[Removal and preparation of the saphenous vein in aortocoronary bypass operations: proposed technic].

Mechanical trauma to saphenous vein harvested and prepared for coronary artery bypass grafting may results in early and late graft occlusion due to subendothelial proliferation and to accelerated atherosclerosis. The distention of the vein a high pressure to overcome spasm and to identify leaks causes most of these pathological changes. On the basis of these considerations we developed a careful technique of vein removal and a convenient system to irrigate veins at controlled and limited pressures.

[Stenosis of the common trunk of the left coronary artery: operative indications and risks].

39 patients with left main coronary artery stenosis (17% of all coronary patients) have been operated of myocardial revascularization from January 1982 to May 1983. 21 patients had right coronary artery occlusion; 17 had peripheral coronary artery stenosis, 3.58 bypass grafts/patient have been implanted; in 11 patients (28%) myocardial revascularization was thought incomplete. There have been 2 postoperative deaths (5.1%) and 3 myocardial infarctions (7.7%). Authors believe that in this group of patients operative risk is higher than in patients with 3 vessel disease (less than 2% in 1982) because, in sight of the poor short-term prognosis with conservative therapy, there is surgical indication even in the presence of peripheral stenosis. Frequent clinic and haemodynamic critical status of these patients worsen operative risk so that any technical fault may be lethal.

[Indications and technic of surgical treatment of pulmonary embolism].

Authors describe the surgical technique for pulmonary embolectomy and review the cases of the patients undergoing such operation. On the basis of the obtained results, Authors think that the unsuccessful cases are mainly due to the very deteriorated preoperative clinical state of these patients and to associated illness. Authors believe that the familiarity with this kind of procedure and the prompt decision to perform it before a further worsening of the clinical conditions have a positive influence on its results and that surgical indication should not be limited to the patients in cardiac arrest, but instead extended to those cases showing rapid deterioration of the clinical state.

[Is aortocoronary bypass indicated in patients with lesions of the common trunk of the left coronary artery incapable of complete revascularization?].

Obstructive lesions of the left common coronary trunk need surgery if our operability criteria: possibility of at least making a by-pass on the left descending anterior coronary branch and a not seriously compromised miocardial contractility exist. Of the 25 patients operated, there was a mortality of 8% and 3 intra-and post-operatory infarcts; two other patients died after some time (one for extracardiac causes). Other infarcts were not observed. 48% of the patients had incomplete riva-scolarization which after some time had an effect on the objective state of the patients but not on their survival. The authors conclude their studies by affirming that total surgical correction is indicated even on non susceptible patients on the basis of coronarography.

[Gore-Tex prostheses in arterial reconstructive procedures (author's transl)].

The early and long-term results using the polytetrafluorethylene (Gore-Tex) are reported. In 20 patients, 12 ilio-femoral bypasses, 10 femoro-popliteal and 2 aorto-subclavian bypasses have been performed; 5 of these patients underwent simultaneously femoro-popliteal bypasses with Gore-Tex and ilio-femoral bypasses with Dacron. Only once the Gore-Tex was used as patch. The results have been satisfactory. However, longer follow-up and many more patients are needed to determine if the Gore-Tex will be the substitute for the autogenous saphenous vein in peripheral vascular reconstruction.

[Prostheses reimplantation with porcine xenograft. Our experience (author's transl)].

The Authors explain the experience about the porcine xenograft, the results of the application of 98 bio-prostheses implanted between January 1976 and November 1979. The distribution of the implantations is: 52 mitral valve reimplantation; 16 aortic valve reimplantation, 2 tricuspid valve reimplantation, 26 xenograft were implanted with the association of another prosthesis or aorto coronary by-pass graft. The patients were selected for clinical criteria (age, sex, geographic provenance) and haemodinamic criteria. Three early embolism were checked, one lethal. The total mortality is of 9 patients: 4 patients were submitted to multiple surgical operation. The running of porcine xenograft is good on the middle term.

[Our experience with mitral valve surgery (author's transl)].

Our experience is based on 323 surgical operations on the mitral valve. Prosthetic valves were substituted in 308 patients and in 15 an anulo-valvulo-plastic operation. Mitral valve operation was associated with the correction of other heart lesions in 132 patients and in 197 patients, (objects of this study) surgery was carried out on the left atrio-ventricular valve. A detail report of the types of lesions found is made: 78 patients had massive calcification and 15 had a process of acute and subacute endocarditis. Mechanical prosthesis (Starr-Edwards 51; Björk-Shiley 77) were used 128 times and biologic prosthesis (Hancock 44; Carpentier-Edwards 7; Angell-Shiley) 54 times. Preference was given to the use of porcine prosthesis in patients presenting generic or specific controindications to anticoagulant therapy. Total mortality rate was 9,9% and in the last two years this fell to 5%. In our opinion, the sharp fall in mortality could be attributed to refined technic, better selection of cases, better myocardial protection consequent to the clinical use of cardioplegia and hypothermia.

[Congenital arteriovenous fistula between the subclavian artery and vein. (Discussion of a surgically treated case) (author's transl)].

The Authors report a case of a congenital arteriovenous fistula between the subclavian artery and vein in a 42 years-old woman. The clinical examinations are discussed for the exact classification and the topographic localisation. The surgical treatment (extirpation of the fistula) is necessary especially because arteriovenous fistulas have a tendency to progress, which may lead to severe cardiac insufficiency, aneurismatic dilatations and risks of endocarditis.

[On 23 cases of postinfartual interventricular septal rupture surgically treate. Immediate e long term results (author's transl)].

An experimental study on myocardial viability after acute occlusion of the left anterior descending coronary artery (LAD) for period of six hours is described. For this study 24 adult dogs were used in which an acute myocardial infarction for an area of 20-30% of left ventricul were obtained with ligatures completely occluding the LAD and his collaterals. Following ligature release, on the LAD only, ECG, BP, VCP and histology were studied with the general clinical conditions. The Authors affirm that clinical and ECGraphic data became worse after reperfusion; instead histological data show the possibility of recovery of many muscolar cells of ischemic and neighbouring areas.

[Experimental revascularization of acute myocardial infarction. II: Activity of various oxidoreductive tissutal and serum enzymes (author's transl)].

The behaviour of some enzymatic activities, such as monoamino oxidase (MAO), diamino oxidase (DAO), catalase, peroxidase and creatin chinase (CPK) have been studied both in blood serum and myocardial tissue of acute infarcted dogs (obtained by coronary occlusion). The most significant results are the changes of the DAO activity (--50% from the control) and peroxidase activity (+60%), 6 hours after acute ischemia. The effect of reperfusion was studied 2 hours later. A recovery of DAO activities was shown, while the peroxidase activities stayed elevated. All the enzymatic activities studied were evaluated in the serum, under the same experimental conditions. An increase of all these activities was observed until 6th hour of coronary occlusion. The reperfusion of acute ischemia, after six hours, causes a further increase of CPK and MAO activities and a decrease of catalase peroxidase and particulary evident DAO activities. The results of this experiment show that reoxygenation, under our experimental conditions, increases a further enzymatic release and in part causes a metabolic recovery of heart muscle.

Experimental revascularization of acute myocardial infarction. III: Correlation between the behaviour of the free nucleotide acid solubles and some aliphatic biogenic amines.

Free nucleotide, polyamine and nucleic acid synthesis was studied in myocardial acute ischemia and reperfusion. An early stimulation of biosynthesis of these compounds was observed during ischemia, than later, a remarkable decrease of their specific radioactivity appeared. Reperfusion experiments cause a significant increase of nucleic acids and related compound biosynthesis. The results indicate that the biochemical mechanism of myocardial protein synthesis, under our experimental conditions are not irreversibly damaged by ischemia. In fact, the reperfusion causes a remarkable recovery of nucleic acid, free nucleotide and polyamine specific radioactivity. The changes during the period of infarction and subsequent reperfusion show that polyamines play a key role in myocardial ischemia and reperfusion.
