[Prevalence of simple sporadic goiter in relation to other non-neoplastic thyroid pathologies in an internal medicine department].

BACKGROUND AND AIM: Thyroid pathologies, in particular goiter, are commonly found in everyday clinical practice in hospitalised patients. METHODS: The authors carried out a prospective type survey lasting 80 months in a mountain community with about 30,000 inhabitants with uniform socio-demographical characteristics. A total of 8,034 subjects (4,131 males and 3,903 females) were included in the study who were hospitalised in an Internal Medicine Department. All patients underwent screening using standard diagnostic criteria. RESULTS: Thyroid pathology was identified and classified into its various forms. There was a net prevalence of goiter which, owing to the unique features identified by the study, was defined as the sporadic form. The study demonstrated a marked prevalence in females, the familial pattern, the homogeneity of distribution throughout the area, the progression of disease with age and its evolution into immersed forms, the prevalence in both sexes in adults, the frequent use of surgery and its inherent problems, the inadequacy of treatment. CONCLUSIONS: The authors affirm that this pathology is regularly underestimated because it is not adequately identified, treated and monitored. They suggest a clinical and methodological approach based on a correct nosological definition, adequate diagnostic and prognostic stratification, an appropriate therapeutic protocol and clinical controls performed over time.

An alternative to continuous subcutaneous infusion of desferrioxamine in thalassaemic patients.

Sixteen patients with thalassaemia major were treated with subcutaneous desferrioxamine (DF) 50 mg/kg/d, 5 consecutive days a week, for 8 weeks. Every other week the total dose was administered by 12 h infusion pump or by rapid injection of the same dose (25 x 2 mg/kg) twice a day. The two methods of DF administration produced no significant differences in urinary iron excretion. No significant changes in serum ferritin levels were observed at the end of the study. Compared with continuous infusion, rapid injection is equally efficacious, does not induce serious side-effects, is better accepted by the patients, and can improve their compliance to the iron-chelating therapy.

Anaesthetic implications of temporomandibular joint disease.

The purpose of this article is to review the functional anatomy of the temporomandibular joint (TMJ) mechanism in man, to discuss the various types of TMJ dysfunction and to suggest a plan for assessment, anaesthesia and airway management when reconstructive or unrelated surgery is required. Patients with restricted mouth opening, who require critical care treatment of severe upper airway obstruction or impending ventilatory failure are a special risk group. Regional anaesthesia or the use of a laryngeal airway should be considered. When tracheal intubation is essential and mouth opening is less than 25 mm, it is unlikely that the larynx will be visualized by direct laryngoscopy. Fibreoptic intubation is then indicated. All patients with severe TMJ dysfunction require assessment by an oral surgeon.

[Iatrogenic risks in orthodontic treatment].

This first task of a dentist, regardless of his speciality, is to maintain de dentoperiodontal unit in the best of conditions and prevent possible iatrogenic damage. Particularly in orthodontic treatment, a sort of scaling of the dental crown in the debanding phase is advisable.

[Endodontic treatment of the fourth canal].

This article points out that autonomous apical outlets of the fourth canal of upper molars are much more frequent than commonly thought. This is all the more reason for maximum commitment on the dentists part to research and instrumentation.

Role of motor performance in cognitive processes of parkinsonian patients.

Sixty-seven parkinsonian patients and 44 control subjects were tested for cognitive function and motor performance, using reaction times and movement times. Parkinsonian subjects did significantly worse in tests that required visuoperceptual and perceptual motor abilities, such as Object Assembly, Block Design, and Zazzo's test. Analysis of covariance of test scores showed that Block Design and Object Assembly scores were not significantly different between patients and controls when adjusted for reaction and movement times. Also, reaction and movement times were more delayed in parkinsonian patients than in controls. These results stress the main role of motor dysfunction in visuospatial and perceptual motor impairment.

[Possibility of using B-mode echography in the early diagnosis of the carotid localization of atherosclerosis].

B-Mode ultrasonography is an important complementary tool in the early diagnosis of carotid pre-atheromatous vascular disease. The authors report their personal experience concerning 86 patients affected or not by neurological transitory or persistent symptoms, and point out two main semeiological remarks of endoluminal pathologic conditions. The findings obtained are related to the more recent physiopathological and pathological-anatomical data related the growth and the evolution of atheromatous lesions of the carotid district.

L-dopa long-term treatment in Parkinson's disease: age-related side effects.

One hundred ninety L-dopa-treated parkinsonian patients have been studied according to the age at onset and to age at last examination. The frequency of major mental disturbances was significantly higher in patients older than 60 years, whereas abnormal involuntary movements and on-off phenomenon were more frequent in patients with onset before age 60. The association of normal aging and of Parkinson's disease may be responsible for the prevalence of mental disease in older patients.
