[Extraperitoneal cesarean section--an alternative or routine?].

OBJECTIVE: Evaluation of the importance of extraperitoneal Caesarean section in contemporary obstetrics. DESIGN: Review. SETTING: Gynaecological and Obstetrics Department, Bata Hospital Zlín. SUBJECT: A historical review of views on the approach to the lower uterine segment in Caesarean section and definition of some myths associated with extraperitoneal Caesarean sections. Retrospective analysis of the development of indications for this operation in 1977-1999 and their possible advantages. RESULTS: Analysis of 318 operations proved possible advantages of the operation during the perioperative period as well as during convalescence. CONCLUSION: Extraperitoneal Caesarean section can be considered an alternative of other surgical methods and procedures in infected cases and also an operation which can be implemented under all conditions except peracute states. The limiting factor is in particular the surgeon not familiar with the extraperitoneal technique. It is an operation with reduced invasiveness.

Intravenous radionuclide angiography after unilateral hypogastric artery ligation. Suture material and recanalization.

Pelvic angioscintigraphic examination of seven patients after unilateral hypogastric artery ligation has been performed. Immediately after the operation asymmetric perfusion has been observed in all women. Subsequent control examination of 4 patients ligatured by nonabsorbable suture has shown persisting unilateral hypoperfusion. Angioscintigraphy of 3 patients with ligature, done by absorbable suture proved normalization of hypoperfusion and restoration of symmetry.

Thermography of gluteal regions after hypogastric artery ligation.

Thermographic examination of buttocks of 7 patients after hypogastric artery ligation using non-absorbable suture has been performed. In 6 patients the possibility of eventual recanalization has been eliminated by aortography, in 1 patient by pelvic angioscintigraphy. Repeated thermographic examination of 3 patients after bilateral ligation has shown no anomalous results. In 1 among 4 patients after unilateral ligation a cooler area could be repeatedly observed on the ipsilateral side. Thermography, in combination with other methods of examination, seems to be suitable for complex study of the state of circulation after hypograstric artery ligation, performed to stop a life-threatening hemorrhage.
