[Use of cytochrome C in the prevention of myocardial reperfusion injury during heart valve prosthesis implantation under conditions of extracorporeal circulation].

Cytochrome C used at the postischemic period causes a rapid reestablishment of coronary circulation, hemodynamic parameters, prevents activation of lipid peroxidation in reoxygenation of the heart after prolonged total myocardial ischemia in patients with acute bacterial endocarditis in whom the prostheses of heart valves had been fulfilled. Parallel with the positive inotropic effect, Cytochrome C reduces the postloading and, as a result, transfers the cardiac muscle to a more profitable regimen. At the end of the myocardial ischemia period Cytochrome C provides rapid and effective recovery of the bioelectrical function of the heart, improves its pumping function, thus allowing the dose and duration of inotropic stimulation of the myocardium at the postischemic period to be reduced, lowers pulmonary resistance and finally leads to less postoperative intrahospital lethality from acute heart failure.

[Effect of various levels of hypothermic ischemia on ATPase activity of the functional element of the myocardium].

Activity of ATPase in functional element of myocardium (its qualitative and quantitative aspects) has been researched in normal tissue and in one with various levels of hypothermic ischemia (28-30 degrees C and 10-12 degrees C) by methods of electron microscopic histochemistry on dogs. It has been found that hypothermia of myocardium (10-12 degrees C) optimally prevents ischemic disorders of energetic processes. Reproduction activity of ATPase during reperfusion of myocardium after that hypothermia maintains contractile function of heart in proper high level.

[The effect of different temperature regimens in reperfusion on the recovery of myocardial contractile function after hypothermic cardiac ischemia].

The changes of the contractile function of the heart and cAMP content in myocardium depending on the temperature conditions of reperfusion (28 degrees C, 32 degrees C, 37 degrees C) after 90 minutes of hypothermal ischemia have been investigated on isolated hearts of rats. The comparative analysis of the investigation results has shown that, after cooling a cardial muscle to 8-12 degrees C, reperfusion with initial temperature of perfusate of 32 degrees C promotes fast restoration of independent cardiac activity, does not cause formation of reperfusion contracture, normalizes processes of synthesis and cAMP utilization in cardiomyocytes.

[Lipid peroxidation and myocardial contractile function in the post-ischemic period depending on the level of hypothermic protection of the heart].

The experiments on rats isolated hearts showed lipid peroxidation state to depend on myocardial cooling level in ischemic period. Cooling to 8-12 degrees C does not induce significant impairment in the system of lipid peroxidation/antioxidant activity, thus preventing the development of reperfusion impairment in cardiac activity restoration. Temperature decrease to 4-6 degrees C during the ischemic period results in lipid peroxidation, antioxidant cell system exhaustion and impairment of contractive myocardial function in reperfusion.

[Erythrocyte and microcirculatory functions during artificial circulation].

Erythrocyte morphology, their aggregation ability and microcirculatory pattern in the mesentery of the small intestine have been studied stepwise on 12 dogs under artificial blood circulation. It has been shown that erythrocyte lesions followed by pathological aggregation play an important role in the pathogenesis of microcirculatory failure. Prolonged artificial circulation provokes structural reorganization in erythrocytes, thus affecting both blood rheology and gas exchange.

[Effect of sodium oxybutyrate on macroergic phosphates, function and ultrastructure of the myocardium in blood loss].

Experiments on dogs with long-term hypotony caused by blood loss have demonstrated irreversible lesions in the majority of cardiomyocytes and hypodynamic condition of the heart with a high content of macroergic phosphates in the myocardium. Intravenous injection of sodium hydroxybutyrate in a dose of 180-200 mg/kg after 60 min of hypotony seems to improve energy transport and utilization in the ischemic myocardium. It increases the working capacity and power of the heart muscle, pump function of the heart and makes longer the period of ultrastructural changes in cardiomyocytes.

[Fatty acid absorption in the heart of male mongrel dogs].

Cardiac absorption of nonesterified fatty acids (NEFA) and acids making part of cholesterol and triglyceride ethers from the bloodstream of male dogs was investigated. Changes were assessed on the basis of arteriovenous difference between fatty acid content in lipid fractions of the heart's in- and outflow. The dogs on predominantly animal-fat diets showed outflow changes in the quantitative and qualitative composition of most NEFA, which provide energy substrate, and in the fatty-acid composition of potential NEFA suppliers--cholesterol and triglyceride ethers.

[Effect of sodium hydroxybutyrate on coronary hemodynamics and microcirculation during hypotension caused by blood loss].

Experiments of 34 dogs were made to study alterations in the cardio- and hemodynamics and microcirculation in the mesenterium during hypotension caused by hemorrhage after intravenous injection of sodium hydroxybutyrate and intravenous re-infusion of heparinized blood under a pressure of 80-120 mm Hg. It was established that single injection of sodium hydroxybutyrate in a dose of 180-200 mg/kg after a 60-minute decrease in the arterial pressure to 40 mm Hg increases pump function of the heart, venous blood returns to the heart, and improves microcirculation. This improvement manifests by the rise in the blood flow rate in the microvessels and in the number of functioning capillaries, and by diminution of red cell aggregation. Favourable effect of the drug is seen within the first minutes after injection and remains unchanged for one hour. Intravenous blood pumping under long-term hypotension is an effective approach to making for the lost blood in the presence of the preceding injection of sodium hydroxybutyrate.

[Morphological changes in the lungs in the early recovery period after clinical death from acute blood loss depending on the method of its compensation].

Pathomorphological changes in the lungs in the period of clinical death due to acute blood loss and within 1 1/2--2 hours of resuscitation using intraarterial, intravenous, and intraosteal routes for blood loss substitution were studied in dogs. In the period of clinical death significant changes in the lung morphology were observed (capillary plethora, sludge, leukostasis, hemorrhages, fat embolism, irregular aeration of pneumonic tissue), in the early postresuscitation period the intensity of the changes varied in relation to the method of blood loss substitution. The above-mentioned methods are compared.

[The metabolic effects of the neurotropic action of actovegin during hypoxia].

Recent data on the mechanisms of the effect of aktovegin on the brain in hypoxia are systematized. It is shown that the drug improves the transport and utilization of oxygen and glucose, activates the aerobic routes of energy metabolism and, as a result, improves the functional state of the central nervous system cells.

[The effect of cytochrome c on the myocardium during reperfusion].

Experimental and clinical trials have shown high effectiveness of cytochrome c (0.5 mg/kg) in prevention of reperfusion myocardial disorders in cardiosurgical patients. Cytochrome c inhibits development of myocardial ischemic contracture, depression of coronary circulation, activation of lipid peroxidation. It reduces myocardial consumption of oxygen and promotes earlier restoration of fatty acids metabolism under lower glucose consumption by the heart muscle. By correction of myocardial metabolism, the drug suppresses fermentemia in early reperfusion.
