Anatomo-topographic landmarks on the lateral wall of nasal cavity used in endonasal surgery.

INTRODUCTION: A dissection of the lateral wall of nasal cavity that imitates a true endonasal operation was the design of the present study. The object of the study was by measuring the distance between a pivot landmark and important anatomical structures revealed successively during an endonasal operation to create additional highlights that will minimise the hazard of injuring vital structures. METHODS: Twenty anatomical preparations of sagittally sectioned heads from cadavers (elderly males of Bulgarian origin) were dissected to study the topographic relationships between the great paranasal cavities (maxillary, frontal, and sphenoidal sinus) and the ethmoidal labyrinth. Using punctum subnasale as a pivot landmark, the distances to crucial microendoscopic landmarks (i.e., nasolacrimal duct, uncinate process, ethmoidal bulla, base of the skull, anterior wall of the sphenoid sinus, orifice of the maxillary sinus) were measured at different angles to the horizontal plane. RESULTS: The mean distances from punctum subnasale to the natural ostium of the maxillary sinus and anterior wall of the sphenoidal sinus, measured at an angle of 30 degrees to the floor of the nasal cavity, were 48.25 +/- 0.75 and 67.105 +/- 0.794 mm, respectively. At an angle of 45 degrees, the mean distance to the nasolacrimal duct was 38.056 +/- 0.591 mm; to the uncinate process 45.25 +/- 0.57 mm; to the ethmoidal bulla 50.25 +/- 0.57 mm; and to the base of the skull 66.053 +/- 0.818 mm. Mean distance of 43.158 +/- 0.568 mm to the lacrimal sac and 60.25 +/- 0.68 mm to the base of the skull were measured at an angle of 60 degrees. CONCLUSION: The analysis of the results imply that the selected distances will contribute to a more precise application of this contemporary surgical method.

Reaction of the periosteum to subperiosteal implantation of a foreign body.

Early cellular reactions of tibial periosteum following its mechanical ablation from the bone surface were studied in white female rabbits. Sterile capron nets were implanted subperiosteally thus obtaining a model of aseptic inflammation. Seven stages in the morphological picture of the reactive process were differentiated. THe chronology, cytomorphology and cytophysiology of the stages were established by studying the cellular reactions in the reactive exfoliation field. Using this experimental setting we show the peculiarities of periosteal reaction in an intact and a broken bone.

Micromorphometric characteristics of osteons in compact bone of growing tibiae of human fetuses.

A micromorphometric study of compact bone from 150 tibiae of human fetuses of 6, 7-8 and 9 months of gestation was performed. The following variables were determined: number of Haversian canals per mm2, distance between adjacent Haversian canals, mean diameter of Haversian canals, volume density of the Haversian canals, and volume density of developing osteons. In the earlier periods of intrauterine development the compact bone displays a high volume density of osteons, a few wide bone lamellae, wide Haversian canals with high relative volume and density, and a small thickness of the bone between the canals. Late in gestation, the bone contains osteons with many lamellae, and small relative volume density. Haversian canals are narrow, comparatively sparse, with low volume density and considerable thickness of the intercanal tissue layer. The differences between late and mid-gestation bone tissue are statistically significant.

Ultrastructural features in the structure of blood capillaries of the Haversian canal.

Electron microscopy was used to study osteon capillaries of the compact tissue of human growing long bones. The capillaries were found to be of the continuous type. On the lumenal surface of endothelial cells of the arterial capillaries there were a great number of micropinocytotic vesicles as well as a lot of branching cytoplasmic processes forming a characteristic spongioid layer. Vesicles and filopodia were considerably fewer on the basal surface of the cells. The endothelial cells of the venous capillaries had a small number of micropinocytotic vesicles and single cytoplasmic projections on their lumenal and basal surfaces. The capillary wall contained a great amount of pericytes. There were a lot of micropinocytotic vesicles in the cytoplasm.

Stereological characteristics of the mesenchymal complex in the degenerative-osteogenic zone of the growth cartilage of the tibia of premature neonates.

The volume density of the so-called mesenchymal complex in the degenerative-osteogenic zone of growth cartilage from tibiae is determined by stereological methods. The mesenchymal complex in ossification includes several types of cells of mesenchymal origin (osteoblasts, osteoclasts, chondroclasts, vasothelial and haemopoetic cells) taking part in the processes of cartilage erosion, new formation, growth and shaping of bone tissue. The complex includes also capillary loops and other components of connective tissue located among the remnants of calcified cartilage matrix. In the degenerative-osteogenic zone of the proximal and distal growth cartilage of the tibia, the volume density decreases from the epiphysis to the diaphysis. From the peripheral to the central sections of the zones, there is almost no variation of volume density. The obtained data are related to the rates of the longitudinal growth of the tibia.

Change of some quantitative indices of the proximal and the distal growth cartilage of the human tibia during the period of the prenatal osteogenesis.

The zones of the tibial growth cartilage of human fetuses (6th lunar month), premature newborns (7th-8th lunar month) and mature newborns were micrometrically studied. The zone thickness, the chondrocyte number in a certain area and in the cell columns as well, and the volume density of the chondrocytes were determined. The obtained quantitative data show that the three indices undergo certain changes during the three age periods of the prenatal osteogenesis studied. They differ in the proximal and in the distal cartilages as well. The fetal tibia is found to be growing more intensively at its distal end. The quantitative processes within the growth cartilages of the tibia are more active during the earlier stages of the intrauterine period while the qualitative processes are found to be more active at the end of the intrauterine period.
