[Specific IgE antibodies to Pityrosporum orbiculare in patients with atopic dermatitis].

By means of RAST investigations, we detected specific IgE antibodies against Pityrosporon orbiculare in the plasma of patients with atopic dermatitis. The patients suffering from the so-called head and neck dermatitis showed an average specific IgE antibody titer of RAST class 3, whereas in those with predominant involvement of the extremities we found an average antibody titer of RAST class 1.

[Current problems in expert assessment].

Problems concerning occupational dermatology usually arise before the actual expert opinion is given. Preventive measures should be taken into consideration very carefully in order to reduce the risk of an occupational dermatosis to a minimum. Further problems are accurate methods of patch testing, the formal wording of the expert opinion, and the assessment of the degree of disablement.

[Glossodynia--indication for patch testing?].

100 patients suffering from glossodynia without pathologic changes of the oral mucosa and 16 patients with suspected allergic contact stomatitis underwent patch testing using standard patch test and dental materials. The results were compared with those of patients in dental occupations suffering from hand eczema. As there is no clinical relevance of allergens in patients with glossodynia, patch testing seems to be unnecessary. On the other hand, patch test results of patients with stomatitis and dentures often suggest a causal allergy. In most of those cases, this was confirmed by complete healing after changing the denture bases, whereas in glossodynia such replacements never brought about a convincing improvement of the symptoms.

[The suitability of propylene glycol (1,2-propanediol) as an active antimicrobial adjuvant in ointments].

Propylene glycol was tested for its antimicrobial activity as well as its possible irritant effect in ointments. With regard to the antimicrobial effect, vehicles containing more than 5% propylene glycol proved to be superior to those containing glycerol. 269 volunteers with or without eczema who had been treated with 10% propylene glycol under occlusion and by means of the Duhring chamber method, showed very few irritations; there was almost none observed in patients tested with an ointment containing glycerol instead. We did not find any allergic sensitization against propylene glycol.

[Hay fever therapy with 2 non-sedating antihistaminics].

In a double blind study patients with hay fever were treated with Astemizole or Terfenadine for 3 to 4 weeks, 12 patients with Astemizole and 6 patients with Terfenadine. Doctors and patients judged Astemizole to be more effective than terfenadine. Statistical evaluation was not possible because of the small group. There was no sedation induced by either of the drugs.

[Professional hand eczema and atopy. A clinical prospective study on correlation].

683 patients with hand eczema were examined for history and clinical signs of atopy. 242 of these patients suffered from occupational eczema. In more than 50% of this latter group we saw at least 3 references to atopy. This rate exactly corresponds to one established in 441 patients without industrial eczema but with predominantly definite atopic eczema. Thus, atopy exists largely in patients with occupational eczema and in fact furthers the incidence of this disease relevant in social medicine. Early diagnosis of atopy resulting in efficient vocational guidance can diminish the number of cases of industrial eczema.

[Prospective study of the importance of constitutional parameters in the development of eczema in hairdressers and construction workers].

The considerable rate of professional hand eczema in hairdressers, especially in young ones, emphasizes the necessity of practicable methods for early diagnosis of risk and manifestation of eczema. Therefore, we investigated different constitutional parameters of the skin (hyperhidrosis, dyshidrosis and acrocyanosis of the hands, seborrhea, sebostasis, white dermographism) in a prospective study of 210 hairdressers' and 95 bricklayers' apprentices over 3 years. In addition, the alkali resistance and the permeability of the horny layers (by nitrazine yellow) were examined every year in each of the probands. Among other results dyshidrosis and sebostasis proved to be statistically relevant in the development of hand eczema, whereas no statistic causality was seen for acrocyanosis and hyperhidrosis. Risk of eczema could not be diagnosed early by alcali resistance tests. Frequent wet-work and insufficient skin protection were further factors causing eczema, especially in atopic subjects. There was also a statistically relevant correlation between a periumbilical eczema (caused by metallic buttons on jeans), indicating allergy against nickel, and pierced earlobes. The results of this prospective study show that by simple clinical methods serious risks of eczema in hairdressers and bricklayers can be detected already in the first years of work. Several practical recommendations for prevention of occupational eczema in hairdressers are given.

Generalized cutaneous calcinosis and mixed connective tissue disease.

A 49-year-old female patient with a history of fever episodes, muscular weakness, Raynaud's phenomenon, cardiac insufficiency and increasing cutaneous calcinosis over a period of 4 years is reported. Based upon clinical as well as histological and immunological findings, the disease is diagnosed as mixed connective tissue disease with generalized cutaneous calcinosis in an extent yet unpublished, to our knowledge, in this 'overlap syndrome'. The pathogenesis of cutaneous calcinosis is discussed with particular reference to the phenomenon of calciphylaxis, and some therapeutic implications are considered.

[Parodontoplethysmography. A method of objective evaluation of the course of progressive scleroderma].

Parodontoplethysmography was repeatedly performed in seven patients with systemic sclerosis over a period of six years. The data obtained exactly correlated to improvement or deterioration in the patients' condition. Regular parodontoplethysmography is recommended in order to observe the course of the disease more objectively. Some of the examined patients roentgenographically showed a widening of the periodontal space, a typical feature of systemic sclerosis.

[Erythema annulare centrifugum Darier].

The symptoms of erythema anulare centrifugum Darier are demonstrated in a 60-year-old patient. A paraneoplastic syndrome was sufficiently ruled out. The skin lesions quickly healed during treatment of an urinary tract infection.

Erythema Multiforme

[Pitfalls in occupational dermatology: airborne contact eczema caused by plants].

Two case reports are presented involving the occurrence of airborne contact dermatitis in a farmer and a gardener. In both cases, direct contact dermatitis or photoallergic dermatitis was suspected at first. Possible allergens for occupational airborne contact dermatitis in agriculture are discussed.

[Occurrence of contact allergies in atopic eczema. Relation to simultaneously detected atopic markers].

In a prospective study the risk of delayed-type hypersensitivity was investigated in 65 patients with atopic eczema (AE) and a noneczematous control group (n = 78) largely similar in age and occupational distribution. The diagnosis of AE was established according to the diagnostic criteria of Hanifin and Rajka. Patch tests were performed using the European standard series. There was no significant difference in the occurrence of contact sensitivity. Atopics showed a significantly higher incidence of reactions to nickel (atopics 28%/controls 6%/p less than 0.01), especially in females (females 29%/9%, p less than 0.05). Sensitivity to nickel could be regarded as a further minor atopic criteria. Patients with AE and simultaneously delayed type hypersensitivity showed significantly higher IgE levels (IgE means = 733 U/ml), than atopics without contact sensitivities (IgE means = 290 U/ml), but no differences in the occurrence of any major or minor criteria.
