Effect of household processing and unit-to-unit variability of pyrifenox, pyridaben, and tralomethrin residues in tomatoes.

Residue levels of pyrifenox, pyridaben, and tralomethrin were determined in unprocessed and processed tomatoes, grown in a experimental greenhouse, to evaluate the effect of three different household processes (washing, peeling, and cooking) and the "unit to unit" variability of these pesticides in tomatoes. The study was carried out on 11 greenhouse tomato samples collected during a 5 week period in which two successive treatments with the studied pesticides were applied. Residue levels in unprocessed and processed tomato samples were determined by means of ethyl acetate extraction and gas chromatography-electron capture detection determination. The washing processing factor results were 0.9 +/- 0.3 for pyridaben, 1.1 +/- 0.3 for pyrifenox, and 1.2 +/- 0.5 for tralomethrin, whereas the peeling processing factors were 0.3 +/- 0.2 for pyridaben and 0.0 +/- 0.0 for both pyrifenox and tralomethrin. The average loss of water in the tomato pure samples during the cooking process was approximately 50%; the cooking processing factors were 2.1 +/- 0.8 for pyridaben, 3.0 +/- 1.1 for pyrifenox, and 1.9 +/- 0.8 for tralomethrin. The unit-to-unit variability factors were determined on three different greenhouse samples analyzing 10 different units of unprocessed tomatoes from each sample. In all cases, the unit-to-unit variability factor results were within the range of 1.3-2.2.

[Generalized eczema secondary to combined treatment with peginterferon alfa-2a and ribavirin in a patient with chronic hepatitis from the hepatitis C virus].

Adverse skin effects from interferon (IFN) plus ribavirin combined therapy are relatively frequent, but they are usually local and related to the IFN injection site. However, distant or generalized eczematous reactions secondary to this treatment are rare. The introduction of pegylated interferons may increase the frequency of these skin lesions.

[Cutaneous bronchogenic cyst].

Bronchogenic cysts rarely affect the cutaneous surface, so they are difficult to diagnose clinically and are often confused with other types of cysts. We present the case of a young adult in whom a cystic lesion showing characteristics of a bronchogenic cyst was excised from the upper thorax area. We emphasize the fact that, in nearly all cases, the diagnosis of this pathology is made through a histopathological study, and we make special reference to the differential diagnoses.

[Evaluation of a screening system for patients with pigmented lesions using store-and-forward teleconsultation].

INTRODUCTION: Pigmented lesion clinics (PLC's) were developed as a quick referral system for patients with pigmented lesions. However, the most appropriate method of selecting patients who need to be seen in these units is not clearly defined. Teledermatology is a tool whose usefulness as a patient selection system for PLC's needs to be evaluated. OBJECTIVE: To evaluate teleconsultation as a screening system for patients with pigmented lesions in terms of efficacy, accuracy and satisfaction. METHOD: Teleconsultations received at a PLC over a period of 12 weeks were evaluated. Teleconsultation patients reported changes in a pigmented lesion, a lesion that had recently appeared, multiple lesions, symptomatic lesions or concern about a nevus. We calculated the time intervals in sending the teleconsultation report and in patients being seen at the "physical" PLC consultation, the intraobserver, interobserver and pathologist kappa coefficients, as well as the degree of satisfaction of patients and Primary Care (PC) physicians. RESULTS: 219 teleconsultations were evaluated, 49.3 % of which were referred to the "physical" consultation. The most frequent reason for the teleconsultation was concern about a nevus (37.0 %). The teleconsultations received responses in an average time of 43.9 hours, and patients were seen at the "physical" consultation within 2 weeks. The intraobserver agreement was kappa = 0.93 (95 % CI 0.87-0.98); interobserver agreement, kappa = 0.91 (95 % CI 0.87-0.96); and the agreement between the teledermatologist and the pathologist, kappa = 0.79 (95 % CI 0.70-0.89). 86 % of the patients and 91 % of the Primary Care physicians said that they were "very satisfied" with the implementation of this new system. CONCLUSIONS: Teleconsultation is an accurate screening system for patients with pigmented lesions. With this methodology, waiting times for patients with malignant lesions or those suspected of malignancy can be shortened at the same time as the PLC's excess workload is decreased. However, more experience is needed to establish the true usefulness of this filtering system in the early diagnosis of melanoma.

[Pyoderma gangrenosum over a saphenectomy scar in a cANCA-positive patient with diverticulitis].

Pyoderma gangrenosum is a chronic, painful, ulcerative skin disease of unknown etiology. In 50 % of the cases, it is associated with systemic disease, primarily with inflammatory bowel disease. It can develop over areas of trauma or surgical wounds, possibly because of local immunoreactant deposits. We describe the case of a cANCA-positive patient with colonic diverticulitis who developed pyoderma gangrenosum over a saphenectomy scar.

Treatment of palmar hyperhidrosis with botulinum toxin type A: 44 months of experience.

BACKGROUND: Palmar hyperhidrosis (PH) can produce social and occupational difficulties and reduce the quality of life of those who suffer from this kind of problem. When dealing with focal hyperhidrosis, the patients' attitudes and their subjective approaches regarding the process may influence the objective evaluation of the disorder. OBJECTIVE: To evaluate, by means of a scale, the subjective improvement of sweat production after treatment with botulinum toxin type A (BTX-A) in a group of patients with severe, invalidating PH. Patients and methods Over a period of 44 months, 69 patients were treated and followed-up, 27 patients had to be treated twice, and 11 patients required a third application; 80-100 U was injected in each palm. Regional nerve block was performed before the procedure. The patients were asked to evaluate their improvement at 1, 3, 6, 9, and 12 months of baseline. RESULTS: At 1 month, 53.6% of the patients reported an excellent improvement. Three months later, results were still excellent in 33.3% of the patients, and acceptable in 29%. From then on there was a statistically significant decrease of BTX-A effectiveness. A second application was carried out at an interval of 7.5 +/- 2.6 months, and a third one at 9 +/- 4.4 months. The following complications could be observed: transitory weakness of hand muscles in 13 patients, wrist pain in 5 patients, and cramps in 1 patient. CONCLUSIONS: Botulinum toxin is an effective alternative for the treatment of severe, invalidating PH. The maximum improvement persists up to 3 months; from then on, the effects slowly diminish. In our experience, there were no statistically significant differences with further applications of BTX-A. The patients' subjective assessment can be used to evaluate the sweat production after treatment with BTX-A in those medical centers where a more objective evaluation becomes difficult or impossible.

Sexual experience and conditioned place preference in male rats.

We have previously shown that sexual behavior induces a reward state, as evaluated by conditioned place preference (CPP), only when males or females are able to control the rate of sexual interaction. In the present experiment, we evaluated if male rats that are repeatedly tested in a situation in which they are not able to control the sexual interaction eventually develop CPP. Three groups of sexually naïve male rats were used. One group never mated. A second group was tested once a week for 10 consecutive weeks in a chamber in which they controlled the rate of the sexual interaction. The third group was mated for the same number of weeks in a chamber in which the female, but not the male, controlled mating. The three groups were then tested for CPP. Only the group able to control the sexual interaction developed CPP. The group that had no control over the rate of the sexual interaction did not develop CPP even after 10 tests in which they consistently displayed sexual behavior. These results suggest that an estrous female and/or sexual behavior are powerful incentives that maintain mating even if the rewarding properties of the incentive are reduced.

Safety and efficacy of combined use of 4-hydroxyanisole (mequinol) 2%/tretinoin 0.01% solution and sunscreen in solar lentigines.

The objective of this open-label, noncontrolled study was to evaluate the safety of a combination solution containing 4-hydroxyanisole (mequinol) 2%/tretinoin 0.01% (Solagé) with a sunscreen in the treatment of solar lentigines. The study included a total of 406 subjects for a treatment period up to 24 weeks. Efficacy was evaluated clinically by grading the pigmentation level of the treated areas on the face and forearms. A total of 378 subjects were included in the safety population. Of the 173 subjects with skin-related and treatment-related adverse events, severity was reported as mild in 79 subjects, moderate in 71, and severe in 23. Hypopigmentation was observed in 4 subjects and had definitively resolved in 3 of these subjects at the end of the study or after treatment had been discontinued. Halo hypopigmentation was reported in 16 subjects. No allergic reactions were observed. Efficacy evaluation was based on data for 370 subjects. A total of 325 (88%) subjects had facial target lesions almost clear to clear, and a total of 298 (81%) subjects had forearm target lesions almost clear to clear. Our study shows that the mequinol 2%/tretinoin 0.01% solution is effective, convenient, and safe in the treatment of solar lentigines.

Oral and topical L-phenylalanine, clobetasol propionate, and UVA/sunlight--a new study for the treatment of vitiligo.

BACKGROUND: Vitiligo is a hypopigmented skin condition that usually requires a combination of treatment options. AIM: To demonstrate the effectiveness of topical and oral L-phenylalanine in combination with light plus 0.025% clobetasol propionate at night. PATIENTS AND METHODS: We have performed an open trial on a group of 70 patients with evolutive vitiligo. Participants were treated with oral (100 mg/Kg/day) and topical (gel at 10%) L-phenylalanine, exposed to sunlight (spring-summer) or UVA lamps (autumn-winter), and given 0.025% clobetasol propionate at night. All patients were revisited every 6 months while in the study, with a maximum of 4 revisits. Biochemical studies were performed at the beginning of the treatment and at each revisit. RESULTS: Overall, 90.9% of participants showed improvement, with 68.5% of patients achieving an improvement of 75% or more. This 75% improvement rate was reached 87.9% of the time on the face, 60.4% on the trunk, and 54.6% on the limbs. However, there was a moderate response to the treatment in patients with focal and segmental vitiligo. There was a slight additional improvement in patients receiving UVA lamp light. No biochemical abnormalities were found in any patients. CONCLUSION: L-phenylalanine in combination with 0.025% clobetasol propionate and sunlight during sunny months or UVA lamps in winter, appears to improve evolutive vitiligo without side effects, and therefore is especially recommended on the face or for children.

[Nonvenereal sclerosing lymphangitis of the penis: presentation of a clinical case].

Nonvenereal sclerosing lymphangitis of the penis is an infrequent process that affects the distal lymphatics of this organ. It is characterized by the sudden appearance of a translucent and indurated cord on the coronal sulcus. Its etiology is unknown, although it has been related to microtraumas in the area after intense sexual activity. It is a benign, self-resolving process, so it is not necessary to perform a biopsy in early stages, and initial treatment should be conservative. We present a new case of this disease and discuss its etiopathogenic, clinical, diagnostic and therapeutic characteristics.
