[Human embryonic esophagus squamous epithelial cells cultured through microscopic stripping technique and digestion in vitro].

OBJECTIVE: To discuss the application possibility of the microscopic stripping technique used in the primary culture of human embryonic esophagus squamous epithelial cells, and of the methods for the isolation, depuration and subculture of the esophagus epithelial cells in vitro. METHODS: The squamous epithelial cells were obtained from the esophagus mucous membrane of the 20-week abortion fetus through the microscopic stripping technique, and were digested with trypsin. Then, the morphological, immunohistochemical observation and the growth curve of the isolated cells were studied. RESULTS: The isolated cells were spherical in the cell suspension and spherical-like or polygon-like after attachment to the culture flask. The squamous epithelial specialized cytokeratin staining was strongly positive. And the morphological studies by the transmission electron microscopy indicated that the cultured cells were squamous epithelial cells. The squamous epithelial cells reached the peak level 3-4 days after the transfer of the culture. The absorbance at 3 and 4 days was significantly higher than that at 1, 2, 5 and 6 days (P < 0.05). CONCLUSION: A large number of squamous epithelial cells can be available with the microscopic stripping technique and the digestion method. The cultured squamous epithelial cells can be proliferated quickly, and fit for the tissue engineering study.

[Effect of sodium hyaluronate on proliferation and differentiation of myoblast in vitro].

OBJECTIVE: To investigate the effects of sodium hyaluronate solution on the proliferation and differentiation of myoblasts. METHODS: The 3rd subculture myoblasts from muscle of infant SD rat were cultured in four growth media, in which the concentrations of sodium hyaluronate were 0.05% (group A), 0.1% ( group B), 0.2% (group C)and 0 (group D, control group), respectively. The proliferation rate of myoblasts in each medium was observed through growth curves by means of count and MTT. At the same time, the subculture myoblasts were cultured in differentiated media in which the concentrations of sodium hyaluronate were 0 and 0.1%. The capacity of fusion of myoblasts was compared between two kinds of differentiated media. RESULTS: There were the nearly same proliferation curse in Groups A, B and C: increasing by logarithm at 2 days and reaching peak value at 4 days. The myoblasts in Group D increased slowly: increasing by logarithm at 3 days, doubling at 5 days and reaching peak value at 6 days. MTT has the similar curse to counting. The myoblast proliferation of Group B was more than that of the other groups. The peak value of myoblast fusion was 35% at 6 days in common differentiated media; slowly reached 11.7% at 7 days in the differentiated media in which the concentrations of sodium hyaluronate was 0.1%. CONCLUSION: Sodium hyaluronate at certain concentration can be a decent media for myoblasts, it can accelerate proliferation and differentiation of myoblasts.

[Development in transplantation of the skeletal muscle myoblast].

OBJECTIVE: To review the research progress in transplantation of the skeletal muscle myoblast. METHODS: The recently-published articles concerned with the myoblast transplantation were reviewed, including myoblast culture, modified transplant methods, preparation of the recipient, scaffold choice, and an influence of the recipient's immunity on the transplantation. How to improve the efficiency of the myoblast transplantation was also discussed. RESULTS: The techniques of the myoblast transplantation were improved and transplantation efficiency was increased. CONCLUSION: The transplantation of the skeletal muscle myoblast has a great potential value in clinical practice and a promising future in its clinical application.

[Preliminary research on construction of artificial esophagus with cultured squamous epithelial cells and myoblast cells seeded on small intestinal submucosa].

OBJECTIVE: To study the construction feasibility of a biodegradable artificial esophagus by the squamous epithelial cells and the myoblast cells seeded on the small intestinal submucosa(SIS) and to investigate the growth pattern and angiogenesis of the co-cultured human embryonic squamous epithelial cells and the skeletal myoblasts in vivo. METHODS: The squamous epithelial cells and the myoblast cells were obtained from the 20-week aborted fetus. Both of their cells were marked by 5-BrdU in vitro. The isolated cells were then seeded on the SIS and cocultured in vitro for 24 hours, and then the compound of the cells and the SIS was transplanted into the subcutaneous tissue of the athymismus mice. The observation on the morphology and the cytokeratin AE3 and a-actin specified immunohistochemistry of the squamous epithelial cells and the myoblast cells was performed at each of the following time points: 3 days, 1 week, 2 weeks, and 3 weeks after transplantation. RESULTS: The morphological observation indicated that the cultured cells could penetrate into the small intestinal submucosa and form several-layered cell structures, and that the compound of the cells and the SIS could have angiogenesis within 2-3 weeks. The 5-BrdU specified immunohistochemical observation suggested that the cells growing in the small intestinal submucosa scaffold might be the cells transplanted. The cytokeratin AE3 specified and alpha-actin specified immunohistochemical studies demonstrated that the transplanted cells could differentiate in vivo. CONCLUSION: It is possible to fabricate the framework of a biodegradable artificial esophagus with the epithelial cells and the myoblast cells seeded on the small intestinal submucosa.

[Experimental study on repair of goat tibia defect with marrow stromal cell and bio-derived bone].

OBJECTIVE: To investigate the feasibility of repairing goat tibia defect with marrow stromal cells (MSCs). METHODS: MSCs were cocultured with the bio-derived bone in vitro, and the 20 mm tibia defects were made and fixed with plate in 35 goats, and they were divided into the experimental group, control group and blank group. The defects on the right side were filled with tissue engineering bone as the experimental group, the defects on the left side with bio-derived bone as the control group in 33 goats, and the defect on the both sides were not filled with any materials as the blank group in 2 goats. The repair capability was assessed physically, histopathologically and biomechanically at 2, 4, 6, 8, 12, 16 and 24 weeks after operation in 3 groups. RESULTS: By physical, histopathological and biomechanical examinations, the bio-derived bone was partially absorbed in the experimental group and was rarely absorbed in the control group in the 4th week; the defects were partially repaired in the experimental group, and in the control group, few new bones were observed in the two ends of the implants, in which there was fibrous tissue. The effects of biomechanics had no statistically significant difference between the experimental group and the control group (P>0.05) in the 8th week; the defects were perfectly repaired in the experimental group and the effects of biomechanics had statistically significant difference between two groups (P<0.05) in the 12th weeks. The defects were not repaired in the 24th week in the blank group. CONCLUSION: The tissue engineering bone can efficiently repair bone defect, and its repair capability is better than that of bio-derived bone alone both in quantity and in quality of bone formation.

[Biomechanical evaluation of the valgus stability of elbow after reconstruction].

OBJECTIVE: To evaluate of the valgus stability of the elbow after excision of the radial head, release of the medial collateral ligament (MCL), radial head replacement, and medial collateral ligament reconstruction. METHODS: Twelve fresh human cadaveric elbows were dissected to establish 7 kinds of specimens with elbow joint and ligaments as follow: (1) intact (n=12); (2) release of the medial collateral ligament (n=6); (3) excision of the radial head (n=6); (4) excision of the radial head together with release of the medial collateral ligament (n=12); (5) radial head replacement (n=6); (6) medial collateral ligament reconstruction (n=6); (7) radial head replacement together with medial collateral ligament reconstruction (n=12). Under two-newton-meter valgus torque, and at 0, 30, 60, 90 and 120 degrees of flexion with the forearm in supination, the valgus elbow laxity was quantified: All analysis was performed with SPSS 10.0 software. RESULTS: The least valgus laxity was seen in the intact state and its stability was the best. The laxity increased after resection of the radial head. The laxity was more after release of the medial collateral ligament than after resection of the radial head (P<0.01). The greatest laxity was observed after release of the medial collateral ligament together with resection of the radial head, so its stability was the worst. The laxity of the following implant of the radial head decreased. The laxity of the medial collateral ligament reconstruction was as much as that of the intact ligament (P>0.05). The laxity of the radial head replacement together with medial collateral ligament reconstruction became less. CONCLUSION: The results of this study show that the medial collateral ligament is the primary valgus stabilizer of the elbow and the radial head was a secondary constraint to resist valgus laxity. Both the medial collateral ligament reconstruction and the radial head replacement can restore the stability of elbow. If the radial head replacement can not be carried out, the reconstruction of the medial collateral ligament is acceptable.

[Clinical study on reconstruction of hemifacial atrophy with serratus anterior free-muscle flap].

OBJECTIVE: To study the method of treating hemifacial atrophy with free serratus muscle flap. METHODS: Three patients diagnosed as having serious hemifacial atrophy was treated with free serratus muscle flap. The root of the flap was thoracodorsal artery and thoracodorsal vein, which was anastomosed with superficial temporal artery and vein, facial artery and vein, lingual artery and vein, and so on. During the operation, long thoracic nerve was dissected and anastomosed with facial nerve. The sizes of the flaps were 12 cm x 8 cm - 16 cm x 12 cm. RESULTS: All free-muscle flaps healed well after the transplant. The face and buccal area looked chubby and rounded. There were no obvious protuberance and discontentment on the buccal area. The shoulders of all patients moved well. The facial contour of the patients recovered well during the follow-up period (1-3 years). CONCLUSION: The method has a good result. The long-term effect needs further study.

[A preliminary clinical study on absorbable screws].

To evaluate the application of self-reinforced ploy-DL-lactide (SR-PDLLA) absorbable screws in the treatment of spongy bone and intra-articular fractures, the internal fixation with SR-PDLLA absorbable screw was performed on 58 cases,of which 25 were medial malleolus fractures, 14 condyle of humerus fractures, 9 tibial plateau fractures, 7 ulnar olecranon fractures, and 3 lateral malleolus fractures. Observations were made on the orthopaedic operation sites and overall conditions frequently and radiography was performed for periodic analysis. The results showed that SR-PDLLA absorbable screws were particularly effective and reliable, they had good biocompatibility in the treatment of spongy bone and intra-articular fractures and could be used safely in clinical practice.

[Functional reconstruction of irreversible partial injury of brachial plexus].

OBJECTIVE: To assess treating results of functional reconstruction of irreversible partial injury of brachial plexus and to improve the function of injured upper extremity. METHODS: Seventy-nine cases with irreversible partial injury of brachial plexus were treated in transfer of muscle (tendon) or by functional arthrodesis (fixation of tendon) from January 1984 to June 2003. According to the evaluation criterion by American Shoulder and Elbow, Hand Association, all patients were followed up in motion of reconstructive joint and daily activities after operation for 1 year to 19 years. The effect of the operation was comprehensively scored and evaluated. RESULTS: Final results in 54 cases were as follows: 30 patients with good results, 19 patients with fair results, and 5 with poor results. The results demonstrated some points as follow: (1) if the shoulder was instable, arthrodesis of shoulder would be a better choice; (2) the flexion of the elbow joint should be only reconstructed with the dynamic reconstructive methods. The reconstruction of flexion of elbow by transfer of pectoral major muscle was more effective than that by transfer of flexor carpi ulnaris muscle; (3) the dynamic reconstruction of extension of digital and carpi was better than that of flexion of digital and opposition function of the thumb; (4) the supination of the forearm was effectively reconstructed by transfer of flexor carpi ulnaris muscle. Pronation teres muscle should be studied more in reconstruction of supination function of the forearm. CONCLUSION: The functional reconstruction is effective in treating irreversible partial injury of brachial plexus.

[Limb salvage surgery for wrist in bone tumor by using free vascularised fibular graft with fibular head or simple fibular graft].

OBJECTIVE: To compare the long-term results of vascularised fibular graft and simple autologous fibular graft for reconstruction of wrist after distal bone tumor resection. METHODS: From January 1979 to September 2002, 15 patients with wrist defects due to distal bone tumor resection were treated with vascularised fibular graft or simple autologous fibular graft and followed up 1 year. The results were graded with Enneking's system and evaluated radiographically according to the "International Symposium on Limb Salvage". The grade system included limb function, radiological examination and the function of ankle. RESULTS: The limb function of 8 patients with vascularised fibular graft restored to 80% of normal function and the bone union was achieved within 6 months. The limb function of 6 patients with simple autologous fibular graft restored to 67% of normal function. The bony union was achieved within 6 months in 4 cases with the bone graft less than 5 cm and in the 13th and 16th months in 2 cases with the bone graft more than 12 cm. CONCLUSION: It is suitable to use the head of fibular bone as a substitute for the distal radius. The healing of vascularised fibular graft is very quick and haven't the bone resorption. So in the procedure for reconstruction and limb salvage after bone tumor resection of distal radius, the free vascularised fibular graft with fibular head is an ideal substitute.

[Vascularization in transplantation of bio-derived bone compounded with marrow stromal stem cells in repair of goat tibial shaft defect].

OBJECTIVE: To study the vascularization of the composite of bio-derived bone and marrow stromal stem cells (MSCs) in repairing goat tibial shaft defect. METHODS: Bio-derived bone was processed as scaffold material. MSCs were harvested and cultured in vitro. The multiplied and induced cells were seeded onto the scaffold to construct tissue engineered bone. A 20 mm segmental bone defect in length was made in the middle of the tibia shaft in 20 mature goats and fixed with plate. The right tibia defect was repaired by tissue engineered bone (experimental side), and the left one was repaired by scaffold material (control side). The vascularization and osteogenesis of the implants were evaluated by transparent thick slide, image analysis of the vessels, and histology with Chinese ink perfusion 2, 4, 6, and 8 weeks after operation. RESULTS: More new vessels were found in control side than in experimental side 2 and 4 weeks after implantation (P<0.05). After 8 weeks, there was no significant difference in number of vessels between two sides (P>0.05), and the implants were vascularized completely. New bone tissue was formed gradually as the time and the scaffold material degraded quickly after 6 and 8 weeks in the experimental side. However, no new bone tissue was formed and the scaffold degraded slowly in control side 8 weeks after operation. CONCLUSION: Bio-derived bone has good quality of vascularization. The ability of tissue-engineered bone to repair bone defect is better than that of bio-derived bone alone.

[Repair of extensive bone defect of distal femur with free vascularized two fibulae transplantation in adults].

OBJECTIVE: To investigate the treatment of extensive bone defect of distal femur caused by various diseases in adults. METHODS: From February 1998 to December 2002, 6 cases(aged from 19 to 37) of extensive bone defects of distal femur were treated with two free vascularized fibulae, whose defects were caused by resection of bone tumor, osteomyelitis and trauma. After the resection of distal femur and articular surface of tibia, the fibulae were transplanted and fixed with screws. And the periosteum of the two fibulae was dissected and sutured with each other. RESULTS: The average follow-up time was 3.3 years. Two free vascularized fibulae could give more support to the body and the bone union of the fibulae was possible when the periosteum was incised and sutured with each other. As time went on, both of the medullary canal reunioned to form a new canal as a whole, which would make the grafts stronger. CONCLUSION: Autograft with two free vascularized fibulae can increase the stability in treating extensive bone defect of distal femur, but the union of knee joint will make flexion and extension impossible.

[Experimental studies on procedures and properties of natural derived scaffold materials of tissue engineered bone].

To detect the properties of natural xenogeneic bone derived materials which were processed with different physical and chemical treatments, we made fully deproteinized bone(FDB), partially deproteinized bone (PDPB), partially decalcified bone(PDCB) from pig ribs. Their morphological features, constitute components and mechanical properties were examined by scanning electron microscopy, x-rays diffraction analysis, mechanical assay and so on. The results showed that FDB, PDPB and PDCB maintained natural network pore system. The ratios of calcium to phosphorus were 1.81, 1.74 and 1.50, and the protein contents were 0.01% +/- 0.02%, 22.41% +/- 0.83% and 35.75% +/- 2.12% respectively. The sequence of their mechanic strength was PDCB > PDPB > FDB. These data indicate that FDB, PDPB and PDCB possess natural network pore system. Their organic and inorganic component ratios and contents are different, so their mechanic properties are not alike. Additionally, more investigations will be necessary to detect the biocompatibility of the three different scaffold materials of natural derived bone.

[Experimental studies on histocompatibility of three bio-derived bones].

OBJECTIVE: To study the histocompatibility of three bio-derived bones. METHODS: After treatment with different physical and chemical method, three bio-derived bones, the composite fully deproteinized bone (CFDB), partially deproteinized bone (PDPB) and partially decalcified bone (PDCB) were implanted into rabbits. The toxicity, immune response and subperiosteum osteogenesis of CFDB, PDPB and PDCB were studied through gross observation, serum antibody measurement, evaluation of local cellular immune response and HE staining. RESULTS: The study showed that CFDB, PDPB and PDCB had no toxicity. They could conduct peripheral tissue to grow into them and had no harmful effect on subperiosteum osteogenesis. They could also promote cartilage and osteoid tissue derived from periosteum to calcify to new bone, and combine with the peripheral bone. The degree of immune response caused by them was in the sequence of PDCB > PDPB > CFDB. CONCLUSIONS: The three bio-derived bones, CFDB, PDPB and PDCB have good histocompatibility.
