Synovitis of the knee after intraarticular fracture fixation with Biofix. Report of two cases.

We report two cases of severe aseptic synovitis of the knee 8 and 13 weeks after biodegradable internal fixation (Biofix rods) of a fracture of the intercondylar eminence. Both knees were treated by surgical revision and synovectomy. Histologic examination revealed a severe foreign-body type of reactive synovitis in the absence of infection. It is not advisable to use Biofix intraarticularly.

Femoral shaft fracture after hip arthroplasty.

Femoral shaft fractures after hip arthroplasties were treated in 74 noncemented hemiarthroplasties and 65 cemented arthroplasties. In loose prostheses the best clinical results and the least number of operations were achieved with revision arthroplasty with a long-stem prosthesis, combined with simple internal fixation methods when applicable. In firmly fixed prostheses the results of revision arthroplasty and traction treatment were similar. Cemented revision arthroplasty did not interfere with fracture union. Internal fixation with the prosthesis in situ cannot be recommended because of a large number of secondary revision arthroplasties and nonunions. Removal of the femoral stem prosthesis and internal fixation nearly always require a secondary revision and cannot be recommended.

Epidemiology of proximal humeral fractures.

In an urban population of half a million, all proximal humeral fractures were recorded in 1983. A total of 565 fractures, of which 77 per cent occurred in women, were seen. The overall incidence per 100,000 was 48 in men and 142 in women, with an exponential increase from the 5th decade of age. This rise was due to a higher incidence of minimally displaced fractures associated with moderate trauma, and thus of fractures associated with osteoporosis.

Hemi-arthroplasty of the hip followed by ipsilateral fracture of the femoral shaft.

In a series of 74 ipsilateral fractures of the femoral shaft in relation to hemi-arthroplasties, treatment by a cemented long-stem total hip replacement was found to be superior to conservative treatment or internal fixation without removal of the prosthesis. Acceptable clinical results were obtained in 89 per cent of these cases.

Radiation treatment of giant-cell tumour of bone (osteoclastoma).

Ten patients with giant cell tumours of the bone were treated by radiation therapy. Indications were non-radical primary surgery in 8 cases and recurring lesions in 2. Tumour doses ranged from 23 to 75 Gy delivered with supervoltage equipment in 8 cases. There were 3 recurrences after radiation therapy, all occurring in patients with tumor doses less than 39 Gy; 2 of these had received orthovoltage therapy and 2 died later from lung metastases. Modern supervoltage irradiation is probably as effective as surgery which is nevertheless recommended in operable cases.
