A proposal to cover the uninsured in California.

The lack of health coverage for millions of Californians is a major societal problem. In the absence of federal action, we propose a state-based approach that leverages existing systems to create near-universal coverage within two years. We describe several subsidized benefit options for low-income uninsured Californians, emphasizing preventive and primary care, and we propose catastrophic coverage, at a minimum, for higher-income uninsured Californians. Proposed financing mechanisms include a health care sales tax and an "in-lieu" payroll tax.

Does population management of chronic disease lead to lower costs of care?

The paper by Bruce Fireman and colleagues is an important contribution to the dialogue about the role of chronic disease management in quality improvement and health care cost mitigation. There is much enthusiasm for the potential impact of disease management techniques on the costs associated with chronic and complex health conditions. This Perspective describes several considerations that are important to the interpretation of studies of the cost impact of disease management and to assessments of the future usefulness of these interventions.

The delivery system matters.

To meet the quality, affordability, and access challenges of today's health care system, the Institute of Medicine's (IOM's) Crossing the Quality Chasm report described the critical competencies of a twenty-first-century health care system. A growing body of research suggests that the nation's multispecialty group practices most nearly meet the delivery system challenges set forth by the IOM. A variety of current public and private initiatives and potential policy options could act as catalysts for the development and spread of group practice-based, accountable delivery systems that are effective and efficient.

The changing shape of the physician workforce in prepaid group practice.

Multiple factors have combined to change the size and specialty composition of the physician workforce in the nation's largest prepaid group practices over the past two decades. An examination of these changes can shed some light on the past and potential future impact that changes in medical technology are likely to have on the physician workforce of these organizations and the greater physician community.
