Mechanism of smectic arrangement of montmorillonite and bentonite clay platelets incorporated in gels of poly(acrylamide) induced by the interaction with cationic surfactants.

Structure transitions, induced by the interaction with the cationic surfactant cetylpyridinium chloride in nanocomposite gels of poly(acrylamide) with incorporated suspensions of the two closely related layered clays bentonite and montmorillonite, were studied. Unexpectedly, different behaviors were revealed. X-ray diffraction measurements confirm that, due to the interaction with the surfactant, initially disordered bentonite platelets arrange into highly ordered structures incorporating alternating clay platelets and surfactant bilayers. The formation of these smectic structures also in the cross-linked polymer gels, upon addition of the surfactant, is explained by the existence of preformed, poorly ordered aggregates of the clay platelets in the suspensions before the gel formation. In the case of montmorillonite, smectic ordering of the disordered platelets in the presence of the surfactant is observed only after drying the suspensions and the clay-gel composites. Rheology studies of aqueous suspensions of the two clays, in the absence of both surfactant and gel, evidence a much higher viscosity for bentonite than for montmorillonite, suggesting smaller clay-aggregate size in the latter case. Qualitatively consistent results are obtained from optical micrographs.

Prognostic meaning of temporary clipping in patients with intracranial aneurysm.

The prognostic meaning of the routine use of the methods of temporary clipping of the afferent vessel in patients with intracranial aneurysm (Grading 0-III) was the aim of the analysis in this study. In the period 1 January, 1991-31 December 1997, 304 patients underwent surgery for non-giant intracranial aneurysm and a follow-up angiography. 157 patients were operated by routinely using the temporary clipping of the afferent vessel, whereas in 147 patients the surgical procedure was performed by traditional methods. The statistical analysis showed a significant reduction (p < 0.001) in terms of risk of surgical complications in the patients who underwent surgery with the temporary clip method compared to those operated with the traditional method, with a relative risk of such complications about three times greater in the latter. The routine use of temporary clipping offers, therefore, the possibility of a significant improvement of the surgical results, not influenced by a further involvement for the structure, due to the short application time.

Temporary clipping in the surgery of endocranial aneurysms.

The authors' experience in the routine use of temporary clipping procedures in the surgery of endocranial aneurysms is reported. To analyse the validity of such a method we compared the outcome in a series of 153 aneurysms operated according to the traditional procedure (temporary clipping of the afferent vessel only in the case of intraoperative rupture of the aneurysmatic sac) with that of a more recent series of 225 in which the procedure was applied routinely. An unsatisfactory surgical outcome was found in 12.5% and 6.6% of patients respectively, with a corresponding unfavourable outcome in 5.6% and 2.6%.

[The evaluation of the time of gastric radio-isotopic emptying in subjects with non-insulin dependent diabetes mellitus].

Abnormal gastro-intestinal motility is a well-recognized complication of autonomic neuropathy in diabetics; delayed gastric emptying is frequently documented. Various pharmacologic agents have been used to treat this complication such as cisapride. We have evaluated the effects of cisapride on gastric emptying in nine diabetic patients with autonomic neuropathy through radio-scintigraphic method. Gastric emptying diabetic patients was significantly prolonged compared control subjects (p < 0.01). In our study cisapride increased gastric emptying, but this did not reach statistical significance. We concluded that cisapride may be considered as a good alternative in cases where limited efficacy or side effects preclude the use of metoclopramide.

[The motility of the biliary tract studied with 99mTc-Br-IDA in patients with type-2 diabetes mellitus].

Autonomic neuropathy in diabetes mellitus can cause alterations of the motor function of various segments of the gastroenteric apparatus. With hepatocholangio-cholecystiscintigraphy-HIDA we have studied the motility of the biliary system in patients with diabetes mellitus type II. The research has been carried out in 29 patients with diabetic autonomic neuropathy; 12 healthy volunteers have been studied to compare the results obtained. The results showed a delay in the appearance of radionucleotide in the small intestine of diabetic subjects compared to controls with statistical significance. Moreover the diabetic subjects with a serious neuropathic injury showed increased intestinal transit time. These results match those obtained by other authors that have studied the cholecystic emptying in diabetic subjects with other methods. Consequently the biliary system is also affected by the diabetic autonomic neuropathy that can be in its turn the cause of other pathologies such as biliary lithiasis.

Structure of elsinochrome A: a perylenequinone metabolite.

trans-1,2-Diacetyl-1,2-dihydro-5,10-dihydroxy-3,7,8,12- tetramethoxybenzo[ghi]perylene-4,11-dione, C30H24O10, Mr = 544.51, orthorhombic, P2(1)2(1)2(1), Z = 4, a = 12.428 (3), b = 13.048 (3), c = 14.933 (3) A, V = 2421.5 (9) A3, Dx = 1.494, Dm (by flotation) = 1.48 g cm-3, lambda(Mo K alpha) = 0.71069 A, mu = 1.057 cm-1, F(000) = 1136, T = 293 K, R = 0.046 (2065 observed reflections). Elsinochrome A is shown to exist in the solid state as a nonplanar quinone tautomer; the pigment adopts a helical conformation, in analogy with the related cercosporin, but the perylenequinone moiety in elsinochrome A appears to be significantly less skewed.

Early postoperative angiographic findings after carotid endarterectomy.

This report reviews the early postoperative evaluation of 70 patients who underwent carotid bifurcation endarterectomy within our department (before patients' releasing). Angiography was performed by femoral catheterization or the retrograde brachial route; three times only digital venous techniques were employed. No complications due to radiological procedures occurred. Postoperative angiographic findings include asymptomatic occlusion of ICA, occlusion of ECA, the presence of mural thrombi, diffuse surface roughness, intimal flaps and kink of ICA. Dilatation of the arteriotomized segment was detected with a high rate of frequency. Postoperative vascular changes are then analyzed in relation to surgical techniques, to early postoperative clinical course and to the occurrence of late re-stenoses.

[Cystic meningioma simulating glioblastoma in x-ray computed tomography].

Seven cases of supratentorial meningiomas are reported. CT-Scan showed irregular hypo-hyperdense zones and features of intratumoral cysts. At surgery a well circumscribed tumor has always been found and easily removed "en bloc". In two cases stereotaxic biopsy was performed previously. CT-Scan findings of meningiomas with microcystic structure and/or necrotic areas may mimic a malignant glioma and lead to an incorrect presumptive diagnosis: then, both therapy and prognosis will be very different. It comes that stereotaxic biopsy is mandatory in all suspected cases of cerebral tumors, in the absence of classical angiographic patterns.

[Bowen's disease of the anus].

Bowen's disease of the anus and the perineum is undoubtedly a rare pathology. This is demonstrated by the fact that, while described by M. Bowen in 1912, only 112 cases have been reported in 1979. The differential diagnosis is more difficult than that of Bowen-like papulosis and Queyrat's erythroplasia. Ano-perineal Bowen's disease is, or may be, in a large number of cases, seen concomitantly with a primary ano-rectal or vaginal carcinoma, even if they were diagnosed after Bowen's disease.

Specific technical note concerning the preparation of distal rectal anastomoses.

The present increase of distal conolorectal anastomoses is certainly due to the desire of surgeons to conserve the natural routes of intestinal transit. With the exception of unusual, and in some cases debateable indications, such as haemorrhagic rectocolitis or Crohn's disease, most distal anastomoses involve cancer. At present, the main interest in distal rectal anastomoses is focussed on the choice of operating technique: manual or mechanical. In the opinion of the author, the decisive factor in favour of the use of Staplers was the substantial reduction of the incidence of dehiscence from 16 to 5 percent.

[Treatment of chronic cerebrovascular insufficiency and chronic obliterating arteriopathy of the lower extremities with buflomedil hydrochloride].

Research has been carried out into the effects of a new vasoactive substance, buflomedil hydrochloride, on two groups of patients suffering from cerebrovascular insufficiency and obliterating arteriopathy at the lower extremities. Ten clinical parameters were assessed in the first group of patients (insomnia, headache, vertigo, tinnitus, asthenia, shaking, changes in reflexes, anorexia, memory disturbances, problems of concentration and character disturbances); in the second group, the muscular flow of the gastrocnemius as measured by the muscular clearance of NaI131 at rest, during standard exercise conditions, during ten minutes following exercise and in the post-ischaemic phase. The results can be considered satisfactory in both groups, especially after prolonged treatment and in the early stage of the disease. Drug tolerance was very good.

[Anorectal melanoma].

This paper reviews the general concepts (pathological, therapeutic and prognostic) of melanoma in the light of recent data. Unlike cutaneous melanoma which is usually diagnosed early, ano-rectal melanoma is diagnosed late in its course, which results in a very poor prognosis. This delay in the diagnosis is also probably responsible for the failure of surgery.
