[Diagnostics and treatment of traumatic myocardial infarction].

The authors describe their own clinical observations of four patients with acute chest injury, who developed myocardial infarction due to traumatic dissection of anterior interventricular artery intima and a subsequent thrombosis of the artery. The authors adduce clinicolaboratory characterization of traumatic myocardial infarction and describe early and long-term (three years) results of transcutaneous coronary operations (balloon angioplasty and stenting) performed in these patients.

[Clinical and prognostic significance of disturbed global and regional contractility of left ventricle in diphtheria myocarditis].

Examination of 159 diphtheria patients diagnosed myocarditis in 64 of them. The latter were divided into 3 groups: with mild, moderate and severe myocarditis. The patients with and without diphtheria have undergone two-dimensional echocardiography with estimation of the asinergia index (AI) and left ventricular (LV) ejection fraction. Pronounced systolic dysfunction was revealed only in severe diphtheric myocarditis (DM). AI rose in moderate and severe DM. By the degree of AI elevation it can be judged about myocarditis severity and the disease prognosis. Regional LV contractility and LV systolic dysfunction were correlated. A group of patients with severe DM was identified who had a high risk of death in akinesia of LV segments (IA > 2) and lowering of LV ejection fraction below 35%.

[The importance of an early stress test in patients with myocardial infarct and heart failure in the acute period of the disease].

The mechanisms of acute myocardial infarction depend on the site of myocardial infarction (MI). In anterior MI, the main factor is a large area of injury; in inferior MI, of importance is dysfunction of the papillary muscles and multiple lesions of the coronary artery in addition to the above factor. The site, the area of injury and the status of the coronary bed influence the rate, character and prognostic value of early load test criteria and contractile function of the left ventricle. In anterior MI, elevation of the ST segment is the most frequent criterion; in inferior MI, its depression. Their combination with other criteria for test discontinuation point to multiple lesions of the coronary artery. In patients with anterior MI, the ejection fraction of the left ventricle amounting to 40%, inadequate growth of AP, anginous pain and the T dominant elevation of the ST segment are unfavourable predictors, allowing the patients to be differentiated in accordance with the risk of postinfarction complications.

[The value of early exercise testing in patients with myocardial infarction complicated by evolving left ventricular aneurysm].

The values of an early exercise test limited by criteria for test termination were compared with coronaro- and ventriculographic measurements made 2-3 days after the test in 51 males of working age who suffered from transmural myocardial infarction complicated by local aneurysm in 12 cases and by overall left ventricular aneurysm in 39. The exercise test performed in the early periods was found to substantially increased its sensitivity. The isolated ST-dominant segment elevation in patients with local aneurysm and isolated coronary artery lesion indicated that the patients had a favourable prognosis. On the contrary, the combination of two criteria or more of a positive test in patients with global aneurysm and multivessel disease suggests that the patients with temporary disability should be followed up. The presence of T-dominant ST-segment elevation, anginal pain concurrent with an inadequate BP elevation shows that there is a high risk for postinfarction complications.

[Prognostic significance of disorders of regional left-ventricular contractility detected in the subacute period of myocardial infarction].

The predictive value of left ventricular contractility index, lesion extent, ejection fraction was examined from sector scanning data in 107 able-bodied males with primary transmural myocardial infarction (MI) complicated by left ventricular aneurysm in the acute period (in 28 patients with anterior MI) and heart failure (in 23 patients with inferior MI). The findings suggest that the severity of regional contractility disorders is associated with the location of myocardial infarction and the state of the coronary bed. The significant lesion extent and multiplicity in the coronary arteries showed a high risk for postinfarction events. On the contrary, the lack of severe regional contractility disorders and isolated coronary lesion were indicative of good prognosis.

[Effect of physical training with reference to anaerobic threshold on the indicators of exercise tolerance and restoration of work capacity in patients after myocardial infarction].

In patients who had sustained myocardial infarction including that of complicated history, the anaerobic threshold was defined to choose the optimum intensity of physical training. The latter were also examined for effects on exercise tolerance. It was ascertained that measuring physical exercise by heart rate at the level of the anaerobic threshold made it possible to choose the optimal conditions for the aerobic supply of metabolic demands during physical training. Applying physical training under aerobic conditions greatly increases exercise tolerance, prevents recurrent myocardial infarctions and plays an important role in the secondary prevention of coronary heart disease.

[Prognostic significance of the area of myocardial lesion and the status of the coronary bed during restoration of work capacity in patients with a complicated course of acute myocardial infarction].

The effect of heart failure and acute left-ventricular aneurysm on the recovery of working capacity was assessed with reference to the size of necrotic focus and the extent of coronary arterial involvement in 117 patients with acute myocardial infarction. The size of necrotic focus and magnitude of coronary arterial affection were shown to be the principal determinants of heart failure in acute myocardial infarction. The presence of heart failure and acute left-ventricular aneurysm in combination with multiple coronary-arterial lesions is an adverse predictive sign in the assessment of the severity of acute myocardial infarction and the recovery of patients' working capacity. On the contrary, the presence of a local aneurysm in combination with an isolated coronary-arterial lesion should not be regarded as an unequivocal indicator of disability.

[Value of salt loads in the evaluation of functional status of the kidneys in patients with hypertension].

A 5% NaCl solution (500.0) was administered during 40-50 min to 20 patients with essential hypertension (10 with labile and 10 with stable hypertension) and 10 normal subjects. Total sodiuresis and diuresis values over 5 post-load hours, the timing of their peaks, and the ratio of diuresis/sodiuresis rates (d/s coefficient) were examined. In hypertensive patients, particularly those with stable hypertension, the peak of sodium excretion was displaced towards the beginning of the salt load, yet salty diuresis stopped earlier, and the total five-hour sodium excretion was significantly reduced, as compared to normal subjects. The d/s coefficient showed a less steep decline, and was generally higher, as compared to that of normal subjects at all stages of the study. The findings obtained demonstrate that 5-hour monitoring of the diuresis, sodiuresis and the d/s coefficient following a salt load can detect disorders of renal function.

[Plasma prostaglandin reaction to furosemide in healthy persons and in hypertension patients consuming salt in an excessive amount].

The content of prostaglandins A, E, and F in healthy individuals and in persons with hypertensive disease who had taken salt in excess throughout life was studied by the radioimmune method. The reaction of blood prostaglandins to intravenous infusion of 40 mg of furosemid was watched. It was found that excessive intake of salt is attended with a decrease in the blood prostaglandin E content in healthy individuals and in persons suffering from hypertensive disease. The prostaglandin A content increases simultaneously, evidently for compensation, while the level of prostaglandin F is reduced. The changes in the prostaglandin content of a compensatory character disappear with the development of hypertensive disease. In healthy persons taking salt in excess, the reaction to the furosemid infusion was directed at providing maximum diuresis and natriuresis; in individuals of the control group it was aimed at arresting excess excretion of sodium and water from the body. In patients with hypertensive disease who took salt in excess, the reaction of prostaglandins to furosemid diminished and was practically absent in patients with a stable arterial pressure level.
