[Chondropathia patellae--preoperative and postoperative demonstration of the patellar structure in plain roentgenography (author's transl)].

The study is based on 86 patients surgically treated for chondromalacia patellae, who in a 2 year follow-up were controlled by défilé-radiographs and clinical examination. While there was an improvement of all clinical symptoms examined, no significant change in the radiologic appearance of the patellar structure was observed as compared with the pre-operative status. This includes that neither significant changes indicating development of arthrosis nor aggravation of pre-existing arthrotic lesions could be found. However, it is commonly suggested from roentgenographic findings with type 3 lesions, that severe chondromalacia patellae may be precursor of osteoarthrosis.

[Sports injuries of the cervical spine (author's transl)].

The article reports on injuries of the cervical spine occurring during sports activities. An attempt is made to reconstruct the movements which led to the cervical spine injuries in question. In two cases of accidents occurring during bathing, one football accident and a toboggan accident, the injuries concerned point to hyperextension of the cervical spine as cause of the injury. In another football accident and a riding accident, the changes observed allow us to conclude that the movement leading to the injury must have been a hyperflexion. One accident occurring while jumping on the trampoline resulted in an injury of the upper cervical spine pointing to the action of a compressive force on the cervical spine in addition to the force resulting in hyperflexion.

[CT imaging of traumatic lesions of the vertebral column (author's transl)].

The article reports on 30 patients with traumatogenic lesions of the vertebral column examined by computerised tomography. The pros and cons of CT of the spine are discussed. Since, in the authors' opinion, the advantages of CT of the spinal column are greater than the drawbacks, it is claimed that CT is justified especially in polytraumatised patients and in unstable fractures of the vertebrae. As a rule, CT of the vertebral column can replace conventional x-ray tomography and myelography in polytraumatised patients.

[Roentgenological methods for measuring the spine (author's transl)].

Roentgenological measuring methods applicable to the vertebral column are reviewed on the basis of the roentgenological and orthopaedic literature, and described in the form of sketches and instructions regarding their application. The measuring methods reviewed in this article are used by radiologists and clinicians to assess the extent and severity of diseases of the spine, as well as to keep a watch on the course of such diseases.

[On the perforation of central venous catheters; radiological aspects of diagnosis and prevention (author's transl)].

Three cases of cardiac tamponade resulting from perforation of central venous catheters are presented. Purpose of the case reports and of the brief literature review is to derive criterions for X-ray examination of the catheter position since only routine roentgenographic control can prevent the occurrence of catheterization -- induced complications significantly. The catheter tip should not exceed the right sternoclavicular border by more than two centimeters. Roentgenograms of catheters inserted via the brachial venous route have to be obtained with the arm abducted 90 degrees. On adequately exposed Roentgen films the vertebral column should be clearly discernible within the cardiac shadow. Injection of radiopaque dye through the catheter helps to locate its tip and, in addition, confirms the diagnosis of catheter perforation and concomitant pericardial tamponade.

[X-ray diagnosis of Crohn's disease in the region of the colon (author's transl)].

The article reviews the catalogue of signs visualized on x-ray film in colitis granulomatosa Crohn, underlining the typical changes such as segmental involvement, right-sided colitis, preferential involvement of that part of the wall of the colon which is close to the mesentery, pronounced tendency to fistula formation, as well as the various forms of mucosal lesions. Attention is drawn to the possibility of roentgenologic differentiation between colitis granulomatosa Crohn and colitis ulcerosa. The individual signs of both diseases are compared with each other in tabular form.

Predictive value of radionuclide methods in the diagnosis of unilateral renovascular hypertension.

The validity of noninvasive (iodine-131 iodohippurate renogram, iodine-131 ortho-iodohippurate clearance, indium-113m EDTA--technetium-99m DTPA sequential renal scan) and invasive (xenon-133 washout) radionuclide screening tests was evaluated in the diagnosis of 105 patients with unilateral renovascular hypertension (RVH) and in 45 patients with essential hypertension (EH). In RVH positive findings on the stenosed side were noted in 73% of renograms, 73% of o-iodohippurate-clearance tests (N = 22), 81% of sequential renal scans, and 90% of xenon-washout studies (N = 67). In a subgroup of 55 retrospectively selected patients with normal or improved blood pressure following renovascular surgery, the preoperative findings had been positive on the stenosed side in 78% of renograms, 75% of o-iodohippurate-clearance tests (n = 20), 85% of sequential renal scans, and 93% of xenon-washout studies (n = 23). The sequential renal scan appears to be a sufficiently reliable method in noninvasive screening for unilateral RVH, although invasive xenon-washout studies show a higher percentage of hemodynamic alterations in the stenosed kidney. o-iodohippurate clearance tests, and in particular xenon-washout studies, can reveal arteriosclerotic lesions in the contralateral, non-stenosed kidney, which may be of importance when the decision for renovascular surgery is pending.

[Tuberous cortical irregularity at the distal femoral metaphysis--an often malignancy pretending radiological finding (author's transl)].

Four juvenile patients are described who were referred to us because of a tuberous, irregularly demarcated cortical lesion at the dorsal surface of the distal femur metaphysis suspected to be a "malignant tumor". The histologic evaluation in three of these patients revealed a fibroplastic reaction of the periosteum with periosteal new bone formation and focal, partly also lacunar bone resorption by osteoclasts. During six months of observation there were no signs of further growing. No therapy was necessary. This tuberous cortical defect at the dorsal surface of the distal femur metaphysis has been found almost exclusively in children and juvenile persons. As causative agent an increased muscle power of the gastrocnemius muscles has been discussed at the site of its bone insertion. If the bone formative component prevails a tuberous proliferation is evident radiographically, however, if the fibroplastic reaction is predominant there is more evidence for a cortical defect. The resemblance of the tuberous cortical defects in the four patients discussed here with those cases described in the literature under various names is pointed out.

[Progressive pulmonary fibrosis during combination chemotherapy with BCNU (author's transl)].

In two patients with acute leukaemia, the development of progressive interstitial pulmonary fibrosis was observed following chemotherapy with BCNU, Cytoxan and Ara-C. The x-ray changes were accompanied by restrictive changes of pulmonary function and, later on, by severe hypoxia. Serologic tests did not reveal infection with cytomegaly virus or mycoplasma pneumoniae. These findings, together with reports in the literature, suggest a toxic effect of BCNU on the lung. The combination with Cyclophosphamide may contribute to this toxic reaction.

[Progressive pulmonary fibrosis due to combined treatment with BCNU, cyclophosphospahmide and cytosin-arabinoside (author's transl)].

Two patients treated for acute leukaemia with BCNU, cyclophosphamide and cytosin-arabinoside are reported, in whom pulmonary fibrosis developed and progressed during therapy. The development of lung fibrosis during combined treatment, together with serological exclusion of other diseases known to be associated with pulmonary fibrosis, make a causal connection between the treatment and the fibrosis very probable.

[Thorium-induced neoplasia of the spleen? (author's transl)].

A patient is presented in whom multiple lienal circular foci are seen 25 years after application of a thorium-containing contrast medium for vascular visualization, over and above the well-known characteristic ThO2 deposits in the liver and spleen. The article discusses the problem whether these circular foci which meet the angiographic criteria of an angioblastic sarcoma, should be considered as a thiorium-induced neoplasia.

[Long-term radiological and clinical observations following surgery for tibio-fibular syndesmosis after fractures of the upper ankle joint (author's transl)].

Forty-two patients treated surgically for tibio-fibular syndesmosis were observed radiologically and clinically for a period of five years; the lesions followed fracture-dislocations type B, type C and following intra-articular fractures. Surgery consisted, as usual, of bone reconstruction and, where necessary, of transfixation of the syndesmosis with a horizontal screw introduced 2 cm. above the syndesmosis at a 30 degree angle to the frontal plane (15 cases). A new classification of the severity of subsequent arthrosis, as seen radiologically, was introduced and its correlation with clinical findings were determined. There was good agreement between the degree of arthrosis and functional results. The earliest signs of posttraumatic arthrosis following an incompletely treated lesion can be seen within the first year following the injury. An arthrosis of severity O due to sclerosis is of no, or only little, significance; it does not share the poor prognosis of severity grades I to III which, once they have developed, always show further deterioration.
