No allele variation of the MAOA gene promoter in male Chinese subjects with attention deficit hyperactivity disorder.

The aim of the current study was to examine any difference in the repeat sequence of the monoamine oxidase A (MAOA) gene promoter between males with attention deficit hyperactivity disorder (ADHD) and randomly selected subjects in a community. The role of the MAOA gene promoter in ADHD cases was also investigated. The total of 244 participants included 57 male ADHD subjects as the case group and 187 males, also in southern Taiwan, as the community controls. There was no significantly different distribution in the repeat sequence of the MAOA gene promoter (chi2 = 3.895, d.f. = 3, p = 0.273), and no significantly different distribution of 'long-form' and 'short-form' alleles between the ADHD group and the male community group was noted (chi2 = 2.484, d.f. = 1, p = 0.115). Some aspects of clinical response are mentioned in the discussion of this study which are worth exploring further in the future.

A module map showing interaction between apolipoprotein E and phospholipase A2 polymorphism in lipid profiles.

OBJECTIVE: Establish a possible conceptual relationship among Apo E and PLA2 polymorphism and lipid profiles. METHODS: Five hundred subjects aged 65 to 74 years were randomly selected from a community in southern Taiwan to assess the relationship between Apo E and PLA2 polymorphisms and lipid profiles. Two hundred fifty-six participants agreed to have venous blood drawn for DNA studies. RESULTS: By multiple linear regression, the PLA2 A2 allele showed a statistically significant influence on LDL-C (p = 0.0097), and the Apo epsilon2 allele showed a statistically significant influence on HDL-C (p = 0.0004), however, the interaction between the PLA2 A2 allele and the Apo epsilon2 allele was found to be significant in the blood fraction of HDL-C (p = 0.0388) and LDL-C (p = 0.0002). Decreasing HDL-C and increasing LDL-C were found when the PLA2 A2 and Apo epsilon2 allele co-existed. CONCLUSION: The presence of a physiologic balance contributes significantly to homeostatic and compensatory responses regulating blood HDL-C and LDL-C profiles. A module map of the generation-control cycle and conditional activity among Apo E, PLA2, and lipid levels is presented, and both behaviours and biological perspectives under the consilience model may suggest a new approach to many kinds of complex disorders.

Priapism associated with typical and atypical antipsychotic medications.

Priapism is a urologic emergency, and a urologic consultation should be obtained as early as possible. Its etiologies are numerous and diverse. Patients who use antipsychotic drugs should be informed about the complications of priapism, especially those patients with a history of prolonged erections associated with other alpha-adrenergic blocking agents or a history of sickle cell disease. If antipsychotic medication is considered to be needed in the treatment of patients with a history of priapism, physicians should select a drug with a low peripheral alpha-adrenergic blocking property. Physicians should be aware of this rare but potentially serious complication of antipsychotic drugs. Early intervention and appropriate treatment are essential to prevent permanent impotence and other serious complications.

Dopamine D4 receptor gene and the -521C>T polymorphism of the upstream region of the dopamine D4 receptor gene in schizophrenia.

OBJECTIVE: The dopamine D4 receptor (DRD4) is a candidate gene for increasing genetic susceptibility to schizophrenia. A recent study found that a -521C>T promoter base pair change affects transcriptional regulation of the DRD4 gene. The present study was designed to investigate the role of both the -521C>T single nucleotide polymorphism and the DRD4 variable number tandem repeat (VNTR) polymorphism. METHODS: A case-control study of 630 Chinese schizophrenic patients and 428 Chinese controls was conducted to test for allelic association with schizophrenia. RESULTS: The number of DRD4 VNTR fragments was associated with schizophrenia. Long DRD4 VNTR fragments as opposed to short fragments were commoner in schizophrenia. No evidence was found for allelic association between the -521C>T DRD4 polymorphism and schizophrenia. CONCLUSIONS: This study provides preliminary and unconfirmed evidence for the involvement of the DRD4 repeat VNTR in the pathogenesis of schizophrenia.

The mental health of hospital workers dealing with severe acute respiratory syndrome.

BACKGROUND: The aim of this study was to explore the impact of the severe acute respiratory syndrome (SARS) on health care workers in Taiwan. The possible predisposing and perpetuating factors in developing mental symptoms were investigated. METHODS: Participantsconsisted of 135 health care workers who had had contact with or took care of patients who were probable or suspected SARS cases, during the SARS outbreak, at a teaching hospital in southern Taiwan. All participants completed questionnaires, including the Chinese Health Questionnaire, Eysenck Personality Questionnaire and Parental Bonding Instrument. RESULTS: Twenty-two participants (17.3%) developed significant mental symptoms, and 105 participants (82.7%) showed no obvious symptoms. The structural equation model showed that maternal care (beta = -0.18, p = 0.011) and neuroticism (beta = 0.54, p < 10(-6)) directly influenced the ability of health care workers to deal with the impact of SARS. Maternal overprotection (beta = 0.30, p < 10(-4)) had an indirect influence on the ability to cope with the impact of SARS. CONCLUSION: Both neuroticism and maternal attachment influenced the mental health of health care workers during the SARS epidemic. The results can be a helpful reference for intervention and prevention programs for health care workers facing disasters in the future.

Hypertension--a possible vulnerability marker for depression in patients with end-stage renal disease.

OBJECTIVE: This study explored the association of hypertension and psychiatric morbidity in patients with end-stage renal disease (ESRD) under adjusted personality characteristics and parental attachment. METHODS: The mental health of 121 patients with ESRD in a general teaching hospital was evaluated using the 12-item version of the Chinese Health Questionnaire (CHQ). Only 40 males and 49 females completed all the questionnaires. Ten of the 40 males and 21 of the 49 females had high scores (> or = 4) and were allotted to the case group (n = 31). The remaining 58 patients constituted the control group (CHQ < 4). RESULTS: The logistic regression model showed that hypertension, gender, and neuroticism are statistically significant covariates. Hypertension, especially, was strongly associated with depressive vulnerability (odds ratio of hypertension versus without hypertension = 9.07:1). Structural equation modeling revealed that gender difference and hypertension directly influenced the individuals' mental health status and that the influence of hypertension on mental health was highly variable. CONCLUSION: A parsimonious structural equation model provided considerable evidence that hypertension could have an important effect on depression in ESRD patients, when predisposing factors, such as personality characteristics and parental attachment, gender, duration of hemodialysis and other medical diseases were adjusted. Hence, hypertension might be a mediating factor of depressive vulnerability in ESRD patients underling genetic and environmental problems.

Mutation of the p53 tumor suppressor gene in transitional cell carcinoma of the urinary tract in Taiwan.

Early detection of a mutated p53 gene is thought to provide useful information in a wide range of human tumors. The aim of this study was to identify the role of the p53 gene in transitional cell carcinoma of the urinary tract. From March 1992 to July 2003, 75 patients (54 men and 21 women) with a mean age of 66.85 years and pathologically diagnosed transitional cell carcinoma were enrolled in this study. Fifty-eight patients had bladder cancer, eight had ureteral cancer, and nine had renal-pelvic cancer. Rapid screening for mutation of the p53 gene was performed using polymerase chain reaction (PCR), single-strand conformation polymorphism (SSCP), and sequencing analysis. Primer sets were designed to amplify fragments within exons 4, 5, 6, 7, and 8 of the p53 gene. Pathology classified 37 tumors as low grade and 38 as high grade. Tumor stage was pT1 or less in 29 patients and at least pT2 in 46 patients. Of the 75 patients in this study, 47 (62.7%) had a p53 mutation. Of the patients with a p53 mutation, 33 (70.2%) had invasive tumors. Invasive tumors were associated with p53 mutation (p < 0.05). Noted in 20 patients (26.7%), exon 4 was the most common site of the mutation. Of the patients with exon 4 mutations, 15 (75%) had invasive tumors and nine (45%) had high-grade tumors. Additionally, among the 20 patients with a common polymorphism at codon 72, 16 (80%) had invasive tumors and 14 (70%) had high-grade tumors. In this study, 62.7% of patients with transitional cell carcinoma had a p53 mutation, suggesting that the p53 gene mutation may be used as a marker of transitional cell carcinoma. Invasive tumors are more likely to have a p53 gene mutation. A simple analysis of the p53 gene using PCR/SSCP is suitable for screening for p53 abnormalities in transitional cell carcinoma. The relationship between cancer risk and the codon 72 polymorphism of exon 4 needs further investigation.

Telomeric length varies with age and polymorphisms of the MAOA gene promoter in peripheral blood cells obtained from a community in Taiwan.

OBJECTIVE: Telomere shortening and increased MAOA gene activity both occur with aging. We undertook to develop a predictive model of telomere shortening and to investigate the possible association between MAOA gene promoter polymorphisms and telomere length as influenced by age and gender. METHODS: A stratified random household sample was selected from a community in southern Taiwan. Of 1231 subjects attending our health-screening program, 441 agreed to have additional venous blood withdrawn for DNA extraction and genetic study. Exactly 433 subjects completed the questionnaires and genetic analysis. Their telomere lengths were distributed (6.4-11.63 kb). RESULTS: The rate of shortening per year was 69 base pairs, and the gender difference in length was not statistically significant (F = 0.091, P = 0.763). The lognormal distribution of telomere lengths was linear. The polynomial regression analysis showed Ln (telomere length) = -2.57-0.007 x age - 0.34 MAOA (adjusted R-square = 0.60). The gender effect on telomere length was not statistically significant (P = 0.52). No interaction effects were found between age, gender and MAOA gene polymorphisms. The high-activity allele of the MAOA promoter polymorphisms were negatively associated with telomere length (P = 0.013). Structural equation modeling confirmed the null model structure. The present data suggest that high-activity MAOA promoter gene polymorphisms, as in aging, are a risk factor for telomeric shortening. CONCLUSIONS: Central nervous system serotonergic activity correlates with human aggressive behavior and depression in many studies, and the MAOA promoter gene may also serve as a clinical marker in the treatment of cardiovascular disease. The predictive model and table of telomere length presented in this study will provide a quick reference for future studies.

Suicidal ideation and associated factors among community-dwelling elders in Taiwan.

The purpose of the present study was to explore the suicidal ideation of community-dwelling elderly and the factors associated with their intention to commit suicide. Using a multilevel stratified sampling strategy, 1000 elderly subjects were recruited (aged 65-74 years old) in Taiwan during the year 2001. The degree of depression and its correlates were assessed. Suicidal ideation was measured by asking respondents if they had had any suicidal thoughts in the previous week. In all, 16.7% of respondents reported suicidal ideation within the past week; its occurrence was related to sex, religious belief, employment status, marital status, average family monthly income, physical health status, depressive symptoms, and community activity participation. Further multivariate logistic regression revealed that, aside from depressive symptoms and a lower level of education, no community participation in the past 6 months was significantly associated with the appearance of suicidal ideation. The prevalence of suicidal ideation among the elderly in Taiwan is higher than in Western countries. Participation in social activities is negatively associated with elderly suicidal ideation. The dimension of social participation deserves further exploration and should be considered in community mental health promotion interventions for elderly people.

p53 codon 72 polymorphism and susceptibility malignancy of colorectal cancer in Taiwan.

PURPOSE: The p53 tumor suppressor gene plays two important roles in genomic stability: blocking cell proliferation after DNA damage until it has been repaired, and starting apoptosis if the damage is too critical. A recent report suggests that a polymorphism of the p53 tumor suppressor gene that results in the substitution of a proline residue with an arginine residue at position 72 of the p53 protein might act as a risk factor in the malignant transformation of colorectal adenoma to cancer. METHODS: In our study, the samples consisted of 150 patients were analyzed for the mutation in the p53 gene. The age of 150 patients (46 women and 104 men) ranged from 30 to 91 years (mean age 68.46 years). RESULTS: The polymorphism showed 52.04% mutant in the codon 72 of exon 4 in the Taiwanese population. Both of the chi-square for trend test (chi-square = 4.97, p = 0.034) and logistic regression (p = 0.037, odds ratio = 1.699) showed significant differences in the distribution of polymorphism of codon 72 in the p53 gene and Dukes classification of colorectal cancer. CONCLUSIONS: There were significant relationship between the polymorphism of codon 72 and the malignancy of colorectal cancer in Taiwanese population. There is 1.70 times in each grade change (Dukes A-D) more risk of CCC polymorphism than that of CCG polymorphism of codon 72 of exon 4.

Personal characteristics of adolescent prostitutes and rearing attitudes of their parents: a structural equation model.

The aim of this study was to investigate the risk factors of family structure, personality traits, and other variables among adolescent prostitutes. The subjects comprised 158 adolescent prostitutes in a halfway house as the case group and 65 high school girls as the control group. Data were collected by using questionnaires about demographic information, the Junior Eysenck Personality Questionnaire and the Parental Bonding Instrument. A high rate of tobacco, alcohol and drug use was found in the case group. Numerous factors distinguished the cause-effect relationship among adolescent prostitutes. Of all risk factors studied, maternal protection, paternal care, neurotic characteristics, tobacco use, discontinuous schooling and a dysfunctional family had the most direct effect. These results provide useful information for the evaluation of and interventions with adolescent prostitution.

Cognitive impairment and associated risk factors among aged community members.

OBJECTIVE: The objective of this paper is to investigate the prevalence of and risk factors associated with cognitive impairment in a community-dwelling elderly population. METHODS: 1000 community residents aged 65-74 years were recruited in the cross-sectional survey of associated factors of cognitive impairment in southern Taiwan. We employed a multilevel stratified sampling strategy and conducted this interview survey from April to June 2001. The survey questionnaire included background information and the Short Portable Mental Status Questionnaire. The relationship between cognitive impairment and each individual variable in personal attributes and social participation were explored. RESULTS: Eight hundred and ninety-four (89.4%) subjects completed the interview and questionnaires. The cognitive impairment rate among all subjects was 4.92%. Of those surveyed, 54.98% were male and 45.02% were female, and the average age was 69.37 (SD 2.73) years. Most survey respondents (72.05%) were married, and 68.90% of the subjects did not participate in any social activities in the last 6 months. Females, those who were illiterate or did not attend school, those who had an average family income below US$860 per month, and persons who did not participate in any social activities had a significantly higher rate of cognitive impairment. CONCLUSION: The educational level and participation in social activities were associated with the cognitive impairment among the aged community dwellers.

Adverse effects of risperidone and haloperidol treatment in schizophrenia.

PURPOSE: Side effects of pharmacological treatment in schizophrenia continue to be a major issue in spite of the development of new antipsychotics. The aim of this study is to explore the adverse effects of conventional and atypical antipsychotic drugs and their associated factors. METHODS: Over 3 months, 41 patients with schizophrenia were randomized to treatment with risperidone 1-12 mg (n=21) or haloperidol 2-20 mg (n=20) daily. Efficacy was assessed by improvement of psychotic symptoms, measured on the Positive and Negative Syndrome Scale (PANSS). The safety and tolerability were assessed with the Extrapyramidal Symptom Rating Scale, the UKU Side-Effect Rating Scale and clinical laboratory assessments. RESULTS: Each treatment reduced psychotic symptoms. PANSS total scores, positive scores, and general psychopathology scores declined as trial went on without significant differences between the two groups. While PANSS negative scores improved better in the risperidone group than in the haloperidol group. The tolerability of antipsychotics was statistical significantly better in the risperidone than in the haloperidol-treated patients. The most frequent adverse effects for both groups were tremor and rigidity. Antipsychotics, their doses, and hyperprolactinemia predict short-term extrapyramidal side effects. Serum prolactin levels could predict parkinsonism and dyskinesia severity. However, dyskinesia was best predicted by the doses of neuroleptics. The predictive factor of dystonia was the antipsychotic drug itself. After adjusting drug doses and concomitant medications, side effects could be markedly improved. CONCLUSIONS: This study suggested that risperidone was superior to haloperidol in improving negative symptoms and better tolerated during the 12 weeks' treatment of schizophrenia. Serum prolactin levels could predict the severity of parkinsonism and dyskinesia.

Comparison of quality of life for people with schizophrenia and mental health of caregivers beteen commn-based and hospital-based services.

This study is a comparison of the quality of life and family stress levels in community-based and hospital-based services for people with schizophrenia. Fiscal considerations of the health insurance industry in Taiwan require the evaluation of a community support program versus the traditional, hospital-centered program for reform of mental health policy concerning schizophrenia. The study involved 52 schizophrenic patients, 27 in a community-based program and 25 in a hospital-based treatment model, and was conducted from January to December 2001 in the psychiatric department of a general teaching hospital in southern Taiwan. Outcomes were determined using the World Health Organization quality of life questionnaire (WHOQoL-BREF, Taiwan version), Brief Psychiatric Rating Scale (BPRS), General Health Questionnaire (Chinese version), rate of loss to follow-up, job conditions, and social function. Comparisons of quality of life and caregiver mental health between the two groups were accomplished using descriptive analysis, independent sample t test, and the generalized estimating equation-I. No significant differences between the two groups were found in quality of life or family mental stress according to the General Health Questionnaire after controlling for sex, age, disease duration, full IQ, and total BPRS score. Long disease duration predicted a hospital setting, while a high IQ was predictive of a community setting. We found no decrease in quality of life for schizophrenic patients in a hospital-based program and no increase in family mental stress among the community-based group. To improve patients' quality of life and the mental health of caregivers in both services, it is important to ameliorate severe symptoms associated with schizophrenia.

Psychosocial characteristics of criminals committing incest and other sex offenses: a survey in a Taiwanese prison.

The purpose of this study was to investigate the psychosocial characteristics of criminals who had committed incest or other sexual offenses. The participants, 240 criminals serving sentences for sex offenses in a Taiwanese prison, were divided into two groups: incest offenders (20.4%) and other sex offenders (79.6%). The psychosocial characteristics taken into consideration included age, parental survival, education, marital status, previous crime records, drug and alcohol abuse, diagnosed mental disorders, and victim abuse at the time of the offense. After an analysis of the data, the authors concluded that even though incest offenders showed fewer mental disorders, they needed psychiatric treatment and that this treatment should be focused not only on their mental disorder and related symptoms but especially to correct their abnormal behavior. Also, attention should be given to their psychosocial characteristics.

Mutations in the p53 tumor suppressor gene in colorectal cancer in Taiwan.

Mutations in the tumor suppressor gene p53 have been reported as occurring prevalently in a wide range of human tumors. Detection of a mutated p53 is thought to provide useful information for the clinical management of colorectal neoplasm. In this study, we used polymerase chain reaction/single-strand conformation polymorphism (PCR/SSCP) and sequencing analysis to rapidly screen for mutations in p53 in colorectal cancer in Taiwan. Genomic DNA was purified from colorectal cancer specimens obtained from 80 patients at a teaching hospital in southern Taiwan. Primer sets were designed to amplify fragments within exons 4-8 of p53. We found p53 mutations in 38 of 80 patients. This is the first identification of a mutation at codon 143 of p53 in colorectal cancer in Taiwan. In addition, we found two insertions in exon 5 of p53. The p53 mutation rate among colorectal tumors in Taiwan, found in this study, is 43%. The results indicate that p53 mutation is not significantly associated with tumor grade, age, or gender (p > 0.05). We found that two-fifths of colorectal cancer patients in Taiwan have a p53 mutation, which could be used as a marker of colorectal cancer.

Ethnic heterogeneity in allele variation in the DRD4 gene in schizophrenia.

The goal of the present study was to use a meta-analysis on previous studies plus our own unpublished data to confirm and extend findings which indicate that the variation in the dopamine D4 receptor (DRD4) gene is best represented by a mixture of two different ethnic groups. The genotype distribution was divided into either a long or short form using a mixture analysis of normal controls of different ethnic origins under the assumption that there is a single major gene. The meta-analysis was based on the data from 19 independent samples, 18 association studies, and from our own unpublished data, including a total of 1431 schizophrenic patients (sporadic cases 1309, familial cases 122) and 1439 controls. No significant genotype differences were noted between patients and controls for the whole sample. However, reorganization of the studies into different groups by the geographical origin of samples revealed significant ethnic heterogeneity. In addition, there was a significant association between the long form of DRD4 gene and schizophrenia in Caucasians, especially those with familial schizophrenia.

Parental rearing style, premorbid personality, mental health, and quality of life in chronic regional pain: A causal analysis.

The aim of the current study was to establish the causal model among parental bonding, personality characteristics, mental health, quality of life, and chronic regional pain (CRP). Thirty CRP patients and 56 mental illness patients were compared using the Visual Analogue Scale (VAS), Chinese Health Questionnaire (CHQ), Eysenck Personality Questionnaire (EPQ), World Health Organization Quality of Life-Brief-Tawain Version (WHOQOL-BREF-TW), and Parental Bonding Instrument (PBI). There were significant differences in mental health, personality characteristics, and quality of life between the CRP and mental illness groups. Structural equation modeling showed that parental bonding could directly affect personality characteristics, and, hence, directly impact disease and quality of life. CRP is different from mental illness in many dimensions. In this study, CRP appeared to be caused by actual physical dysfunction rather than mental dysfunction.
