[Phrenic nerve palsy after heart surgery. The Bichat Hospital experience].

The frequency of phrenic nerve palsy varies from 2.5 to 8.3%, according to different authors. Contact of the phrenic nerve with cold (ice or saline) seems to be the most frequent etiology. This study points out that good insulation between phrenic nerve and ice slush, used in the pericardium, results in almost complete disappearance of this complication, which is not a benign one, as emphasized by the two deaths in our series.

[Role and possibilities of pathology in the diagnosis of osteoporosis].

Iliac crest biopsy and clinical laboratory investigations are the important criteria to diagnose metabolic osteopathies and osteoporosis. The technic of iliac crest biopsy, the histological technic and the histomorphological and histometric criteria will be discussed. The necessity to compare clinical data with the histomorphology and the importance of surgical pathology, especially osteopathology, will be emphasized.

First report on the occurrence of human cases of alveolar echinococcosis in the northeast of Austria.

Since Posselt (1928) it is known, that Echinococcus multilocularis, the causative organism of alveolar echinococcosis, is prevalent in the western and southern provinces of Austria (Vorarlberg, The Tyrol, Salzburg, Carinthia, Styria). Recent distributional data confirmed the existence of these endemic areas in the mountain range of the Alps. Within the period from 1983 to 1988 human cases of obviously autochthonous alveolar echinococcosis were detected in the northeast of Austria, north of the river Danube, for the first time. This paper presents a clinical, diagnostic, therapeutical and epidemiological documentation of these three cases (2 females, 1 male), which represent the eastern most records of autochthonous E. multilocularis infections of the Central European distribution area of alveolar echinococcosis.

Reactions of surrounding tissue to the cementless hip implant Ti-6Al-4V after an implantation period of several years. Autopsy studies in three cases.

Three femoral endoprosthetic stems implanted without cement, made of Ti-6Al-4V alloy, with implantation periods of between 2 and 4 years were examined histologically in the form of undecalcified thin ground sections after having been split into segments and embedded in plastic. During the implantation period total osseointegration in the metadiaphyseal region takes place, while in the proximal area reaction forms with direct contact between metal and bone on the one hand and with an interposed membrane of connective tissue and adjacent secondary bony shell on the other must be distinguished. The osseointegration of the stem is the result of a primary healing of the bone and is due to the biocompatibility of the metal alloy and to the primary stability obtained by the press-fit method.

[Is high-density polyethylene suitable as an implant material in cement-free anchoring of hip endoprostheses? A histomorphologic study of an explanted polyethylene screw-in acetabula].

Firmly attached screw-in polyethylene acetabula which had been implanted for between 16 and 54 months were explanted after autopsies and subjected to macroscopic, radiologic and histologic examination. Metaplasias were seen around the threads and on the floor of the acetabulum. Their tendency to ossify represents an attempt at secondary stabilization. Due to the low stability of the polyethylene this causes increased wear on the floor of the acetabulum. The small defects in the polyethylene found in the threads, resembling damage done by mice, may be a sign of biodegradation. In view of the tissue reactions pointed out, the material stability of the polyethylene needs to be improved or implantation must be restricted to a very limited range of indications.

Kidney dimensions in ultrasound compared to somatometric parameters in normal children.

In 196 children aged between 4 weeks and 16 2/12 years a sonographic investigation of kidneys was performed and put into relation to somatometric data including bodyweight (BW) bodylength (BL), length of the trunk and the upper and the lower limb, breadth of thorax and pelvic, head circumference, maximum cranial length and maximum cranial breadth. None of the children had signs of renal disorder. Morphometric data, as kidney length, width, depth, parenchymal areas in longitudinal and transverse sections and the prolate ellipsoid volume (PEV) were documented. The children were divided into five groups (group 1: 2-6 months, group 2: 6-31 months, group 3: 31-99 months, group 4: 99-131 months, group 5: 131-194 months). Mean values of all parameters were calculated. For each group a multiple regression analysis was performed and a regression equation of kidney length, parenchymal areas and volume was established. This method allows an individual determination of renal size and parenchymal mass.

[Esophageal injuries caused by a swallowed foreign body. A life-threatening condition].

Report on a case in which a protracted foreign body in the esophagus (chicken bone) had perforated the wall in the cervical area and had led to premonitory bleeding and finally to massive hemorrhage. Neither before had further symptoms occurred regarding to the esophageal perforation such as mediastinitis or subcutaneous emphysema nor had the attempt been successful to picture the transmural lesion. With an appropriate history of the foreign body and relapsing gastro-intestinal hemorrhage--despite a negative endoscopic result--an esophageal lesion must be highly suspected as causing bleeding. The immediate thoracotomy for wound repair is strongly indicated.

[Efficient diagnosis of prostate carcinoma].

If rectal examination reveals conspicuous palpation findings of the prostate gland, a biopsy must be taken from the corresponding area, because there are no suitable laboratory measures that could help make up a definite diagnosis. Independently from methods, acid phosphatase as well as urine hydroxyprolin show elevated levels only in cases of metastatic formation. Lymphography results are insufficient and inaccurate. Staging lymphadenectomy is the best procedure to evaluate the lymph nodes; because of the relatively high complication rate, however, this operation is only indicated in case of curative treatment. Good results have been achieved through trans-cutaneous fine needle biopsy of the lymph nodes. In order to detect bone metastases, bone-scanning has proved to be valuable. In case of suspicious findings, X-ray procedures must follow in order to reassure the diagnosis. Topical scientific methods are discussed.

[The psoriatic bone manifestation].

Besides the known joint involvement as seen in the whole-body scan which uses 99m technetium-phosphate-complexes, skeletal scintigraphic investigations in 17 patients with psoriasis have demonstrated circumscribed or diffuse accumulative pathological concentrations of the radionuclide within the skeleton of the skull, the thyroid cartilage, the bony thorax and osseous long bones. These pathological accumulations of radionuclides of TcP-complexes which are distant from joint regions, point to proliferative phenomena in the bone in case of psoriasis especially in alterations of the collagenous metabolism.

[Histamine levels in interstitial fluid of lesional and perilesional skin in patients with chronic idiopathic urticaria (author's transl)].

In 8 patients with idiopathic chronic urticaria suction blisters were produced simultaneously on lesional and perilesional skin in the periumbilical area without application of heat. The suction pressure was 250 mm Hg, and thus the blister production lasted from 60--120 min. The blister fluid histamine was determined with a modified autoanalyzer technique developed by Siraganian. In this method the extractionof histamine, the condensation of o-phthaldial-dehyde with histamine and the measurement of the fluorochrome is automated. With the described method it is possible to measure values less than 0.5 ng/ml. Histamine levels in perilesional skin were nearly in the same range as in the periumbilical skin of 10 normal persons. The range of the normal tissue fluid histamine levels was 0--15 ng/ml and they did not differ significantly from the plasma levels. In all 8 patients with chronic urticaria the histamine values were higher in lesional than in perilesional skin. In 1 patient the lesional blister fluid histamine value reached 35.5 ng/ml. Four of the patients showed only low differences (less than 3 ng/ml).
