[Clinico-immunological parallels in patients with focal scleroderma].

Combined immunological examination of patients with focal scleroderma included studies of the humoral and cellular immunity parameters. The findings evidence that mainly immunological disorders underlie the pathogenesis of this disease, whose various combinations form this of that immunopathological pattern. An immunological classification of focal scleroderma is suggested. Clinical and immunological parallels in patients with different immunological variants of focal scleroderma are analyzed. The results permit a conclusion on the two basic mechanisms in the pathogenesis of focal scleroderma: (1) immunodeficiency involving the effector B component and (2) an isolated hyperfunction of natural killer cells.

Purification and characterization of intestinal alkaline phosphatase from harp seal.

1. Alkaline phosphatase (EC 3.1.3.1.) from harp seal (Phagophilus groenlandicus) has been purified by concanavalin A-Sepharose chromatography to homogeneity with a specific activity of 1200-1500 units/mg of protein. 2. The mol. wt of the enzyme and its subunits were estimated as 260,000 and 70,000, respectively. By chromatofocusing the isoelectric point of this enzyme is 5.5. 3. With p-nitrophenylphosphate, pH-optimum and KM for the enzyme are 9.8 and 0.9 mM, respectively. 4. The enzyme was strongly inhibited by Sn4+, Fe3+ and Zn2+, whereas Mg2+ and Mn2+ were effective activators of the enzyme. Seal alkaline phosphatase was slightly inhibited by high concentrations of Ca2+ and Cr3+. 5. The enzyme activity reached a maximum at 55-60 degrees C. It was shown that the heat stability of seal and calf intestinal alkaline phosphatases were equal at 37 and 56 degrees C.

[Comparative characterization of the polystyrene plates used in enzyme immunoassay].

The data on the comparison of the quality of polystyrene plates manufactured in the USSR with those manufactured by a number of foreign producers are presented. These data indicate that the capacity of antibodies adsorbed on a polystyrene plate for the sorption of antigens depend on the properties of the polymer surface and vary for different plates.

[Determination of antibodies to mycobacterial antigens in tuberculosis by erythrocyte immunoadsorption with a rosette marker].

A solid-phase enzyme immunoassay system for the determination of antibodies to mycobacterial antigens, based on the method of erythrocyte immunoadsorption in microchambers for immunological reactions, has been developed. To detect antibodies specifically bound with the solid-phase antigen, the affinity rosettes of Staphylococcus aureus strain Cowan I, carrying protein A, with erythrocytes conjugated with human gamma globulin have been used. The significant correlation of the titers of 34 sera, determined by means of erythrocyte immunoadsorption, with extinction values obtained in the solid-phase enzyme immunoassay of antibodies to Mycobacterium tuberculosis has been established. The coincidence of the results in 92% of cases has been noted.

[Physico-chemical properties of alkaline phosphatase from the seal small intestine].

Alkaline phosphatase of the Greenland seal was purified to homogeneity, using immobilized concanavalin A. The specific activity of the enzyme is 1200-1500 mu/mg protein. The molecular mass of alkaline phosphatase as determined by electrophoresis performed under non-denaturating conditions is 260 kD, whereas that determined in the presence of beta-mercaptoethanol and SDS is 70 kD, which points to the tetrameric type of the seal alkaline phosphatase molecule. Using the atomic adsorption method, it was demonstrated that the phosphatase molecule contains four zinc atoms. Some physico-chemical parameters of seal alkaline phosphatase (pH-dependence, effects of temperature and cations on the enzyme activity, pI, thermal stability) were determined.

[Ultramicromethod of erythrocyte immunoadsorption in screening for somatic antigens in acute myocardial infarct].

Test-systems for the identification of somatic myoglobin antigens and fibrin-fibrinogen degradation products in the sera of patients with acute myocardial infarction have been developed and tested. A significant correlation between erythrocyte immunoadsorption technique and solid-phase immunoenzyme assay was observed, the former technique being simpler and express.

[Possibilities of using immunoenzyme test systems for determining levels of myoglobin, fibrinogen, fibrin-fibrinogen degradation products and fibronectin in the diagnosis of somatic diseases].

Clinical trials of immunoenzyme test-systems for the quantitative determination of fibronectin, fibrinogen, fibrin/fibrinogen degradation products and myoglobin have been performed on serum and plasma samples obtained from patients and healthy donors. The tests were informative and possessed diagnostic value in the following conditions: fibronectin--in pyogenic septic complications of newborns, burn infections; fibrinogen and fibrin/fibrinogen products--in thrombosis, myocardial infarction and disseminated intravascular coagulation syndrome; myoglobin--in myocardial infarction.

[Fibrinogen and fibrin degradation products in the blood of acute myocardial infarct patients at the hospital rehabilitation stage].

An enzyme immunoassay was used for a study of the time course of the content of fibrinogen degradation products (FDP) and free hemoglobin (fHg) in the blood of patients with acute myocardial infarction (AMI) during uncomplicated hospital rehabilitation. A considerable increase in the levels of FDP in the blood serum and fHg in the blood plasma of the AMI patients were noted. These levels were particularly high on the 6-12th day of rehabilitation with further fluctuations on the 22-24th day from the beginning of disease resulting from an increase of the patients' motor activity during rehabilitation which might cause the depletion of endothelial reserves of fibrinolysis activators and an increase in thrombinemia, however changes in the content of FDP and fHg in the blood were more likely associated with DIC-syndrome inherent fluctuations in the system of hemostasis. The content of FDP and fHg in the blood of AMI patients was recommended to be used as a marker of DIC-syndrome and assessment of corrective therapy.

[Comparative study of the binding of human blood plasma fibronectin with clinical strains of staphylococci].

The interaction of 62 S. aureus clinical strains and, respectively, 20 and 17 isolated S. epidermidis and S. saprophyticus strains with human blood plasma fibronectin (FN) has been studied. The specific interaction of FN with bacteria has been evaluated simultaneously by the binding of 125I with FN (method 1), the FN-mediated agglutination of staphylococci (method 2) and the character of colonies formed in 0.15% agar medium containing FN (method 3). The data obtained in this investigation indicated that all S. aureus strains under study react with FN to a different extent. When evaluating the binding of FN with bacteria, the most pronounced correlation was observed between methods 1 and 3. None of the methods used in this investigation has revealed interaction between FN and S. epidermidis and S. saprophyticus strains under study. The authors suggest that a preliminary inference on the capacity of the isolated clinical strains of staphylococci for reaction with FN may be made from the character of colonies formed in 0.15% agar medium containing FN.

[Fibronectin level of the blood plasma of patients with congenital heart defects].

Enzyme immunoassays have shown mean fibronectin (FN) level to be 241.0 +/- 15.1 micrograms/ml in normal children between 2 and 15 years of age. Similar assays in 57 patients with congenital heart diseases (CHD) before and after surgery demonstrated preoperative FN decrease in 56% of patients, that was directly related to the extent of hemodynamic disorders, while postoperative FN level depended on the range and type of surgical trauma and the presence of inflammatory or suppurative-septic complications. Within 3 postoperative days, FN drops below normal level in 95% of patients. In spite of therapeutic use of blood and blood components, FN deficiency was not compensated for until day 10 after surgical correction of the valvular defect.

[Plasma heparin precipitate as a source of fibronectin in the treatment of patients with trophic skin lesions].

A study with the help of the hard phase enzyme immunoassay has shown that during incubation in the cold of fresh and fresh-frozen donor plasma in the presence of 10 I. U./ml-30 I.U./ml of heparin over 80% of fibronectin with relation to its basal level in the same plasma sample passes to the residue (heparin precipitate). The effectiveness of local use of the preparation of plasmic heparin precipitate (fibronectin concentration of 1 to 1.5 mg/ml) for therapy of patients with heavy trophic skin lesions was substantiated.

[Dynamics of changes in the levels of myoglobin and anti-myoglobin autoantibodies in the blood serum of patients with myocardial infarction].

The mean level of myoglobin and autoantibodies to myoglobin in the blood of healthy donors was 77.57 +/- 8.17 ng/ml and 18.01 +/- 1.85 micrograms/ml respectively. The level of myoglobin in the blood of patients with primary transmural myocardial infarction was rapidly increased, reaching its maximum in 9-12 h and returning to normal in 9 days. The mean level of autoantibodies was decreased in the first 66 h and got back to normal by the 6th day of disease. In primary large focal nontransmural myocardial infarction the concentration of myoglobin in the blood of patients was also increased, reaching its maximum in 3-9 h and returning to normal by the end of the 2nd day after onset of an angina attack. A decrease in the level of autoantibodies to myoglobin was observed up to the 18th day of disease. The peculiarity of repeated large focal nontransmural myocardial infarction was a two-peak curve of changes in a MG level with maximum levels in 9-12 and 21-24 h after onset of a pain attack. Final normalization of the level of myoglobin in the blood of patients of this group occurred in 69 h. The concentration of autoantibodies to myoglobin was more than once decreased up to the 6th day of disease. The results obtained showed that groups of examinees differed in the time course of changes in the level of myoglobin and autoantibodies to myoglobin. Such differences can be used for diagnostic purposes.

[Effect of exogenous fibronectin on the healing of skin wounds as evidenced by tensiometric data].

Fibronectin (FN) solution purified from human blood plasma significantly increased the breaking strength of skin wounds in rats, when applied locally and/or intravenously. The best effect (more than 2-fold) was revealed after combined local and i. v. application of FN. Maximum stimulating activity was observed on the 3rd postoperative day, directly indicating the importance of FN for the early phases of wound healing. The data suggest the possibility of using FN preparations for the stimulation of tissue repair processes.
