlined. Material from which solvent has been removed by heating was also found to resolvate upon soaking in appropriate solvents. In summary, the title compound forms a host network that is partially robust and in part flexible. It is possible that this fluxional nature of the host network derives from the weakness of the connecting interactions.

[Surgical treatment of old dislocated condylar process fractures].

OBJECTIVE: To analysis the result of surgical treatment on old dislocated condylar process fractures. METHODS: 22 cases of old dislocated condylar process fractures with a duration of more than 3 weeks (average 4.6 weeks) were treated by free and reimplantation of the dislocated condylar process. The postoperative result was evaluated on the basis of occluding relation and joint function, as well as radiographic assessment of condylar changes. RESULTS: Normal occluding relation was resumed in 19 cases after the operation. And there were 19 cases showing that the process was replaced in normal position, and homogeneous joint space and smooth cortex surface of the process were showed on pantomography also. The average mouth opening was 28.9 mm and the average mouth opening deviation was 6.3 mm before the operation reduced to 1.4 mm 6 months after the surgical treatment. CONCLUSIONS: The structure and function were recovered postoperation in 86 percent of the cases, with only slight complications in several patients. So the free and reimplantation was an effective method in treating old dislocated condylar process fractures.

Channel- and layer-type anionic host structures in inclusion compounds of urea, tetraalkylammonium terephthalate/trimesate and water

New crystalline adducts of tetraalkylammonium terephthalate/trimesate with urea and water molecules result from hydrogen-bond directed assembly of complementary acceptors and donors, and the anionic host lattices are described using the graph-set notation to identify distinct hydrogen-bonding motifs and patterns. Tetra-n-butylammonium terephthalate-urea-water (1/6/2), C46H104N14O12 (1), triclinic, space group P1, a = 8.390 (2), b = 9.894 (2), c = 18.908 (3) A, alpha = 105.06 (2), beta = 94.91 (1), gamma = 93.82 (2)o, Z = 1, is composed of hydrogen-bonded terephthalate-urea layers, which are intersected by urea layers to generate a three-dimensional network containing large channels for accommodation of the cations. Tetraethylammonium terephthalate-urea-water (1/1/5), C25H58N4O10 (2), triclinic, P1, a = 9.432 (1), b = 12.601 (1), c = 14.804 (1) A, alpha = 79.98 (1), beta = 79.20 (1), gamma = 84.18 (1)o, Z = 2, has cations sandwiched between hydrogen-bonded anionic layers. Tetraethylammonium trimesate-urea-water (1/2/7.5), C35H86N7O15.5 (3), triclinic, P1, a = 13.250 (1), b = 14.034 (1), c = 15.260 (1) A, alpha = 72.46 (1), beta = 78.32 (1), gamma = 66.95 (1)o, Z = 2, manifests a layer-type structure analogous to that of (2). Tetra-n-propylammonium hydrogen trimesate-urea-water (1/2/5), C35H78N6O13 (4), orthorhombic, Pna2(1), a = 16.467 (3), b = 33.109 (6), c = 8.344 (1) A, Z = 4, features hydrogen trimesate helices in a three-dimensional host architecture containing nanoscale channels each filled by a double column of cations.

A comparative study of the dose-response and time course of recovery of atracurium and rocuronium.

OBJECTIVE: To compare the characteristics of the dose-response relationship and the time course of action between atracurium and rocuronium in adult patients anesthetized with N2O-O2-fentanyl-thiopene. METHODS: Sixty patients, ASA grade I, aged 18-50 years, scheduled for elective plastic surgery were studied. All patients were randomly divided into either the atracurium or rocuronium group. General anesthesia was maintained with 60% nitrous oxide and 40% oxygen, thiopentone, and fentanyl. Neuromuscular function was assessed using an accelograph with train-of-four (TOF) stimulation at the wrist every 12 seconds. The percentage depression of the first twitch (T1) was used as the study parameter. The dose-response relationship of atracurium and rocuronium was determined by the cumulative dose-response technique. RESULTS: According to the dose-response curves established by a least squares linear regression, the potency ratio of atracurium to rocuronium was 1:1.2. There were significant differences in the ED50, ED90, and ED95 between the two drugs. After the intravenous administration of equipotent doses of both drugs (1.5 x ED95), the duration of peak effect, clinical duration, and total duration were significantly different between the two drugs, but their recovery indexes were similar. CONCLUSIONS: Atracurium and rocuronium are low-potency nondepolarizing relaxants with intermediate duration. As compared to atracurium, the neuromuscular blocking effect of rocuronium was about 20% less potent and its duration of action was shorter.

Crystal structures of

The crystal structures of [18]aneN(6)H(2)K[Co(CN)(6)].4H(2)O [potassium dihydrogen hexacyanocobaltate-1,4,7,10,13,16-hexaazacyclooctadecane-water (1/1/4)], [16]aneN(4)H(2)K[Co(CN)(6)] [potassium dihydrogen hexacyanocobaltate-1,5,9,13-tetraazacyclohexadecane (1/1)] and [12]aneN(4)H(3)[Co(CN)(6)].2H(2)O [trihydrogen hexacyanocobaltate-1,4,7,10-tetraazacyclododecane-water (1/1/2)] have been determined. For these supercomplexes, the [Co(CN)(6)](3-) binding with protonated polyammonium macrocycles is dominant in the binding competition between K(+) and [Co(CN)(6)](3-). It is suggested that the binding of [Co(CN)(6)](3-) with protonated polyammonium macrocycles is independent of the cavity size of the macrocycle, whereas that of K(+) is size-match selective. For [18]aneN(6)H(2)K[Co(CN)(6)].4H(2)O, the formation of a two-dimensional lamella may be derived from a chain-like arrangement of four-, five- and eight-membered rings in the network through the mutual balance between electrostatic and hydrogen-bonding interactions. [16]aneN(4)H(2)K[Co(CN)(6)] is a novel example demonstrating the anchoring of cobalticyanide through hydrogen-bonding interactions inside a zeolite-type cavity in the three-dimensional network formed by K(+) and the macrocycles through Coulombic interactions. Also, a three-dimensional network was formed mainly through hydrogen-bonding interactions between [Co(CN)(6)](3-), protonated macrocycles and water molecules in [12]aneN(4)H(3)[Co(CN)(6)].2H(2)O.

Outcome and cost comparison of percutaneous transluminal renal angioplasty, renal arterial stent placement, and renal arterial bypass grafting.

PURPOSE: To compare the outcomes and costs of percutaneous transluminal renal angioplasty (PTRA), percutaneous transluminal stent placement (PTSP) of renal arteries, and renal arterial bypass grafting (RABG) in treatment of renovascular hypertension. MATERIALS AND METHODS: Medical records and angiograms of 130 patients who underwent PTRA, PTSP, or RABG were retrospectively studied to determine success and complication rates, angiographic stenoses, blood pressures, antihypertensive medications, and serum creatinine levels. Actual costs were analyzed in detail. RESULTS: Technical success rates for PTRA, PTSP, and RABG were 91%, 98%, and 92%, respectively. Complication rates were 13%, 16%, and 38%, respectively. The mean arterial pressure was initially lowered by 29.2 mm Hg, 30.3 mm Hg, and 27.3 mm Hg, respectively, and maintained at 21.0 mm Hg, 19.8 mm Hg, and 20.2 mm Hg below baseline at 12 months. The number of antihypertensive medications was initially reduced on average by 0.63, 0.72, and 0.58, respectively, but returned to baseline in all patients by 12 months. The serum creatinine level did not change substantially with any treatment. Initial treatment costs were $1,402, $2,573, and $15,393, respectively. CONCLUSION: PTRA, PTSP, and RABG were equally efficacious for control of renovascular hypertension. The initial treatment cost for bypass grafting was substantially higher than that for PTRA and PTSP of renal arteries.

Cloning and expression of beta 2-glycoprotein 1 recognized by antiphospholipid antibodies and its clinical investigation.

OBJECTIVE: To investigate cloning and expression of beta 2-glycoprotein 1 (beta 2GP1) recognized by antiphospholipid antibodies and to study the clinical significance of anti-beta 2GP1 antibodies in patients with autoimmune disease. METHODS: By using reverse transcription-PCR method, two kinds of expression plasmid which expressed beta 2GP1 and the fifth domain of beta 2-glycoprotein 1 (beta 2GP1-D5) proteins respectively were constructed in this study. Their antigenic activities were identified by immunoblots using rabbit anti-beta 2GP1 antibodies. Anti-r beta 2GP1 and anti-r beta 2GP1-D5 antibodies in the 112 patents were detected by ELISA using r beta 2GP1 and r beta 2GP1-D5 as coating antigens. RESULTS: A significant correlation in statistics (r = 0.667, P < 0.01) between the levels of anti-r beta 2GP1 and anticardiolipin antibodies (aCL) was found. The presence of anti-r beta 2GP1 antibodies was associated with an increased frequency of history of thrombosis and/or recurrent abortion. Anti-r beta 2GP1 assay provided better specificity than conventional aCL assay. The binding of anti-r beta 2GP1 from the sera of patients with antiphospholipid syndrome (APS) to r beta 2GP1 was inhibited by r beta 2GP1-D5. Meanwhile, of 28 patients who had positive anti-r beta 2GP1 antibodies in sera, 27 (96.4%) had positive anti-r beta 2GP1-D5 antibodies. CONCLUSION: It is indicated that antigenic epitope of beta 2 GP1 might be located in its fifth domain. Detection of anti-r beta 2GP1 antibodies may be of potential value for evaluating the risk of thrombosis and/or other APS associated symptom.

A surveillance study on penicillin-resistant streptococcus pneumoniae in China.

OBJECTIVES: To find out whether there are also penicillin-resistant streptococcus pneumoniae and the resistant rate in China. METHODS: A surveillance study which is a part of the international surveillance on pneumococci resistance to penicillin and other antimicrobial agents was conducted in Beijing, China. More than 900 pediatric patients with respiratory tract infections aged from six months to three years selected from two pediatric units were enrolled in the study. Perthroat swabs were immediately streaked onto blood agar plates. Isolates were identified as pneumococci by their typical appearance, gram stain, confirmation tests. Antibiotic susceptibility was assessed by the disk diffusion method and minimal inhibition concentration (MIC) determination according to Protocol and National Committee for Clinical Laboratory Standards (NCCLS). RESULTS: Of the 51 streptococcus pneumoniae isolates, 5 strains had zones of inhibition indicative of penicillin resistance, of which two had penicillin MICs > or = 0.1 mg/L and were considered to be relatively or fully resistant to penicillin. The MICs of two penicillin resistant strains were 2 mg/L and 4 mg/L. Resistant rates to ceftriaxone and cefotaxime were 0% and 10% respectively. For the other ten antimicrobial agents, the resistant rates were as follows: ampicillin 12%, piperacillin 26%, furbenicillin 14%, cefuroxime 6%, erythromycin 54%, clarithromycin 52%, meleumycin 64%, roxithromycin 52%, chloramphenicol 35%, tetracycline 100%. CONCLUSIONS: The study suggested that penicillin resistant streptococcus pneumoniae strains existed in China and were also resistant to ampicillin, piperacillin, furbenicillin and cefuroxime. Moreover, the resistant rates of penicillin resistant streptococcus pneumoniae to macrolides and tetracycline were high.

[Effect of acute moderate isovolumic hemodilution on the pharmacokinetics of vecuronium].

OBJECTIVE: To determine the effect of acute moderate isovolemic hemodilution on pharmacokinetics of vecuronium. METHODS: Twenty-six adult patients, ASA grade I, undergoing elective plastic surgery, were randomly divided equally into the control and hemodilution group. The blood samples were taken for 5 hours following intravenous bolus of 100 micrograms/kg of vecuronium in two groups. The plasma concentrations of vecuronium were determined with an improved fluorometry and the pharmacokinetic parameters were obtained by fitting the data with a 3P87 program. RESULTS: The disposition of vecuronium can be best described by a three compartment open model. As comparison with the control group, the values of Vc and Vdss in hemodilution group were greater than that in the control, while T1/2 beta was markedly prolonged in this group. There were no significant differences in T1/2 phi, T1/2 alpha, K12, K21, K13, K31, K10, MRT, AUC, and Cl between the two groups. CONCLUSION: Acute moderate isovolemic hemodilution could change the pharmacokinetics of vecurinium, particularly in distribution and elimination phases.

Electrochemical treatment of lung cancer.

A pilot study of electrochemical treatment (ECT) as a therapy for 386 patients with nonsmall cell lung cancer was undertaken. There were 103 stage II cases, 89 stage IIIa cases, 122 stage IIIb cases, and 72 stage IV cases. Two ECT methods were used: For peripherally located lung cancer, platinum electrodes were inserted transcutaneously into the tumor under x-ray or CT guidance. For central type lung cancer or for those inoperable during thoracotomy, electrodes were inserted intraoperatively directly into the cancer. Voltage was 6-8 V, current was 40-100 mA, and electric charge was 100 coulombs per cm of tumor diameter. The number of electrodes was determined from the size of cancer mass, because the diameter of effective area around each electrode is approximately 3 cm. The short-term (6 months after ECT) results of the 386 lung cancer cases were: complete response (CR), 25.6% (99/386); partial response (PR), 46.4% (179/386); no change (NC), 15.3% (59/386); and progressive disease (PD), 12.7% (49/386). The total effective rate (CR + PR) was 72% (278/386). The 1, 3, and 5 year overall survival rates were 86.3% (333/386), 58.8% (227/386), and 29.5% (114/386), respectively. The main complication was traumatic pneumothorax, with an incidence rate of 14.8% (57/386). These clinical results show that ECT is simple, safe, effective, and minimally traumatic. ECT provides an alternative method for treating lung cancers that are conventionally inoperable, that are not responsive to chemotherapy or radiotherapy, or that cannot be resected after thoracotomy. Long-term survival rates suggest that ECT warrants further investigation.

Effectiveness of electrochemical therapy in the treatment of lung cancers of middle and late stage.

OBJECTIVE: To investigate the effect of electrochemical therapy (ECT) in the treatment of middle and late-stage lung cancers. METHODS: Three hundred and eighty-six cases (287 males md 99 females aged 25-78, with an average age of 51 years) with middle and late-stage lung cancers were treated with ECT. Two hundred and three patients had squamous cell carcinoma, 138 adenocarcinoma and 45 undifferentiated cancer. Diameters of the cancers were respectively 4.0-6.0 cm in 153 cases, 6.1-8.0 cm in 82 cases, 8.1-10.0 cm in 102 cases and over 10.0 cm in 49 cases. In this group, none was at stage I, 103 cases were at stage II, 89 cases at IIIa, 122 cases at IIIb and 72 cases IV. Of the 386 cases, 152 cases (39.4%) also suffered from hypertension, heart disease etc. Anode and cathode platinum electrodes were inserted accurately into the tumour mass. Distance between two electrodes was 2.0-2.5 cm. Electrodes were connected to a special ECT instrument. The current was maintained at 6-8 V and 80-100 mA. One hundred coulombs are used for treating 1 cm diameter of tumour mass. RESULTS: Of the 386 cases, 99 cases (25.6%) achieved complete remission (CR). 179 cases (46.4%) achieved partial remission (PR), 59 cases (15.3%) had no change (NC) and 49 cases (12.7%) had progression of disease (PD). Effective rate CR + PR) was 72% (278 cases). One to five year survival rates were 86.3%, 76.4%, 58.8%, 39.9% and 29.5%, respectively. CONCLUSIONS: ECT is easy to use. It is effective, safe, less traumatic and makes patients recover quickly. This is a new and effective method to treat patients with tumours who are inoperable and can not receive chemotherapy or radiotherapy.

[A recombinant autoantigen U 1 RNP 70,000: expression in E. coli. and its serodiagnostic application].

OBJECTIVE: To express the recombinant autoantigen U 1 RNP 70,000 and to evaluate its serodiagnostic value. METHOD: We used molecular cloning techniques and immunoblotting technique. RESULTS: The recombinant proteins had authentic U 1 RNP 70,000 antigenicity, with the molecular weight corresponding to their theoretical value. 112 of 115(97.4%) anti-70,000 positive sera(IBT) could be detected by r 70,000. CONCLUSIONS: r 70,000 has good specificity and can be used in further detailed epitopes mapping. r 70,000 can be put into use for serodiagnostic detection.

[A pharmacokinetic study of lidocaine in patients undergoing liposuction with tumescent technique].

The purpose of this study was to evaluate the pharmacokinetics of lidocaine in the patients undergoing liposuction after subcutaneous infiltration of 0.06% lidocaine. It was found that the absorption of lidocaine was slow. The mean peak concentration of lidocaine that was 1.16 ug/mg with a range of 0.67 to 2.84 ug/mg occurred 4-13 hours after lidocaine administration. The peak concentration (Y) was correlated with the dose (X) of lidocaine, which was drawn as Y = -0.494 + 0.071X (r = 0.934, P < 0.001). We concluded that the tumescent technique is an effective and safe method for the patients undergoing liposuction.
