[Investigations of the influence of radiation therapy upon the course of the DNA repair in peripheric lymphocytes and tumorous tissues from patients with malignant disease (author's transl)].

The influence of radiation therapy upon the capacity of DNA repair in peripheric lymphocytes and tumorous tissues was studied in eleven patients with malignant manifestations. A comparison was made between the effects of short-term and of long-term irradiation. The individual cases presented differing results. An enhancement of DNA repair as well as a diminution was observed in peripheric lymphocytes as a response to irradiation. In tumorous tissues, on the contrary, a total inhibition of DNA repair was generally found.

[Malignant disease of the paranasal sinuses (author's transl)].

During the period 1931 to 1974 167 men and 97 women with tumours of the maxilla or of the paranasal sinuses were treated at the Department of Radiotherapy and Oncology, Lainz Hospital. The peak incidence is found in the age group 51 to 70 years. The survival rate is determined by the extent of spread of the tumour. Cases in which the malignant process is still localized in the mucosa, without infiltration of the adjacent bony structures, have a good prognosis and so have the early cases of all tumours of the paranasal sinuses (Stage T1 and T2, according to Schwab) with a 5-year survival rate of 59.3%. Of the late cases (stage T3 and T4), however, no more than 20% survive for 5 years. In sarcomas there is no difference in survival rate between early and late cases because of their natural tendency to systemic spread. The 5-year survival rate is 33% in each group. Chemotherapy of these malignant tumours with Bleomycin appears promising, with a reduction of even massive infiltrations of the osseous structures in squamous cell carcinomas. However, there is no evidence as yet of any increase in 5-year survival rate with this type of therapy.

[On the radiotherapy of the carcinoma of the tonsils (author's transl)].

In the period 1932 to 1975 469 patients were admitted with a malignoma of the tonsils. 74,9% were stage T3. Nearly 50% showed up with involvement of fixated and suspicious lymphnodes. The relation men to women was 80:20. The highest incidence was found in the age group 61-70 a. The patients were treated using Radium: interstitial implants and external radiation, later Cobalt sources and the Betatron. The over-all 5 year survival rate is 21.6% for carcinomas and 32% for sarcomas. In the case histories there are predominantly late ones because difficulties in swallowing are neglected for a long time. The prognosis for cases stage T3 is rather bad. Considering the literature our material is the greatest out of a single department.

[Uric acid balance in radiotherapy].

Administration of uricostatics within the scope of radiotherapeutic treatment is indicated in all cases of manifest primary gout; in cases of latent primary gout, uricostatics should be given especially after administration of high integral doses or in case of large tumors with distinct radiation sensitivity. The decision depends on the renal function, as an increased output of substances to be eliminated by urine may induce an attack of gout in cases of latent gout, or, otherwise, may cause renal lesions followed by acute uric acid nephropathy.

[On the radio-surgery of the carcinoma of the tongue (n = 1,000) (author's transl)].

In the period 1931 to 1976 1,116 patients were admitted to our department--1,000 were treatable. The 5 year survival rate is 26.7% out of 975 cases with follow-up. There is a marked difference between the regions of the corpus and the radix linguae which present with different symptoms and--in relation to these--have a very different prognosis. The Ca linguae and sublinguale are without symptoms--after treatment--in 29.15%, the Ca radicis linguae in 10.5% only. The method of choice was in case of circumscribed invasion radio-surgery, in case of Radix Ca or diffusion or involvement of fixated lymphnodes irradiation with external beams: Radium, Cobalt, Betatron. 20 years ago, Wasserburger, the late leader of our departement, published the results of his investigations on data of 628 patients with Ca of the tongue. The good results which are due to a methodical approach and meticulous treatment were not equalled by another method, neither irradiation alone nor surgery. To preserve the unique homogeneity of this material we stuck to this therapy which consists in the coagulation of the tumor, interstitial implants of radium-needles in the layers adjacent to the electrocoagulation and in external radiation of the regional lymphnodes and additional chemotherapy if indicated. On january, 21st., 1976, the thousandst patient with Ca of the tongue was admitted to our departement to be treated and so continues the chain of patients which were extensively documented and are followed up in coordinative work with the ENT departement in our hospital. In summary there were 1,116 cases, 116 were too progredient to be treated.

[A forty-year survey of radiation therapy of the nasopharyngeal carcinoma].

Dueing the last forty years, 295 patients with carcinoma of the nasopharynx were admitted to our department. Dur to retarded discovery, the total five-year survival rate amounts to only 15.5% of all patients admitted. In 48.5%, the first symptom was a swelling of the lymphatic ganglions at the neck. Squamous cell carcinoma with 44.2% ranges at the first place followed by retotheliosarcoma with 11% and lympho-epithelioma with 6.4%. Three periods of therapy are confronted. During the first one, brachyradium and teleradium were used; during the second, brachyradium and telecobalt; during the third period, telecobalt and high-energy electrons were employed. Most important, after all, is not the ionization technique but the stage with a 47.1% five-year survival rate in T1N0 as compared to T2N0 with 28.6% and T1N1 with 19.4%. Lympho-epitheliomas with a five-year survival rate of 38.6% offer the best prognosis in contradistinction to sarcomas with 25% and squamous cell carcinomas with 15.04%. At the moment, investigations are going on to verify if afterloading techniques using 60Co are preferable to conventional irradiation techniques.

[Therapy of penile plastic duration using radium moulages].

During the last 42 years, 1524 patients with I.p.p. were treated in our department. In 69% of the cases (173 out of 248) an improvement was observed within 13 months, following therapy with radium moulages. Through improved radiation protection this therapy can be performed without undue exposure of the testes. A 15 year old patient ist the youngest case on record. Trend-calculations show a progressive tendency of this disease in later years.

[Pulmoscintigraphic long-term observations during postoperative irradiation of the mammary region (author's transl)].

The effect of ionizing rays on the pulmonary tissue during postoperative high-voltage therapy of breast cancer was investigated by a long-term study with the help of X-rays and nuclear techniques. The results prove that--in spite of optimal irradiation techniques--a local reduction of the pulmonary perfusion takes place as well as a shift of the cardiac output into other pulmonary areas, which are not exposed to the irradiation. Hereby, the pulmonary global function shows no significant change, as other authors have already pointed out. Likewise, no alteration was seen of the simple X-ray film after the use of this irradiation technique. In the pulmonary scintigram, however, a local reduction of the pulmonary perfusion was already seen after the application of 1500 rd, which increased still more in the posttherapeutic period. There were no signs of relapses within the observation period of more than 17 months.
