A recently developed 99mTc radiocompound, hexamethyl-propyleneamine oxime (99mTc-HM-PAO) exhibits favorable properties for regional cerebral tomograms in man utilizing conventional instrumentation (SPECT). Planar and tomographic studies using a rotating gamma camera equipped with a high sensitivity, low energy, collimator were performed in 5 normal subjects and 20 patients suffering from different cerebral diseases. SPECT abnormalities observed in patients with CVD, brain tumors and hydrocephalus were compared with results from X-ray CT. Our preliminary study demonstrates the feasibility of assessing regional brain perfusion using SPECT and a 99mTc radiopharmaceutical which is lipid soluble and has a high extraction fraction in the brain.
A recently developed Tc-99m radiocompound, hexamethylpropyleneamine oxime (Tc-99m HM-PAO), exhibits favorable properties for regional cerebral blood flow study in man. The authors present a case of luxury perfusion syndrome observed in a 72-year-old patient with acute stroke and a right-sided hemiplegia, documented by planar scintigraphy and SPECT study in correlation with CT scan. The metabolic basis of this phenomenon is discussed and the usefulness of assessing regional brain perfusion by Tc-99m HM-PAO with conventional nuclear medicine equipment is underlined.
The effects of a synthetic vasopressin analogue, DDAVP (1-deamino-8-d- arginine-vasopressin) were studied in 5 adults and 2 children with acute central diabetes insipidus secondary to neurosurgery. 5 mcg DDAVP was administered intranasally twice a day to the two children, whereas the adults received the drug i.m. (1 mcg twice a day, 3 patients; 1 mcg once a day, 1 patient; 4 mcg twice a day, 1 patient). All subjects displayed an early, prolonged response (12-24 hr). There were no side-effects. The effectiveness of DDAVP, its prolonged action, and virtual freedom from side-effects when given intranasally or parenterally make it the drug of choice in the treatment of acute neurosurgical diabetes insipidus.
Plasma and ventricular cerebro-spinal fluid (CSF) Beta-endorphin concentrations were evaluated after chromatographic separation in patients carrying a ventricular shunt before and after the administration of diclofenac or placebo. In the same subjects the ventricular CSF concentrations of the serotonin and the catecholamine metabolites 5-hydroxyindole-acetic acid (5-HIAA), homovanillic acid (HVA) and MOPEG were also evaluated. Plasma, but not ventricular, Beta-endorphin concentrations increased significantly after diclofenac, while placebo was ineffective. No significant changes in ventricular 5-HIAA, HVA or MOPEG levels were observed. These data suggest a role for Beta-endorphin in the analgesic effect of diclofenac.

Four children with migraine raised some problems of differential diagnosis (cerebral circulatory disorders; structural brain diseases; or primary psychiatric disorders). Organic lesions were excluded through special investigations (EEG, CAT, Scintigraphy, Angiography etc.). Relationships between migraine and epilepsy in children were considered and the recent literature was reviewed. No agreement was reached on how to evaluate paroxysmal electroencephalograms recorded during migrainous episodes. In a child with personal and family history of migraine suffering from a sudden attack of acute headache accompanied by focal neurological deficits, migraine is the most likely diagnosis. Only if the neurological deficit fails to resolve rapidly (after a few hours), a detailed investigation is necessary (CAT etc.). A paroxysmal electroencephalogram during the attack of migraine does not imply necessarily a diagnosis of epilepsy. Children with history of migraine must be treated with specific anti-migranious drugs even if paroxysmal EEGs are recorded during the migraine attack. It must be remembered that both disorders (i.e. migraine and epilepsy) can exist in the same patient.
