[Extrawound programmed vulnosynthesis in the treatment of festering wounds].

The treatment of 200 patients with granulating wound of soft tissues was analyzed. A conventional method of treatment was used in 90 patients. In the study group (110 patients) different types of adaptive-reposition devices and the method of extrawound programmed vulnosynthesis were used. This method demonstrated technical advantages compared with standard methods of treatment of festering wounds that permitted to improve the results of complex treatment.

[Evaluation of treatment efficacy of non-healing wounds and trophic ulcers of the lower extremities].

A method of objective evaluation of treatment efficacy and outcomes prognosis of plastic surgeries in patients with non-healing defects has been developed. The method is based on dynamic study of free serotonin and histamine concentration in blood obtained from the elbow vein and vein of the affected limb. Level of these biogenic amines depends on disease severity and area of affection. In 32 patients phonophoresis with hydrocortisone was used that had positive influence on healing of tissue defect. This procedure is accompanied by reduction of free serotonin and histamine concentration. If their concentrations do not decrease ulcers heal very slowly. Study of biogenic amines concentration before and during treatment permitted the authors to identify favorable and unfavorable group of patients.

[Improvement of surgical methods and techniques in the treatment of varicosities of the lower extremities].

The authors have devised a technique of consecutive performing the steps of operative interventions for varicose disease of lower extremities. The method includes extirpation after Babcock of the main trunks of the large and, if necessary, the small subcutaneous veins with the help of a spiral-shaped device; preliminary percutaneous-supracutaneous temporary ligation with kapron of sinuous lateral superficial veins; subcutaneous dissection of ligated veins between ligatures with minimal blood loss. For this purpose a phlebotom of simple construction in the shape of a clamp was used. One of its jaws was a cutting element with a point and cutting edge. The instruments proposed allow to make the technique easier, to avoid excessive traumatization of the tissues and to achieve stable obliteration of the injured veins. Recurrence of the disease was noted in 3.5% out of 145 patients with varicose disease within 1-8 years.

[Surgical treatment of a carbuncle].

The results of the treatment of 250 patients with a carbuncle of various location were analyzed retrospectively. A differential approach to choice of surgical method with consideration of the degree, phase and localization of inflammation is preferable. Early and radical surgery, antimicrobic drugs, infusion therapy make base of up-to-date and adequate treatment. In the phase of infiltration deep incision often stops the disease or promotes a decrease of the necrotic zone. In suppurations linear incisions were made only on the face for cosmetic reasons. In other areas a few large connected T-, [symbol: see text]-, [symbol: see text]- or cross-formed incisions were preferable. In 30 cases I-formed incision with two rectangular flaps was used.

[Surgical technologies in the treatment of lower extremity varicose disease].

Rational method of surgery for lower extremities varicosis are developed. Method consists of Babkok's extirpation of greater and minor (if it is necessary) saphenous veins with spiral device; preliminary transcutaneous-supracutaneous temporary caprone ligation of changed superficial veins, subcutaneous dissection of ligated veins with minimal blood loss. Original flexible spiral saphenoextractor, inflatable hemostatic device, phlebotoms of simple construction are used. These devices permit to simplify surgery, to decrease tissues trauma and to achieve good cosmetic effect. Recurrence over 1-8 years was seen in 145 operated patients.

[Clinical efficacy of using ximedon in surgical practice].

Results of clinical trial of the new pyrimidine derivative--ximedone are presented. The drug was administered per os 0.5 g x 4 times a day (0.5 h before meals). Total course dose achieved 60 g. Ximedone implementation for complex treatment of burns, trophic ulcer of lower limbs, different inflammations demonstrated positive results. Prophylaxy administration of the drug in abdominal surgery at acute appendix resulted with lower incidence of postoperative complications. High efficacy and good tolerability of the drug allows to recommend it for wide medical use.

[Prophylaxis and treatment of eventration after relaparotomy].

A method of prophylaxis and treatment of eventrations after relaparotomy using apparatus of transtissue fixation was fested clinically. The apparatus was used in 24 patients with acute abdominal surgical disease subjected to relaparotomy. The apparatus of transtissue fixation permits to shorten time of operation, to make easier the work of surgeon and assistants, to minimize damage to sutured inflamed tissues. Lethality was 16.7%. There were no eventrations.

[A preclinical study of ximedone].

Pharmacotoxicological properties of ximedone, a new pyrimidine derivative, were studied experimentally on animals of various species. In the enteral regimen the drug had a favourable effect on thermal burns and postoperative cutaneous and gastric wounds. The results enabled to recommend ximedone for the clinical practice.

[Treatment of trophic ulcers of lower extremities by use of boric acid-hydrocortisone mixture].

Cryostabilized boric-hydrocortisone aquaeous mixture was used in combined treatment of varicose trophic ulcers complicated by eczema and dermatitis in 70 patients. The gauze bandages, moistened by this mixture were used locality in preoperation period. In carrying out cutaneous plastic operation a free graft was preliminarily kept in the mixture for 5-7 min. During postoperation days the bandages were applied daily up to complete healing of the cutaneous flap, only upper layers of the bandage being changed. After the patient was discharged from the hospital the courses of boric-hydrocortisone applications were repeated 3-4 times for the first year. Bacterio-cytolodical examinations were carried out. The effectiveness of the use of the mixture was determined manifested by reduction of the duration of preoperative preparation and by improvement of longterm results.

[Saphenoextractor].

New saphenoextractor, including flexible volute-shaped conductor and string-stylet, is proposed and successfully fried. The instrument contains mobile removable handle with collect as well as a set of olives with collect grip. The conductor is leaded through the vein's lumen, than the string is introduced into its lumen and the conductor is withdrawn. The handle is fixed at one end of the string and the olive--at the other one after which the vein is extracted. Clinical experience of surgical treatment of varicose veins disease in 105 patients has shown the advantages of the proposed device over other well-known veinextractors.

[Sternal osteomyelitis].

The authors present data based on the analysis of diagnosis and treatment of 11 patients with sternal osteomyelitis which in 9 patients was of traumatic origin and in 2 was hematogenous. Early diagnosis is difficult. Clinical manifestations and roentgenography of the sternum are essential for diagnosis. In combined treatment of osteomyelitis preference is given to radical removal of osteomyelitic masses within healthy tissues with tamponade of the residual osteal cavity by local tissues.

[A method for extrafocal controllable wound closure and an adaptable repositioning device].

For quicker healing of vast long-standing wounds the authors propose a specially developed device-wound contractors accelerating processes of contraction. Experimental investigations have shown that processes of concentric contraction depend on the shape and are most typical of wounds having the appearance of extended rectangle. Promising clinical results were obtained in treatment of 17 patients with vast defects of tissues of different localization (trunk--10, femur--3, shoulder--2, head--2).

[Experimental and clinical validation of auto- and allodermoplasty using transverse dermal micrografts in deep burns].

A new method of autoallodermoplasty is proposed which is as follows: a patient's skin band is cut out 40 times less than the total area of the wound surface. The wound in the donor area is sutured tightly. The skin flap is dissected to thin transverse rectangular pieces with a special microdermatome or with a razor and then they are laid with their lateral side onto the wound and covered with the allo-skin. Experimental and clinical observations have shown that at remote terms both the anatomical and functional restitution of the integument occurred. The method is indicated for huge dermal defects of the "functionally passive" areas in patients with restricted, not big resources of the donor sites, especially in patients with high operative risk.
