Sources and distribution of trace metals in the Saricay Stream basin of southwestern Turkey.

Seasonal variation of the concentrations of trace metals (Cd, Co, Cr, Cu, Fe, Mn, Ni, Pb and Zn) were measured by ICP-AES in the water and sediment from the Saricay Stream, Geyik Dam and Ortakoy Well in the same basin. Comparisons between trace metal concentrations in water and sediment in three sources (Stream, Dam and Well) were made. The concentrations of a large number of trace metals in the water and sediment were generally higher in the Stream than in the Well and Dam, particularly in summer. Trace metal concentration ranges in sediments of the Saricay Stream and its sources showed very wide ranges (as mass ratio): Co: 5-476 microg g(-1), Cr: 15-1308 microg g(-1), Cu: 7-128 microg g(-1), Fe: 1120-13210 microg g(-1), Mn: 150-2613 microg g(-1), Ni: 102-390 microg g(-1), Pb: 0.7-31.3 microg g(-1) and Zn: 18-304 microg g(-1), whereas Cd was not detected. Trace metal concentration ranges found in waters were: Co: 9.5-20.7 microg L(-1), Cr: 20.3-284 microg L(-1), Cu: 170-840 microg L(-1), Fe: 176-1830 microg L(-1), Mn: 29.3-387 microg L(-1), and Ni: 4.3-21.9 microg L(-1). Among the trace metals studied, Cd and Zn in two seasons and Pb in winter were usually not detected or in the recommended levels. In addition, Cd was not detected in the sediment during the winter season. The analysis of variance (one-way ANOVA) and correlation matrix was employed for the sediment and water samples of the two field surveys (summer and winter) comparison. The three sources showed differences in metal contents. The metal levels in sediments displayed marked seasonal and regional variations, which were attributed to anthropogenic influences and natural processes. In the Saricay Stream, high values of metals during the dry season showed an anthropological effect from small industry firms, e.g.: an olive mill and a dairy farm or water dilution during summer seasons. Finally, the pollution in this basin probably originated from small industrial, low quality coal-burned thermal power plants, and particularly agricultural and domestic waste discharges.

The incidence and management of acute and chronic rejection after living donor liver transplantation.

Living donor liver transplantation (LDLT) is a good alternative to cadaveric liver transplantation for end-stage liver disease. Herein we report the outcome of 132 LDLTs performed between 1999 and 2005, with special emphasis on the incidence and management of acute and chronic rejection. Among the LDLT population a first acute rejection episode (ARE) was clinically suspected in 24% and proven by liver biopsy in 11%. According to the Banff classification, 50% of AREs were grade 1, and 50%, grade 2. There was no grade 3 AREs. The first ARE occurred between 7 days and 23 months posttransplantation (mean 97 days, median 70 days). Ninety-seven percent (31/32) of the AREs occurred within the first year after transplantation and 3% (1/32) in the second year. Among the patients with ARE, 23% developed a second ARE between 4 and 11 months. A third ARE was detected in 8% of patients after month 18. All AREs responded to adjustment of immunosuppressive doses or steroid boluses. Chronic rejection (CR) was detected in 2%. In conclusion, the incidences of ARE and CR are consistent with the previously reported data. Acute and chronic rejections seem to be mild and easily manageable clinical conditions. Our results also showed a significant difference between clinically suspected and biopsy-proven ARE emphasizing the importance of indicated liver biopsies in the management of the LDLT population.

A randomized clinical trial of magnesium sulphate as a vehicle for nebulized salbutamol in the treatment of moderate to severe asthma attacks.

Although it is well known that intravenous administration of MgSO4 as an adjunct to conventional therapy is effective in treating asthma attacks, the effect of nebulized MgSO4 as a vehicle for salbutamol has been less evaluated. The aim of this study was to compare the effects of nebulized salbutamol administrated through either MgSO4 or isotonic saline solution on the 'peak expiratory flow rate' (PEFR), other respiratory and clinical parameters, and hospitalization rate of patients suffering from moderate to severe asthma attacks. Twenty-six patients with asthma attack were enrolled in the study in a randomized single blind fashion. After obtaining initial peak expiratory flow measurements (PEFR) and clinical evaluation, all patients received 1mg/kg corticosteroids and oxygen therapy and then either isotonic MgSO4 (2.5 ml, 6.3%)+salbutamol (2.5 ml) or saline (2.5 ml)+salbutamol (2.5 ml) through a jet nebulizer (group 1 (n=14) vs group 2 (n=12), respectively). The nebulizations were repeated every 20 min for the first hour and every hour for the rest of 4 h. The PEFR measurements and clinical assessment were performed after nebulization at 20th, 60th, 120th, 180th and 240th minutes. Patients were discharged when PEFR reached the target level of 70% of predicted. The baseline PEFRs and clinical parameters were similar between groups 1 and 2 (50.2+/-18.5 vs 44.1+/-13.9, respectively, p>0.05). The mean% increase in PEFR at different measurement levels was similar between the groups. When the treatment response was evaluated within the groups, group 2 showed statistically significant increase in PEFR (% of predicted) 1h earlier than group 1 (60th vs 120th minute, p=0.003 vs p=0.007). The mean duration of achieving target-PEFRs was 105.7+/-72.1 min for group 1 and 118.3+/-96.7 min for group 2 (p>0.05). This study suggested that the additional usage of MgSO4 to nebulized salbutamol has no beneficial effect on the treatment of asthma attacks.

Primary cardiac lymphoma in a heart transplant recipient.

Posttransplantation lymphoproliferative disease (PTLD) is one of the most serious complications of chronic immunosuppression in transplant recipients. Involvement of the cardiac allograft or development of lymphoma in the heart is extremely rare. We report a primary cardiac lymphoma that developed about 14 months after the operation in a cardiac recipient. The patient presented with vague abdominal complaints. Multiorgan failure developed within a short period of time, and the patient died. The diagnosis of "diffuse large cell lymphoma of B cell type" was made on postmortem examination.

Primary hepatic marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue type in a liver transplant patient with hepatitis B cirrhosis.

We describe the clinical, histological, and immunohistochemical features of primary hepatic low grade B-cell lymphoma of mucosa-associated lymphoid tissue (MALT) in a liver transplant recipient with hepatitis B cirrhosis. MALT lymphomas arise in organs normally devoid of lymphoid tissue, which accumulates as a consequence of chronic antigenic stimulation associated with chronic infection or autoimmune disease. Primary hepatic MALT lymphoma is extremely rare; 13 cases have been reported worldwide to date. Our patient is the first case of primary hepatic MALT lymphoma associated with hepatitis B cirrhosis who was treated with orthotopic liver transplantation.

Detection of infectious laryngotracheitis virus in trigeminal ganglia by avidin-biotin complex method in chickens: short communication.

Detection of infectious laryngotracheitis virus (ILTV) by avidin-biotin complex (ABC) method was studied in trigeminal ganglia (TRG) during the acute and post-acute or latent period in chickens inoculated with an A96 strain of the antigen. TRG tissue samples were collected from uninoculated (10 chickens) and inoculated (30 chickens) animals at various intervals post inoculation (PI), and stained by the ABC method. The results indicated that no ILTV antigen was detected in the TRG between PI days 3 and 11. However, the antigen was detected in most of the chickens between PI days 13 and 41. It is concluded that the ABC method can be used successfully for the detection of ILTV antigens in TRG during post-acute or latent period.

Echocardiographic and morphologic examination of left ventricular false tendons in human and animal hearts.

False tendons are thin, fibrous or fibromuscular structures that traverse the cavity of the left ventricle with no connection to the valvular cusps; they may be single or multiple. We retrospectively analyzed echocardiograms for the prevalence of false tendons in the hearts of 368 (231 male, 137 female) newborns, infants, and children (mean age = 6.28 +/- 4.32 years) who were referred for echocardiography because of suspected acquired or congenital heart disease, but in whom no cardiac pathology was found. In addition, we studied the prevalence of false tendons in 90 hearts from three species of animals (dog, sheep, goat) and eight cadaveric human hearts. In our echocardiographic study, false tendons were detected in 97 of 368 hearts (26.4%). In our gross morphologic studies, false tendons were observed in most of the animal and human hearts: they were present in 5 of 8 (62.5%) human hearts, 14 of 20 (70%) dog hearts, 41 of 50 (82%) sheep hearts, and 16 of 20 (80%) goat hearts. The overall prevalence in animal hearts was 71 of 90 (78.8%). Histologic examination showed the false tendons to be composed of cardiac muscle, blood vessels, fibrous tissue, and Purkinje cells. The possible role of false tendons in innocent murmurs, cardiac rhythm disorders, or left ventricular dysfunction is discussed.

Hemophagocytic syndrome after living-related liver transplantation.

Hemophagocytic syndrome (HPS) is a life-threatening hematological disorder in immunocompromised patients. Although the number of patients with HPS following liver transplantation is scarce the outcome is usually fatal. We report a patient who developed HPS following living-related liver transplantation (LRLT) and was treated successfully by a combination of intravenous (IV) immunoglobulin (Ig) and granulocyte colony-stimulating factor (GCSF).

Prevention of spinal epidural fibrosis by recombinant tissue plasminogen activator in rats.

STUDY DESIGN: A randomized-controlled experimental study was conducted to investigate the effectiveness of recombinant tissue plasminogen activator (rt-PA) on postlaminectomy epidural fibrosis in rats. SETTING: Diyarbakir, Turkey. METHODS: In a rat model, allowing objective histopathological quantification, the postoperative epidural scar formation was investigated and the potential of local rt-PA in preventing the production of epidural fibrosis was assessed. Rats were randomly allocated to local rt-PA (12 rats) or control (12 rats). RESULTS: Findings suggest a beneficial effect of rt-PA in decreasing the epidural fibrosis following laminectomy when compared with control groups for all investigated parameters such as intermuscular scar (P=0.04), middle scar (P=0.001), deep scar (P=0.001) and dural adhesion (P=0.01) except new bone formation. The presence of arachnoiditis was less in treatment group (P=0.01). CONCLUSION: In rats, the topical thrombolysis with rt-PA is safe and efficacious in preventing postlaminectomy epidural fibrosis. This Thrombolytic therapy with rt-PA after spine surgery may come to play an important role in the prevention of epidural fibrosis and arachnoiditis.

Echinococcosis multilocularis in south-eastern Anatolia, Turkey.

OBJECTIVE: To quantify the human prevalence, elucidate possible risk factors and present other epidemiological parameters of human alveolar echinococcosis in the south-eastern region of Turkey. DESIGN: A retrospective study. SETTINGS: South-eastern region (Anatolia) of Turkey by two medical centres: Dicle University Medical Faculty in Diyarbakir and Harran University Medical Faculty in Urfa. SUBJECTS: Forty seven cases of human alveolar echinococcosis were presented between the period of 1980 and 2000. METHODS: For the majority of our patients, personal anamnestic, clinical data and addresses were available from their files, and we visited their villages to investigate the risk factors for human alveolar echinococcosis. Epidemiological parameters were evaluated. RESULTS: The 47 cases consisted of 18 (38.3%) males and 29 (61.7%) females. The average age was 34.6 years. In all cases, lesions occurred in the liver. No village or district had more than one case. The majority of patients (73.8%) were farmers and 33 had a history of dog ownership. CONCLUSION: This region has a higher incidence than Turkey as a whole, and domestic dogs and free-ranging dogs seem to be the main source of infection.

Malignant ovarian germ cell tumors: analysis of 32 cases.

OBJECTIVE: In this study, some clinicopathologic characteristics and the outcome of patients with malignant ovarian germ cell tumors (MOGCT) were evaluated. MATERIALS AND METHODS: The clinical charts and pathologic reports of 32 patients with MOGCT treated at the Department of Obstetrics and Gynecology, and diagnosed at the Department of Pathology, Medical Faculty of Dicle University, Turkey from 1983 to 1999 were reviewed. RESULTS: Thirteen patients (40.6%) had dysgerminoma, nine (28.1%) had immature teratoma (four grade 1, three grade 2, and two grade 3), eight (25%) had endodermal sinus tumor, and two (6.3%) patients had mixed germ cell tumors. Site of involvement was unilateral in 30 (19 on the right and 11 on the left) and bilateral in two. All patients underwent primary surgery and 26 patients combination chemotherapy. There seemed to be a relationship between pathologic findings and clinical outcome, and MOGCT histologic types may affect the prognosis. CONCLUSION: Dysgerminoma had a better prognosis than the nondysgerminomatous group (p < 0.05). This study provides additional data in confirmation of previous reports that management of MOGCT with fertility preservation is safe.

Allopurinol hypersensitivity syndrome as a cause of hepatic centrilobular hemorrhagic necrosis.

Extensive and severe hepatic centrilobular hemorrhagic necrosis is a common finding in hepatic vein obstruction and Budd-Chiari syndrome. Some drugs, including allopurinol, can also cause this histopathologic appearance but to our knowledge in this setting the lesions are not so massive. Here we report a case of a 41-year-old female who developed fever, pruritic skin rash, jaundice, eosinophilia, abnormal liver function tests, and acute renal failure 3 weeks after the beginning of allopurinol treatment, complicated with severe hepatocyte necrosis around most terminal hepatic venules suggesting Budd-Chiari syndrome.

The evaluation of temporomandibular joint disc position in TMJ disorders using MRI.

The purpose of this study was to correlate disc position and the type of disc displacement, intra-capsular effusion and degenerative changes of the condyle as demonstrated in MRI studies. In this study, 126 temporomandibular joints (TMJs) of 63 patients with TMJ disorders were investigated using clinical examination and MRI. One hundred and twelve TMJs were found to have internal derangement as disc displacement. The angle between the posterior margin of the disc and the vertical line drawn through the centre of the condyle was measured on MRI for each TMJ. The positions of the discs were normal, 0 degrees-10 degrees, in 11.11%; slightly displaced, 11 degrees-30 degrees, in 37.30%; mildly displaced 31 degrees-50 degrees, in 15.08%; moderately displaced, 51 degrees-80 degrees, in 7.14% of the TMJs with anterior displacement with reduction (ADDR). The disc position was severely displaced anteriorly, as over 80 degrees, in all TMJs with anterior disc displacement without reduction (ADD), constituting 27.78% of all cases. We found that the smaller the degree of disc displacement the milder the internal derangement and that the intra-capsular effusion was more frequently associated with TMJ with ADDR. The degenerative condylar changes were more severe with an increased degree of anterior disc displacement.

Cavernous hemangioma of the uterus (a case report).

Cavernous hemangioma of the uterus is an extremely rare lesion. We report a postmenopausal patient with abnormal uterine bleeding due to hemangioma and simple endometrial hyperplasia.

Expressions of p53, proliferating cell nuclear antigen, and Ki-67 in gestational trophoblastic diseases.

OBJECTIVE: This study was done to determine whether the expressions of p53, PCNA, and Ki-67 could differentiate spontaneous abortions with hydropic changes from gestational trophoblastic diseases. MATERIALS AND METHODS: Twenty partial hydatidiform moles, 21 complete hydatidiform moles, nine invasive hydatidiform moles, three choriocarcinomas and 19 first trimester hydropic spontaneous abortions were evaluated by means of immunohistochemical methods with antibodies to p53, PCNA, and Ki-67 in this study. RESULTS: The Ki-67, PCNA, and p53 immunoreactivity was significantly higher in the gestational trophoblastic disease group than in the spontaneous abortion group with hydropic changes. None of the three parameters provided reliable discrimination among gestational trophoblastic disease subgroups. CONCLUSION: Our findings suggest that expressions of Ki-67, proliferating cell nuclear antigen and p53 can be used to differentiate between spontaneous abortion with hydropic changes and gestational trophoblastic disease when all three markers are used together.

Heavy metal contents in some macrofungi collected in the northwestern part of Turkey.

Eight metal contents were determined in wild growing macrofungi collected from two different areas in Balikesir and Akhisar (NW Turkey). The analysis has been done by atomic absorption spectrophotometry in 179 samples of 16 species. Pb was accumulated extensively by G. sessile and L. cinerascens in both areas. Concentration of Cd was significantly high in A. pseudopratensis from background area and M. excissa from near road area. The high concentrations of Fe and Zn were found in M. ramealis and L. cinerascens for both areas. The mean concentration of Cu was high in V. pratense from background area and C. butyracea var. butyracea from near road area. The highest mean concentrations of Co and Mn for both areas were seen in G. sessile and M. excissa, respectively. A high Ni concentration was found in G. sessile and H. hypothejus from background area. We found no considerably significant differences in the concentrations of all studied elements between two areas.

Inflammatory myofibroblastic tumor of small bowel wall in childhood: Report of a case and a review of the literature.

Benign intestinal tumors are rare in children, however we describe an inflammatory myofibroblastic tumor (IMT) of the jejunum in a 2-year-old girl who presented with an intestinal obstruction. During laparotomy, an annular mass around the jejunum was resected, from which a histological diagnosis of IMT was made. A review of the literature for this rare entity emphasizes the importance of histological confirmation of its benign nature. Because of the risk of local recurrence, IMT cases should have a long-term follow up.
