[3D-CT and angiography of cast preparations of pelvic vessels: demonstration of arterial blood supply of the acetabulum].

Anatomical studies of the arterial blood supply of the acetabulum were performed utilizing conventional angiography and 3D-CT on 27 hip specimens. In 16 cases the arterial arborization was filled with undiluted barium-sulfate via the internal artery and in 11 cases with a solution of resin and lead power. 3 specimens out of 30 were excluded because of extravasation using barium-sulfate in one case and incomplete filling with resin in 3 cases. Conventional angiograms were performed in all specimens and in preparations with resin/lead power additional three-dimensional reconstructions of the surface of the acetabulum and arterial cast were done with 3D-CT. 3D-CT shows precise relationship of the arteries to the acetabulum in any spatial plane and definition of vascular supply without elaborate preparation. 3D-CT is superior to non-selective conventional angiography because of better definition of arterial arborization. Filling with resin and lead powder is optimal for anatomical studies because of its reliability and stability. It permits comparative 3D-CT and conventional x-ray examination and subsequent preparation of the vessels in corrosion technique. The advantages of 3D-CT in vessels of cadaver specimens should encourage research on its viability in patients.

[MRT in the diagnosis of mandibular osteomyelitis following radiotherapy].

A prospective study was performed to assess the diagnostic potential of MRI in mandibular osteomyelitis after radiation therapy. A total of 60 MRI studies were performed in 39 patients with lymphoma and carcinoma of the head and neck. Clinically or histologically proven osteomyelitis of the mandible occurred in 10 patients depending on the radiation dose absorbed to bone; 9 of these were unequivocally diagnosed by typical MRI appearance, two cases were false positive. MRI is superior to other diagnostic methods such as conventional radiology and clinical examination. It should be performed routinely in an oncological follow-up after radiation therapy depending on the dental status and radiation dose.

Disappearance of a pituitary tumor after 15 months of treatment with CV 205-502, a new dopamine agonist.

We report the case of a 27-year-old woman with a prolactin-secreting macroadenoma of the pituitary gland who was under treatment with a new dopamine agonist, Sandoz CV 205-502. Immediately after diagnosis of a 12 x 10 mm intrasellar prolactinoma, treatment with CV 205-502 was begun at a daily dose of 0.075 mg. Under this low dose, prolactin in the serum normalized after 4 weeks, the initial hormone value being 189.9 ng/ml. After 6 months of therapy there was still a 6 x 6 mm tumor, and after 15 months of treatment the pathological process could no longer be observed with magnetic resonance imaging. In the 20th month of therapy the patient became pregnant. An ovulatory menstrual cycle had been present for a few months.

[Diagnosis of pneumatosis intestinalis coli in CT].

Pneumatosis intestinalis coli seldom causes symptoms of an acute abdomen and is rarely diagnosed in CT. The origin of the gas collections in the wall of the bowel often remains unclear. As shown in our case, pneumatosis may involve any portion of the bowel, but usually spares the rectum. Due to extreme meteorism and coprostasis it can be difficult to recognize pneumatosis radiographically. In CT the gas collections beneath the serosa are well defined, discrete, oval or round.

[Pre-exposure rabies vaccination using a new duck embryo vaccine].

An economical, highly purified and concentrated duck embryo rabies vaccine (PDEV) was investigated in relation to pre-exposure rabies prophylaxis. The vaccine was injected subcutaneously according to the then prescribed scheme for human diploid cell rabies vaccines (HDCV). Additionally, a third dose was given on day 120. Serum anti-rabies antibody levels were determined on days 0, 56, 120, und 170. On day 56 all volunteers showed an anti-rabies antibody level of at least 1.0 IU/ml, indicating that PDEV is sufficiently immunogenic to allow its use according to the pre-exposure schedules for HDCV. Some rather low titers noted on day 120 indicate that an early booster dose or a third primary immunization dose is justified, as recently recommended by the WHO.

[Arterial vascularization of the bony acetabulum].

The arterial supply of the acetabulum is studied in 30 human hip-pelvis specimens utilizing conventional angiography, 3 D-CT and maceration. 3 specimens out of 30 had to be excluded due to extravasation and incomplete filling. The arteries of the pelvis were filled with barium sulfate in 16 cases and in 11 cases with a solution of resin and lead powder. Conventional angiograms were performed in all specimens. In preparations with resin/lead powder additional CT with 3 D reconstructions of the surface of the acetabulum and arteries were done. 3 D-CTs show precise relationship of the arteries to the acetabulum. Filling with resin and lead powder is optimal for anatomical studies. It permits comparative 3 D-CT and conventional X-ray examination and subsequent preparation of the vessels in corrosion technique. The central parts of the acetabulum are vascularized by the obturator artery, the cranio-lateral part by the a. glutea superior and the ischial parts by the a. glutea inferior. The arterial supply of the acetabulum can be saved knowing the way and entry of the arteries into the bone. This is of importance for triple osteotomies of the pelvis but as well for difficult total hip replacements and revisions. The described method can be used for anatomical studies of the vessel to bone correlation in different anatomical sites.

Vaginal atrophy in the postmenopausal woman. The importance of sexual activity and hormones.

The effect of sexual activity on vaginal atrophy was investigated in a group of 52 postmenopausal women (mean age, 57 years). Subjects were divided into two groups: sexually active (intercourse frequency, three or more times monthly) and sexually inactive (intercourse frequency, less than ten times yearly). Two gynecologists examined all subjects and completed an index of vaginal atrophy that assessed six genital dimensions. Blood samples were also analyzed by radioimmunoassay for levels of circulating estrone, estradiol, androstenedione, testosterone, follicle-stimulating hormone, and luteinizing hormone (LH). As predicted, less vaginal atrophy was apparent in the sexually active women as opposed to the sexually inactive women. Further, women with less vaginal atrophy had significantly higher mean levels of androgens (androstenedione and testosterone) and gonadotropins (particularly LH). We discuss herein the implications of this study, particularly the importance of androgens in reducing atrophy and maintaining sexual interest.
