[Problems in the diagnosis and treatment of solitary and multiple kidney cysts].

Over the period 1989-1994, thirty-three patients presenting solitary cysts, and 19--polycystosis undergo clinical study. A variety of examination and differential diagnostic methods are used, namely: echography, venous urography, gamma chamber scintigraphy, CAT and biochemical analysis of the cystic fluid. The listed methods contribute greatly to make exact diagnosis and undertake adequate treatment of the patients.

Biopsy, Needle↗

[Shock and acute kidney failure because of not taking fluids].

A case is presented of shock and acute renal failure due to dehydration in a woman with a psychic disease who had not taken food and fluids for a long time. The post mortem examination revealed the pathomorphologic pattern of shock with microthrombi in the inner organs, brain and leptomeninges.

Acute Kidney Injury↗

["De novo" glomerulonephritis in the transplanted kidney].

A case of an 11-year-old boy is presented who had developed a severe nephrotic syndrome with massive edema, ascites, hydrothorax (protein loss with the urine up to 19 gr/24 h) 7 months after a successful kidney transplantation from a 16 years dead donor and a successfully treated crisis of transplant rejection by a good tissue compatibility. The needle renal biopsy revealed membranous glomerulonephritis I-II histological stage with data of rejection crisis by a basic disease of vesicoureteral refluxes and chronic pyelonephritis. Treatment with heparin, dipyridamole, human albumin, diuretics, sandimun and prednisolone led to a substantial improvement--mastered nephrotic syndrome, lowered to 1 g/24 h proteinuria and normal renal function.

Biopsy, Needle↗

[Outpatient echographic follow-up of patients with Fraley's syndrome].

The follow-up of the degree and evolution of the upper hydrocalicosis and of the parenchymatous changes in patients with Fraley's syndrome is very important. All authors agree that these patients should be followed up periodically. 20 patients with Fraley's syndrome were followed up and the authors conclude that the echographic examination is a very reliable method for prophylactic medical examinations since it is informative, harmless and economically advantageous.

[Clinico-therapeutic and microbiological research on the preparation Monaspor in patients with active pyelonephritis].

The urinary antiseptic efficacy of cefsulodin (Monaspor--Ciba) was studied in 22 patients with active pyelonephritis. The results were considered very good in 81.8% of the patients with urologic infections caused by P. aeruginosa, P. mirabilis and E. Coli. The drug was well tolerated. The microbiological studies in vitro of the urinary antiseptic efficacy of Monaspor on 575 bacterial strains isolated from patients with active pyelonephritis showed that the antibiotic was most active against Pseudomonas sp., beta-streptococcus, Staph. aureus, P. mirabilis.

[Case of periodically occurring total kidney failure (a new syndrome)].

The authors present a patient, observed by them, with renal agenesis and vascular compression upon the pyelo-ureteral segment of the only kidney. Clinically that combined renal anomaly is manifested with periods of acute renal insufficiency developing not characteristically and terminating in complete restoration of the function of the only kidney. It was emphasized that a similar case with the above described complication of the renal anomaly and characteristics of the clinical course has not been described in literature as yet.

Acute Kidney Injury↗

[Direct bacteriological studies of patients with calculous pyelonephritis].

The authors report their observations on the bacteriological studies in 115 patients with chronic calculous pyelonephritis. Intraoperatively, pelvic urine and biopsic material was taken from the pyelonephritis changed areas of the kidney and directly inoculated upon enriched nutrient medium "Difco" Later, inoculations were carried out on solid nutrient media. A total of 225 bacteria of 8 species were isolated. Bacteria were present in pelvic urine or in kidney punched-out material of all patients with calculous pyelonephritis, often--in both places. In 52.2% of the cases the bacteria established in both the places were identical, and in the rest of the cases--different or association of the same with another bacterium. An attempt was made to explain that phenomenon. E. coli, Pr. mirabilis and Staph. aureus were most frequently isolated. Consideration given to the observations, the authors recommend active uroantiseptic therapy in case of clinic of exacerbated chronic calculous pyelonephritis evan in case of sterile urine, as well as not very strict observation of the requirements of antibiogram with bacteriuria established. The experience of the inclusion of antistaphylococcal treatment is indicated in case of a picture of urosepsis in patients with chronic calculous pyelonephritis even in the absence of the isolated Staph. aureus from urine.

Adult↗

[Heterolateral renal dystopia (2 cases)].

The authors observed two cases with crossed renal dystopia, to which venous urography, renal scintigraphy, echographic and gamma-chamber investigations were performed. The venous urography, in case of the appropriate symptomatics, is stressed to be able to establish the presence of heterolateral dystopia by as far as the distributional function of the anomaly is concerned--the gamma-chamber investigation is with the highest information value.

Adult↗

[Gamma-chamber control of patients with Fraley syndrome].

Forty patients with Fraley syndrome were examined via "radioisotopic method for sector investigation of renal function" and the patients were grouped into 5 groups according to the results obtained. It was stressed that via the method used in patients with Fraley syndrome, an effective control could be carried out of the urodynamic disturbances and their effect on the vitality of the respective nephron population; the information obtained objectivizes the pathological deviations hence modelling the therapeutic behaviour.

Adolescent↗

[Vascular impression in the renal calices and pyelonephritis (Fraley syndrome)].

In the clinical roentgenological investigation of 46 patients with Fraley syndrome, it was stressed that the syndrome is evolutive, with clinical significance and often combined with pyelonephritis. It was underlined that in case of that syndrome, complex clinical, laboratory, X-ray and radio-isotope investigations are necessary for the precision of the diagnosis and the therapeutic behaviour.

Adolescent↗

[Vascular impressions on the renal cavitary system].

The clinical significance of forty vascular impressions confirmed at pyelographies and angiographies and based on clinical material is discussed. The vascular impressions upon the cavity system of the kidney is pointed to lead to urostasis with complications, being the sequelae of it. A scheme of their own is presented by the authors for the morbid disorders ethiopathogenetically conditioned by the vascular impressions on the cavity system of the kidney.

Diuresis↗

[Hydrocalyx (Fraley's syndrome)].

Fraley syndrome was observed in 14 cases out of 137 vessel compressions, i.e.--compression of the cranial calix neck, disturbances in urine transport, hydrocalix development with consequent complications. One case with Fraley syndrome is presented and analysis of the clinical progress of the syndrome in ten patients is carried out.
