[Effects of the implementation of Universal Access with Explicit Guaranties (AUGE) Plan on the quality of care of patients with terminal renal failure].

BACKGROUND: The implementation of the AUGE plan for renal failure in Chile in August 2002, generated larger waiting list for outpatient care. AIM: To analyze the incidence of terminal renal failure, the proportion of patients that were admitted to hemodialysis using a definitive vascular access and the lapse of use of transitory catheters, before and after the implementation of AUGE in Calama. MATERIAL AND METHODS: Since 1999, in a dialysis center of Calama, all new patients that are admitted to hemodialysis and the type of vascular access they have are registered. Using this registry, the incidence of terminal renal failure and the lapse between the admission to the center and the installation of a definitive vascular access were calculated for the period 2000 to 2005. RESULTS: From January 2000 to December 2003, the incidence of terminal renal failure was stable in 190 +/- 21 patients per million inhabitants (ppmh). It decreased between January and September 2004 to 124 +/- 18.6. Afterwards, it progressively increased to 221 +/- 21 ppmh. In the study period, the proportion of patients admitted to hemodialysis with a definitive access decreased from 63 to 10% (p<0.01) and the mean lapse of transitory catheter use, increased from 32.9 +/- 42.6 to 73.1 +/- 80.4 days (p<0.01). CONCLUSIONS: The implementation of AUGE for chronic renal failure reduced the quality of care of patients admitted to hemodialysis.

Pharmacokinetic behavior and pharmacokinetic/pharmacodynamic integration of marbofloxacin after subcutaneous administration in goats.

The pharmacokinetic behavior of marbofloxacin was studied in goats after single-dose subcutaneous (SC) administration of 2mg/kg bodyweight. Drug concentration in plasma was determined by high performance liquid chromatography and the data obtained were subjected to non-compartmental kinetic analysis. Marbofloxacin peak plasma concentration (C(max)=1.77+/-0.24microg/mL) was reached 1.25+/-0.50h (T(max)) after SC administration. The elimination half-life (t(1/2beta)) and area under curve (AUC) were 5.74+/-1.21h and 8.15 vs 2.33microg h/mL, respectively. Taking into account the values obtained for the efficacy indices, it was concluded that a SC dose of 2mg/kg/24h of marbofloxacin could be adequate to treat infections caused by high susceptible bacteria like Escherichia coli or Salmonella spp.

The pharmacokinetic behaviour of marbofloxacin in Eurasian buzzards (Buteo buteo) after intraosseous administration.

This study reports on the administration of a single dose of marbofloxacin (2 mg/kg) to five adult Eurasian buzzards (Buteo buteo) by the intraosseous (IO) route, which has been proposed as a rapid and efficient means for the parenteral delivery of antimicrobial drugs. The drug was rapidly absorbed. Peak marbofloxacin concentration (C(max)) in plasma and area under the concentration-time curve (AUC) of 1.92+/-0.78 microg/mL and 8.53+/-2.73 microg h/mL, respectively. The time marbofloxacin remained in the plasma after IO administration was relatively short (elimination half-life, t(1/2beta)=4.91+/-0.65 h; mean residence time (MRT)=5.38+/-0.57 h). Single dose marbofloxacin gave values for C(max)/minimum inhibitory concentration (MIC) of 19.2 and an AUC/MIC value of 85.3h after IO administration. The IO route appears to be practical and effective for the rapid delivery of marbofloxacin to buzzards.

Cat-scratch disease (ocular bartonellosis) presenting as bilateral recurrent iridocyclitis.

An otherwise healthy 9-year-old girl presented with bilateral recurrent anterior uveitis. Thirteen months later, the diagnosis of cat-scratch disease (ocular bartonellosis) was suspected when neuroretinitis appeared. Confirmation was based on serological test results positive for Bartonella henselae. Antibiotic treatment completely cured the disease, and there have been no further manifestations during a follow-up period of 6 years.

Cryopreservation of semen in the dog: use of ultra-freezers of -152 degrees C as a viable alternative to liquid nitrogen.

This experimental work was carried out to validate the use of a -152 degrees C ultra-low temperature freezer to freeze and store canine semen. The semen of three dogs was pooled and processed to obtain a final dilution with a concentration of 100 x 10(6) spermatozoa/mL, glycerol at 5% and Equex at 0.5%. Then, four freezing protocols were tested to evaluate the cryosurvival of sperm at 1, 7, 30, 60 and 120 days after freezing: (I) semen was frozen and stored in liquid nitrogen; (II) semen was frozen in liquid nitrogen and stored in the ultra-low freezer at -152 degrees C; (III) semen was frozen in the vapour of liquid nitrogen and stored in the ultra-low freezer at -152 degrees C; (IV) semen was frozen and stored in the ultra-low freezer at -152 degrees C. Data were statistically analyzed by repeated measures analysis of variance to determine the effect of the freezing protocol and time on the sperm characteristics assessed. The percentages of sperm motility and of dead/live spermatozoa were similar throughout the experimental period, with no significant differences (P < 0.05) to be observed between four different freezing techniques tested. At 120 days after freezing, the percentage of abnormal cells and the percentage of sperm cells with abnormal acrosome were not significantly different between the freezing techniques. Although the number of dogs used was slightly low, in vitro results of this preliminary study showed that the use of ultra-freezers at -152 degrees C to freeze and store canine semen could be a viable alternative to liquid nitrogen.

Pharmacokinetics of marbofloxacin after intravenous and intramuscular administration to ostriches.

The pharmacokinetics of marbofloxacin was investigated after intravenous (IV) and intramuscular (IM) administration, both at a dose rate of 5 mg/kg BW, in six clinically healthy domestic ostriches. Plasma concentrations of marbofloxacin was determined by a HPLC/UV method. The high volume of distribution (3.22+/-0.98 L/kg) suggests good tissue penetration. Marbofloxacin presented a high clearance value (2.19+/-0.27 L/kgh), explaining the low AUC values (2.32+/-0.30 microgh/mL and 2.25+/-0.70 microgh/mL, after IV and IM administration, respectively) and a short half life and mean residence time (t(1/2 beta)=1.47+/-0.31 h and 1.96+/-0.35 h; MRT=1.46+/-0.02 h and 2.11+/-0.30 h, IV and IM, respectively). The absorption of marbofloxacin after IM administration was rapid and complete (C(max)=1.13+/-0.29 microg/mL; T(max)=0.36+/-0.071 h; MAT=0.66+/-0.22 h and F (%)=95.03+/-16.89).

A comparison of diagnosis of pregnancy in the goat via transrectal ultrasound scanning, progesterone, and pregnancy-associated glycoprotein assays.

Real-time ultrasound scanning (US) via the transrectal route, progesterone (P4) assay, and pregnancy-associated glycoprotein (PAG) detection can be used to diagnose pregnancy at around 3 weeks after breeding. Although several studies have been carried out to evaluate each of these different methods individually, it is difficult to establish adequate comparisons due to differences, such as the breed of goat, age, and farming conditions, among others. The aim of the present paper is to compare the accuracy of diagnosis of pregnancy using transrectal US, P4 assay and PAG detection at the same time and on the same animals. Canary dairy goats (n=143) were synchronized with an 11-day fluorogestone acetate (FGA) intravaginal sponge followed by PGF2alpha and eCG 2 days before the FGA withdrawal. Blood samples were collected on Days 20, 22, 24, and 26 after mating to determine P4 and PAG concentrations. Transrectal US examinations were performed at the same time. There were 79 pregnant goats and another 64 non-pregnant. The US via the transrectal method and the determination of PAG concentrations provide very accurate pregnancy diagnosis at 24-26 days after breeding; on the contrary, P4 assay on plasma samples performed on Day 22 after breeding was accurate, in this case, in detecting pregnant animals but did not always detect the non-pregnant does.

Prevention of HIV infection among migrant population groups in Northeast Brazil.

HIV infection is spreading among the poor, women, and migrant communities in the interior of Northeast Brazil. The research focused on different configurations, beliefs, representations, and forms of social organization of behavior thought to be associated with the population's capacity to efficiently follow AIDS prevention measures. Participants located in neighborhoods known for having large migrant populations were identified by Family Health Program Workers in Fortaleza and Teresina. The study adopted a qualitative methodology. Several belief-system concepts and values, as well as the social organization of sexuality revealed in the study, represent obstacles both to AIDS prevention and condom use. Hunger, lack of prospects, and fear are associated with a social situation of poverty, exclusion, prejudice, and total absence of basic human rights. When examined together, these elements define different configurations in the migrants' increased vulnerability to HIV/AIDS. The groups' increased vulnerability relates to the socioeconomic complexity that must be considered in HIV/AIDS control and prevention programs.

[Implementation of AUGE plan for chronic renal failure patients. The implementation of Universal Access and Explicit Guaranties Plan (AUGE), generates longer waiting lists among patients with chronic renal failure].

BACKGROUND: An opportune, efficient and low cost medical attention should be provided to all patients, without discrimination. When the need for medical attention outweighs the available resources, permanently increasing waiting lists are generated. The aim of the Universal Access and Explicit Guaranties Plan (AUGE) is to attend selected patients in periods predefined by the health authority. AIM: To describe the first six months of implementation of AUGE plan for chronic renal failure in a public hospital of Santiago. MATERIAL AND METHODS: As a health care quality monitoring system, the lapse between the generation of a nephrological consultation and the specialist assessment, is registered periodically. These lapses were evaluated before and after the AUGE plan implementation, for eligible and non eligible patients for the plan. Results were modeled according queue theory. RESULTS: After three months of implementation, the AUGE plan prolongs waiting periods and increases the number of all patients seeking for nephrological attention. CONCLUSIONS: According to these results AUGE plan will be successful only if its present functioning is modified and receives more financial resources.
