[Clinical aspect of paroxysmal disorders of heart rhythm].

The clinical analysis of 265 patients with paroxysmal disorders of cardiac rhythm established that 39,6 per cent were with chronic heart diseases, 23,4 per cent--with rheumatic valvular defects, 12,1 per cent--with arterial hypertension, 10,6 per cent--with myocardiopathy with undistinguished etiology, 8,2 per cent--with other diseases, 6,1 per cent--clinically healthy. Supraventricular paroxysmal tachycardia was established in 35,8 per cent, paroxysmal auricular fibrillation--in 37,3 per cent, paroxysmal tachycardia with AV block--15,6 per cent, ventricular tachycardia--11,3 per cent. The duration of the paroxysms in supraventricular tachycardia has lasted for 6,7 years, in auricular fibrillation--3,2 years, in AT with AV block--1,2 years and in ventricular tachycardia--3,4 years. Apart from the basic disease the duration of the paroxysm and heart rate are of importance for the origination of the hemodynamic disorders.

[Early postinfarct syndrome occurring with paralysis of the recurrent nerve].

A brief literature survey is presented in the paper on the incidence, etiology and nature of postinfarction syndrome. Postinfarction syndrome was emphasized to be associated with the processes of autoallergization of organism to the products of cardiac muscle necrosis. A postinfarction syndrome is described that developed in a 32-year patient with extensive anterior transmural infarction--high temperature on the third day, continuous pain behind the sternum, pericardial friction, rash on both hands, accompanied by paresis of left n. recurrens. After the corticosteroids treatment, the symptoma abated within several days and recovery of the voice was possible after the fifteenth day.

[Rhythm and conduction disorders caused by cardiac glycosides].

Cardiac glucosides could cause almost all kinds of rhythm and conduction disorders. In 648 patients treated with cardiac glucosides, rhythm and conduction disorders were recorded in 124 patients--19.1 per cent (58 males and 60 females), aged from 17 to 76. Ventricular extrasystoles rank first according to incidence--47 per cent; ventricular bigeminy--24.3 per cent; AV block stage I--23.4 per cent, etc. From all the 124 patients registered with intoxications induced by cardiac glucosides--one patient died, very likely due to ventricular fibrillation--0.89 per cent lethality. Toxic symptoms with initial saturation with digitalis preparation is most frequently observed in patients with grave myocardial lesions as well as rheumatic activity, accompanied by cardiac decompensation and a parallel administration of potent diuretic treatment that favours hypopotassemia and glucoside intoxication.

[Prinzmetal's variant form of angina pectoris occurring with ventricular tachycardia, ventricular flutter and fibrillation].

After a brief literature survey, the author described a patient of 42, with its first stenocardia paroxysm, accompanied by tachycardia, clonic-tonic convulsion as MAS syndrome, loss of consciousness and spontaneous restoration. After 6 hours a second stenocardia paroxysm followed, when a gigantic ST segment elevation was recorded in precordial leads, appearance of paroxysmal ventricular tachycardia, ventricular fibrillation, loss of consciousness, restored via defibrillation. ST elevation faded by the 25 the minute. The changes are stressed to be most likely due to coronary spasm of r. descendens of the left coronary artery and to changes in the small myocardial vessels. The opinion that Prinzmetal stenocardia is a variant of angina pectoris is confirmed as well as that paroxysm could be with a lethal end due to ventricular fibrillation.

[Bundle-branch block depending on the heart rate].

Twenty nine patients are described with transitory bundle branch block due to medicamentosus treatment of 386 patients with paroxysmal tachycardia. The block lasted from 1 to 48 hours and faded without any drug therapy. Three of the patients followed up for about two years died suddenly (very likely rhythm death), 8 had no further paroxysmal tachycardia, 18 patients had from 8 to 12 paroxysms during that period and in only 6 (20.7%) -- a second transitory bundle branch block recorded. The transitory bundle branch block, depending on heart rate could be explained by the latently existing lesion of one of His bundle branches. That lesion could be manifested during a longlasting paroxysm, due to the highly accelerated cardiac activity, developing into a disorder in the repolarization of branches of the bundle.

[Embolic cardiac infarct].

One case with embolic cardiac infarction is described, that had developed in a patient with mitral-aortic valvular defect with auricular fibrillation and simultaneous embolic dissemination in the right middle cerebral and superior mesenteric arteries. The patient developed hypertension with the embolic incidence, explained by the increased production of catecholamines during the stress state.

[Right bundle-branch block depending on the cardiac frequency].

Two cases with paroxysmal tachycardia are described. A transitory right bundle branch block was induced, lasting from twenty to forty minutes, with the medicament controlling of the paroxysms. The transitory bundle branch block is explained by an existing lesion of one of the His bundle branches. A brief literature survey is presented and the experience in paroxysms treatment is shared.

Optimum conditions for the binding of insulin to its receptor.

The specific binding of insulin to the membranes from lactating mouse mammary gland was studied as a model of hormonereceptor type of binding. The basic ingredients of binding, the concentration of receptor protein and the concentration of labeled insulin were mainly studied. The characteristic changes in specific binding were followed, the adequate regression equation was draen and the optimum conditions of binding were established for further experiments. The expediency of applying shortened orthogonal plans, regression and analysis and graphic conture analysis were proved.
