ECG and XML: an instance of a possible XML schema for the ECG telemonitoring.

Management of many types of chronic diseases relies heavily on patients' self-monitoring of their disease conditions. In recent years, Internet-based home telemonitoring systems allowing transmission of patient data to a central database and offering immediate access to the data by the care providers have become available. The adoption of Extensible Mark-up Language (XML) as a W3C standard has generated considerable interest in the potential value of this language in health informatics. However, the telemonitoring systems often work with only one or a few types of medical devices. This is because different medical devices produce different types of data, and the existing telemonitoring systems are generally built around a proprietary data schema. In this paper, we describe a generic data schema for a telemonitoring system that is applicable to different types of medical devices and different diseases, and then we present an architecture for the exchange of clinical information as data, signals of telemonitoring and clinical reports in the XML standard, up-to-date information in each electronic patient record and integration in real time with the information collected during the telemonitoring activities in the XML schema, between all the structures involved in the healthcare process of the patient.

MobiDis: Toward a Patient Centric Healthcare Information System.

The paper presents some results of the MobiDis project. MobiDis is an information system that includes healthcare consumers and providers in a unique, virtual organisation aimed at promoting a patient centric paradigm in healthcare. It allows logons from desktop or laptop computers, as well as wireless PDAs or tablet PCs connected to Internet. In MobiDis the clinical data of each consumer are stored in the consumer's virtual healthcare record (VHR), a highly structured entity that exists on the network and is simultaneously updated with information from multiple locations. The MobiDis architecture creates an environment for VHRs by providing them with a large variety of services. In order to prove that our proposed architectural solution meets the project goals a prototype was developed. The paper describes the MobiDis architecture and the VHR services, and briefly presents the prototype.

The Italian national telemedicine programme.

The Italian national telemedicine programme included a broad range of research projects, but they all served the common purpose of bringing about improvements in health-care management and performance. The programme consisted of seven projects, each of which had specific research and training objectives, and a three-year duration. The systems developed in the course of the programme were not experimental prototypes:they were intended to be pre-commercial systems. The functional and clinical merits of the products and systems developed were evaluated, and their costs and benefits measured with reference to those already in use. The seven projects were completed in 1999. The Italian Ministry of Universities, Research and Technology granted research funds totalling 50,000,000 Euros for the whole programme. While the results of the research were promising, successful projects alone are not sufficient to reduce costs in health-care. Indeed, far more money can be saved simply by encouraging the uptake on a larger scale of many technologies and practices that already exist.

WITH: a system to write clinical trials using XML and RDBMS.

The paper illustrates the system WITH (Write on Internet clinical Trials in Haematology) which supports the writing of a clinical trial (CT) document. The requirements of this system have been defined analysing the writing process of a CT and then modelling the content of its sections together with their logical and temporal relationships. The system WITH allows: a) editing the document text; b) re-using the text; and c) facilitating the cooperation and the collaborative writing. It is based on XML mark-up language, and on a RDBMS. This choice guarantees: a) process standardisation; b) process management; c) efficient delivery of information-based tasks; and d) explicit focus on process design.

Sharing clinical information. principles and task-oriented solutions.

Increasing cooperation among health professionals--within and across organizations--require a suitable sharing of clinical information from heterogeneous (electronic) documentation. Information originates from healthcare activities and may be organized within record systems in relation to health issues, episodes of care, episodes of illness, etc. Implementation of record systems depends on tasks and attitudes within each particular healthcare environment, that determine (i) the balance among functions of the record system, e.g. supporting human memory and decision making, supporting workflow management, recording circumstances about stored data, (ii) the particular organization of a record, (iii) the details of clinical statements that should be explicit or understood. In this paper we present a set of features of record systems and of their context that affect sharing of clinical information.

The Cadmio XML healthcare record.

The management of clinical data is a complex task. Patient related information reported in patient folders is a set of heterogeneous and structured data accessed by different users having different goals (in local or geographical networks). XML language provides a mechanism for describing, manipulating, and visualising structured data in web-based applications. XML ensures that the structured data is managed in a uniform and transparent manner independently from the applications and their providers guaranteeing some interoperability. Extracting data from the healthcare record and structuring them according to XML makes the data available through browsers. The MIC/MIE model (Medical Information Category/Medical Information Elements), which allows the definition and management of healthcare records and used in CADMIO, a HISA based project, is described in this paper, using XML for allowing the data to be visualised through web browsers.
