Altogether 35 patients operated on for gastric carcinoma were examined. In the postoperative period, a study was made of alterations in gas exchange function of the lungs parallel with the parameters of the hemostatic system. It has been established that in the early postoperative period, arterial hypoxemia was associated with the maximum tension of the functioning of the hemocoagulation system. Administration of heparin in a dose of 20,000 Units/day for 10 days led to a decrease of the period of arterial hypoxemia and reduction of its intensity. This is related to the blockade of intravascular blood coagulation. The heparin dose administered does not influence the plasma content of antithrombin-III and blood fibrinolytic activity.
A method for the prevention of thrombotic complications with heparin was developed during examination of 30 elderly and old-aged patients who underwent operation for gastric carcinoma. It was established that the therapeutic activity of heparin in plasma is maintained for 6 hours after a single subcutaneous injection of 5,000 u. Four injections of this dose of heparin ensures its continuous therapeutic activity for 24 hours in elderly oncological patients. Heparin prevention in a daily dose of 20,000 u is safe from the standpoint of hemorrhagic complications. Under clinical conditions, control of plasma heparin activity may be accomplished by means of activated partial thromboplastin time.
The authors' and reported data have demonstrated the effect of hemocoagulation and fibrinolysis products on gas exchange pulmonary function in patients who suffered surgical intervention. The results of examining 20 patients with gastric carcinoma are provided. It has been established that even before operation the patients manifested the chronic pattern of the disseminated intravascular coagulation (DIC) syndrome, whose intensity increased in the postoperative period. The rise in the blood of soluble complexes of fibrin monomer (SCFM) and fibrin degradation products (FDP) formed during intravascular coagulation was established to play an important role in the impairment of gas exchange pulmonary function in the postoperative period in patients with the chronic pattern of the DIC syndrome. A possible mechanism by which SCFM and FDP influence pulmonary gas exchange is reviewed.
Anaesthesiological and resuscitation service is a special component of disaster medicine. It is of paramount importance in LSFA (life-supporting first aid) training of the lay public. Lack of knowledge and inadequate application of LSFA are estimated to increase the mortality rate of disaster victims by 40%. The paper discusses quantity and quality of anaesthesiological and resuscitation aid delivered immediately at the disaster scene, during triage before and after evacuation, during transportation. Much attention is devoted to pain management and infusion therapy performed at the disaster scene and during transportation.

Central hemodynamics (CH) were studied in 43 elderly (72.4 years) patients by means of the Swan-Ganz catheter. Analysis according to three main criteria--the clinical picture and the values of volume blood flow and jamming pressure in the pulmonary artery--disclosed several hemodynamic types. The "hypovolemic" CH variant was prevalent (17), the eukinetic type of CH was encountered in 8, the "hyperkinetic" type--in 7, and the "hypokinetic" type--in 2 patients. Some parameters of the system of oxygen transport were used as additional information. Patients with limited reserves of circulation were set apart. Additional preoperative management for correcting CH disorders was recommended for them. It is concluded that the approach to the appraisal of the condition and reserves of circulation in the elderly must be more scrupulous.
