[Diagnosis of popliteal cysts and their complications].

Popliteal cysts and their complications (rupture, expansion and pseudothrombosis) all of which may simulate deep venous thrombosis of the lower extremity may be diagnosed reliably by ultrasonography. Even small cysts can be detected, their extension can be defined and rupture with leakage of fluid into the surrounding muscles can be diagnosed. Phlebography can be avoided in most cases. Arthrography rarely gives additional information. Ultrasonography of the erect patient is recommended if rupture is suspected.

[Acyl coenzyme A dehydrogenase deficiency of medium-chain fatty acids in a 9-year-old boy with adymia. A rare mitochondrial cytopathy which may be more common than previously assumed].

The medium-chain acyl-CoA dehydrogenase (MCAD) deficiency of mitochondrial beta oxidation has been identified in a nine-year old boy with a very bland course and easy fatigue as the main symptom. Repeated low frequency stimulation test and EMG for excluding a myasthenia gravis, and screening for urinary organic acid excretion were helpful for the diagnosis. The EMG test at the m. trapezius by stimulation of the n. accessorius showed an extreme decrease of muscle power down to 49%. After i.v. injection of Edrophonium the loss of power of 20% was still significant, so that we could exclude a myasthenia gravis, but we had found signs of a generalised defect in cell chemistry. The diagnosis could be confirmed by a positive 3-phenylpropionic acid-test and moleculargenetic proof of the Adenine to Guanine mutation at position 985 in the MCAD cDNA (G985) with the polymerase chain reaction. The incidence of this organic aciduria is probably 1:60,000 in Germany, but with more attention to this disease and diagnosis of cases with bland courses the incidence will be higher. The MCAD-defect should be considered in the differential diagnosis of patients with Reye syndrome-like encephalopathies, non-ketotic hypoglycaemia or sudden unexpected deaths in infancy.

[Referral of elderly patients with cerebral apoplexy to nursing homes].

A retrospective investigation was undertaken to investigate which factors are of significance in recommendation of patients with cerebral apoplexy in geriatric departments to nursing homes. The investigation includes 116 patients admitted during a period of two years. Thirty-five (30%) were recommended for nursing homes. Sixty-seven (58%) remained severely handicapped (corresponding to Rankin's groups IV and V). Logistic regression analysis revealed that marital status, dementia, lack of motivation and inability to walk were associated with recommendation for nursing homes whereas sex, degree of paresis, aphasia, focal cognitive disturbances, incontinence of urine and hemianopsia did not show any association. Sex was, similarly, of no significance for the functional result of rehabilitation. On the basis of the regression analysis, the probability of recommendation to a nursing home may be calculated with knowledge of the predisposing factors in any given patient.

Obstructive disordered breathing during sleep in patients with spinal cord injury.

Little is known about respiration and sleep in spinal cord injured (SCI) patients, and yet they frequently have complaints related to sleep. Four SCI patients with various sleep complaints were evaluated with nocturnal polysomnography. All 4 had evidence of obstructive sleep apnea (disordered breathing). These findings suggest that obstructive sleep apnea may be contributing to disruptive sleep in SCI patients and may be responsible for many of their daytime symptoms.

Diurnal variation in peak expiratory flow rate among grain elevator workers.

The diurnal variation (DV) in peak expiratory flow rate (PEFR) has been studied among 132 grain elevator workers who accomplished three daily measurements of PEFR during three weeks. DV was calculated as the difference between the highest and the lowest PEFR as a percentage of the mean PEFR on each day. For the whole group the median was 5.9%. DV was higher among smokers and among workers with work related pulmonary symptoms. Analysis of variance showed that only age (p = 0.012) and smoking (p = 0.016) had a significant effect on DV. Pulmonary symptoms, total IgE, and duration of occupation had no independent impact on DV, whereas the exposure level of grain dust tended (p = 0.082) to have an independent effect. Twelve workers had an abnormally high DV (greater than 20%), of whom seven showed no signs of obstructive respiratory disease by spirometry. If only a single spirometric test had been performed the tentative diagnosis of bronchial asthma could have been missed in these seven workers.

"Pure alexia" without hemianopia or colour anomia.

We report a case of "pure alexia" without hemianopia or colour anomia, caused by a small subcortical haematoma in the dorso-lateral part of the occipital lobe, not affecting the splenium of the corpus callosum. It is argued that the reading disorder was due to a visual-verbal disconnection in spite of the fact that this typically is caused by a lesion in the medial part of the left occipital lobe including the splenium.

Storage mite allergy among grain elevator workers.

A total of 139 workers (71% of the available work force) in four grain elevator stores in Aalborg, Denmark, participated in a cross-sectional study on the prevalence of storage mite allergy among grain elevator workers. All underwent interview, prick test with a standard panel of allergens and the storage mites Acarus siro, Lepidoglyphus destructor and Tyrophagus putrescentiae, RAST to the three above-mentioned storage mites plus Glycyphagus domesticus and wheat, measurement of serum total IgE, and spirometry. Pulmonary symptoms were present among 44%, nasal and eye symptoms among 48%. Thirty-one per cent complained of work-related respiratory symptoms. Fourteen per cent of these workers had only nasal symptoms. Comparing the prick test result and the history, 6.4% had respiratory storage mite allergy, while 15.9% were sensitized to storage mites. Storage mites seem to be the most important allergen among grain elevator workers in Denmark. Faunistic examination of samples of grain and dust showed that storage mites, in particular A. siro, L. destructor and T. longior, were found in 73% of grain samples, while all dust samples contained mites.

Spontaneous pneumothorax in chronic obstructive pulmonary disease: complications, treatment and recurrences.

Data from 303 patients with 389 admissions for spontaneous pneumothorax from 1970 to 1980 at Ullevaal Hospital, Oslo, Norway, were reviewed. Spontaneous pneumothorax carried a significantly higher complication and mortality rate in patients suffering chronic obstructive pulmonary disease (COPD). Their higher median age compared to non-COPD patients contributed to this, but did not account for the increased mortality. The risk of developing wound infection and/or pneumonia was significantly higher after 7 days of chest tube treatment in both patient groups, independent of age. There was no association between recurrence rate and COPD/non-COPD, age or duration of chest tube treatment (1-7 days, 8 days or more). Complications were not more frequent after thoracotomies performed in COPD patients. Therefore operative treatment for both primary and COPD-related spontaneous pneumothorax should be considered if tube treatment is not successful after 1 week and there are no contraindications.
